【摘要】：目的：探討毫火針結(jié)合放血療法治療肺胃熱盛型面部脂溢性皮炎的臨床療效。方法：采用隨機(jī)對(duì)照試驗(yàn)研究方法,把符合標(biāo)準(zhǔn)的面部脂溢性皮炎患者70例,隨機(jī)分為治療組和對(duì)照組,各35例。設(shè)立毫火針結(jié)合放血療法為治療組,3天治療1次。藥物外用(地奈德乳膏)為對(duì)照組,2次／日。兩組治療周期均為兩周,并于治療后4周隨訪。觀察指標(biāo)為治療組和對(duì)照組的皮損、癥狀情況評(píng)分,于治療前、治療2周、完成治療后4周分別記錄；同時(shí)記錄治療前后患者皮膚病生活質(zhì)量量表評(píng)分,運(yùn)用相關(guān)量表評(píng)價(jià)比較,期望通過比較療效、癥狀、生活質(zhì)量改善等方面,從而探討毫火針結(jié)合放血療法治療面部脂溢性皮炎的臨床療效。結(jié)果：1.治療組與對(duì)照組的基線相比較,分布比較均衡,具有可比性。2.組內(nèi)比較：兩組治療前后癥狀總積分、各癥狀積分,皮膚病生活質(zhì)量積分及兩組治療前與治療結(jié)束后4周癥狀總積分、各癥狀積分比較有統(tǒng)計(jì)學(xué)差異(P0.01)；3.組間比較：(1)兩組治療后癥狀總積分及兩組治療結(jié)束后4周癥狀總積分比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。(2)兩組治療后各癥狀積分比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在斑丘疹、斑片面積及瘙癢方面,治療組優(yōu)于對(duì)照組,在鱗屑及糜爛滲出方面,對(duì)照組優(yōu)于治療組。(3)兩組治療結(jié)束后4周各癥狀積分除斑丘疹兩組差異無統(tǒng)計(jì)學(xué)意義(P0.05),其余癥狀比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。在斑片面積及瘙癢方面,治療組優(yōu)于對(duì)照組,在鱗屑及糜爛滲出方面,對(duì)照組優(yōu)于治療組。(4)兩組治療后皮膚病生活質(zhì)量積分比較差異有統(tǒng)計(jì)學(xué)意義(P0.01),治療組優(yōu)于對(duì)照組。(5)兩組治療后總有效率差異無統(tǒng)計(jì)學(xué)意義(P0.05)。(6)兩組治療結(jié)束4周后復(fù)發(fā)率評(píng)價(jià)差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療組優(yōu)于對(duì)照組。(7)兩組安全性及依從性均良好(P0.05)。結(jié)論：1.毫火針結(jié)合放血療法治療肺胃熱盛型面部脂溢性皮炎療效確切,與常規(guī)西藥治療的總體療效相當(dāng)。2.毫火針結(jié)合放血療法治療面部脂溢性皮炎是一種安全有效、且復(fù)發(fā)率低的非藥物治療方法。
[Abstract]:Objective: to investigate the clinical effect of the treatment of seborrheic dermatitis of lung and stomach heat. Methods: 70 patients with seborrheic dermatitis were randomly divided into treatment group and control group. The treatment group was treated once for 3 days. The drug was used as control group twice a day. The two groups were followed up for 4 weeks. The indexes of observation were skin lesions and symptom scores of the treatment group and the control group. Before treatment, 2 weeks after treatment, 4 weeks after the treatment, the score of quality of life of patients with skin diseases was recorded before and after treatment. The clinical effect of seborrheic dermatitis on facial seborrheic dermatitis was discussed by comparison of curative effect symptom and quality of life. The result is 1: 1. Compared with the baseline of the control group, the distribution of the treatment group was more balanced and comparable. 2. 2. Intra-group comparison: total symptom score, quality of life score of skin disease and total symptom score before treatment and 4 weeks after treatment were compared between the two groups, and there was significant difference between the two groups in the symptom score before and after treatment (P 0.01). There was no significant difference between the two groups in the total symptom score after treatment and the total symptom score at 4 weeks after treatment. The treatment group was superior to the control group in terms of macular papules, macular area and pruritus, and was superior to the control group in scales and erosion and exudation. The control group was superior to the treatment group (P < 0.05). There was no significant difference between the two groups in every symptom score except macular papules at 4 weeks after treatment, but there were significant differences in the other symptoms between the two groups (P 0.05). In the area of spot area and pruritus, the treatment group was superior to the control group in scale and erosive exudation, Control group is better than treatment group.) there is significant difference in quality of life score of dermatosis between two groups after treatment (P 0.01), treatment group is superior to control group. 5) there is no significant difference in total effective rate after treatment in two groups (P 0.05, P 0.05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05). There was significant difference in the recurrence rate between the two groups (P 0.05). The safety and compliance of the treatment group was better than that of the control group (P 0.05). Conclusion 1. The curative effect of acupuncture combined with bloodletting therapy on seborrheic dermatitis of face with lung and stomach heat is exact, which is equivalent to the general curative effect of routine western medicine. 2. It is safe and effective to treat seborrheic dermatitis of facial seborrheic dermatitis.
