【摘要】：目的：觀察腕踝針結(jié)合針刺華佗夾脊穴治療腰椎間盤突出癥的臨床療效,為該治療方案的臨床運(yùn)用提供科學(xué)的試驗(yàn)依據(jù)。方法：采用隨機(jī)對(duì)照研究方法,選取60例符合納入、排除標(biāo)準(zhǔn)的腰椎間盤突出癥患者,隨機(jī)分為試驗(yàn)組和對(duì)照組,每組30例。試驗(yàn)組采取腕踝針結(jié)合針刺華佗夾脊穴治療,對(duì)照組采取常規(guī)針刺法治療。兩組均隔天治療1次,10次為1個(gè)療程,共治療1個(gè)療程。分別于治療前后采用視覺模擬評(píng)分法(VAS)及JOA下腰痛量表評(píng)分法進(jìn)行測(cè)評(píng),觀察、分析、判斷腕踝針結(jié)合針刺華佗夾脊穴治療腰椎間盤突出癥的臨床療效。結(jié)果：1.基線比較,兩組在年齡、性別、病程、VAS評(píng)分及JOA評(píng)分上比較無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),提示組間具有可比性。2.總體療效,兩組總體臨床療效比較,無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)；進(jìn)一步分析,試驗(yàn)組總有效率為86.67%,對(duì)照組為82.76%,經(jīng)比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)；兩組顯愈率分別為66.67%和41.38%,經(jīng)比較有統(tǒng)計(jì)學(xué)差異(P0.01),提示試驗(yàn)組的痊愈顯效率高于對(duì)照組。3. VAS、JOA評(píng)分變化比較,試驗(yàn)組和對(duì)照組VAS評(píng)分治療前后組內(nèi)比較,治療后評(píng)分均較治療前有顯著的降低,差異具有統(tǒng)計(jì)學(xué)意義(P0.01),治療前后試驗(yàn)組VAS差值較對(duì)照組差異明顯,經(jīng)檢驗(yàn)有統(tǒng)計(jì)學(xué)差異(P0.05)：試驗(yàn)組和對(duì)照組JOA評(píng)分治療前后組內(nèi)比較,治療后評(píng)分均較治療前明顯升高,有統(tǒng)計(jì)學(xué)差異(P0.01),治療前后JOA差值組間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：1.腕踝針結(jié)合針刺華佗夾脊穴治療腰椎間盤突出癥有確切療效,且其在提高痊愈顯效率方面較常規(guī)針刺更具優(yōu)勢(shì)。2.腕踝針結(jié)合針刺華佗夾脊穴能夠有效地改善患者的腰椎功能及疼痛癥狀,且其改善疼痛的效果優(yōu)于常規(guī)針刺。3.腕踝針結(jié)合針刺華佗夾脊穴取穴少、痛苦小、安全、療效肯定,值得臨床推廣。
[Abstract]:Objective: to observe the clinical effect of wrist-ankle acupuncture combined with acupuncture at Huatuo Jiaji point in the treatment of lumbar disc herniation. Methods: 60 patients with lumbar disc herniation were randomly divided into experimental group and control group with 30 cases in each group. The experimental group was treated with wrist-ankle acupuncture combined with acupuncture at Huatuo Jiaji, while the control group was treated with routine acupuncture. Both groups were treated once a day for 10 times as a course of treatment, a total of one course of treatment. Visual analogue scale (VASs) and JOA low back pain scale (JOA) were used before and after treatment to evaluate, observe and analyze the clinical efficacy of wrist ankle acupuncture combined with acupuncture at Huatuo Jiaji point in the treatment of lumbar disc herniation. The result is 1: 1. Baseline comparison showed that there was no significant difference in age, sex, course of disease, VAS score and JOA score between the two groups (P 0.05), indicating that there was comparability between the two groups (P < 0.05). There was no statistical difference between the two groups in overall clinical efficacy (P 0.05). The total effective rate of the test group was 86.67 and that of the control group was 82.76. There was no significant difference between the two groups (P 0.05), and the effective rates of the two groups were 66.67% and 41.38, respectively, which indicated that the effective rate of the treatment group was higher than that of the control group. The scores of VAS JOA in the experimental group and the control group before and after treatment were significantly lower than those before and after treatment, and the difference was statistically significant (P 0.01). The difference of VAS between the experimental group and the control group before and after treatment was significant. There was significant difference in the JOA scores between the test group and the control group before and after treatment, the scores were significantly higher after treatment than before and after treatment, there was a statistical difference (P 0.01). There was no significant difference in JOA difference between the two groups before and after treatment (P 0.05). Conclusion 1. Wrist ankle acupuncture combined with acupuncture at Huatuo Jiaji point has definite curative effect on lumbar disc herniation, and it has more advantages than conventional acupuncture in improving cure efficiency. 2. Wrist ankle acupuncture combined with acupuncture at Huatuo Jiaji point can effectively improve the lumbar vertebrae function and pain symptoms, and the effect of improving pain is better than that of routine acupuncture. 3. Wrist-ankle acupuncture combined with acupuncture at Huatuo Jiaji points is worth popularizing.
