【摘要】：目的:探討脫敏湯對(duì)肺脾氣虛型變應(yīng)性鼻炎的臨床療效,對(duì)患者治療有效率及癥狀改善情況進(jìn)行評(píng)價(jià),以期為脫敏湯治療變應(yīng)性鼻炎提供臨床觀察數(shù)據(jù)。方法:隨機(jī)對(duì)照方法,在兩個(gè)中心(1、西南醫(yī)科大學(xué)附屬中醫(yī)院;2、江津區(qū)中醫(yī)院)各選取肺脾氣虛型變應(yīng)性鼻炎確診病例60例為研究對(duì)象,按照數(shù)字隨機(jī)分為對(duì)照組60例與治療組60例。對(duì)照組采用口服依巴斯汀片,每天一次,每次10mg,和丙酸氟替卡松鼻噴劑,每天一次,每次50ug,連續(xù)4周以上。治療組采用孫永東教授自擬脫敏湯治療(黃芪、白術(shù)、黨參、防風(fēng)、蟬蛻、茯苓、甘草、五味子、桂枝、細(xì)辛等),每天一劑,連續(xù)4周以上。治療2周和4周后,通過卡方檢驗(yàn)比較兩組患者總有效率,以及通過視覺模擬量表(VAS)和中醫(yī)主要癥狀體征記分比較兩組患者臨床癥狀改善情況,并進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:(1)對(duì)照組總有效率為71.7%(43/60),治療組總有效率為95%(57/60)。對(duì)照組患者治療有效率顯著低于治療組,P=0.0270.05,差異有統(tǒng)計(jì)學(xué)意義。(2)主要中醫(yī)癥狀體征積分比較:兩組患者中醫(yī)癥狀體征積分都出現(xiàn)下降,2周后隨訪治療組鼻塞、打噴嚏及鼻粘膜腫脹癥狀較對(duì)照組改善明顯。4周后隨訪治療組各癥狀積分較對(duì)照組明顯下降,P0.05,差異有統(tǒng)計(jì)學(xué)意義。(3)患者VAS評(píng)分比較:在完成治療4周后進(jìn)行比較,兩組患者VAS評(píng)分都出現(xiàn)下降,但治療組VAS評(píng)分顯著低于對(duì)照組,P0.05,差異有統(tǒng)計(jì)學(xué)意義。結(jié)論:脫敏湯能顯著改善變應(yīng)性鼻炎患者臨床癥狀和體征。
[Abstract]:Objective: to investigate the clinical effect of desmin decoction on allergic rhinitis with deficiency of lung and spleen qi, and to evaluate the effective rate of treatment and the improvement of symptoms in order to provide clinical observation data for the treatment of allergic rhinitis with desmin decoction. Methods: 60 cases of allergic rhinitis with deficiency of lung and spleen qi were selected from two centers, affiliated Hospital of traditional Chinese Medicine of Southwest Medical University and Hospital of traditional Chinese Medicine in Jiang Jin District. 60 cases of allergic rhinitis of lung and spleen qi deficiency were selected as the study objects. The patients were randomly divided into control group (n = 60) and treatment group (n = 60). The control group was treated with oral Ebastine once a day, 10 mg / time, and fluticasone propionate nasal spray once a day, 50 ugs per day for more than 4 weeks. The treatment group was treated with Professor Sun Yongdong's self-made desensitization decoction (Astragalus, Atractylodes macrocephala, Codonopsis, Fangfeng, Cicada slough, Poria cocos, Licorice, Schisandra chinensis, Cinnamon, Asarum, etc., one dose per day for more than 4 weeks. After 2 weeks and 4 weeks of treatment, the total effective rate of the two groups was compared by chi-square test, and the improvement of clinical symptoms was compared by visual analogue scale (VASS) and the score of main symptoms and signs of traditional Chinese medicine (TCM), and the statistical analysis was carried out. Results the total effective rate of the control group was 71.7% and the total effective rate of the treatment group was 95 / 57 / 60. The effective rate of treatment in the control group was significantly lower than that in the treatment group (P < 0.0270.05). The difference was statistically significant. The symptoms of sneezing and swelling of nasal mucosa were improved significantly after 4 weeks compared with the control group. The scores of symptoms in the treatment group were significantly lower than those in the control group (P 0.05). The difference was statistically significant (P < 0.05). Comparison of the VAS scores of the patients: after 4 weeks of treatment, the scores were compared. The VAS score of the two groups decreased, but the VAS score in the treatment group was significantly lower than that in the control group (P 0.05). Conclusion: desmin decoction can significantly improve the clinical symptoms and signs of allergic rhinitis.
