【摘要】：目的:分析某中醫(yī)院2013-2016中藥注射劑臨床應(yīng)用情況,并選取丹紅注射液作為典型藥物,對(duì)其治療糖尿病性冠心病的臨床療效進(jìn)行Meta分析,為臨床制定合理治療決策提供參考。方法:根據(jù)中醫(yī)藥辨證理論對(duì)某中醫(yī)院常用中藥注射劑進(jìn)行藥理分類(lèi)。統(tǒng)計(jì)分析某中醫(yī)院所用中藥注射劑銷(xiāo)售額、數(shù)量、用藥頻度(DDDs)、日用藥金額(DDC)以及中藥注射劑銷(xiāo)售金額占全院藥品銷(xiāo)售總金額的百分比。利用計(jì)算機(jī)數(shù)據(jù)庫(kù)檢索文獻(xiàn)并篩選,對(duì)納入符合研究標(biāo)準(zhǔn)的文獻(xiàn)進(jìn)行方法學(xué)質(zhì)量評(píng)價(jià)后,應(yīng)用RveMan5.3軟件,采用隨機(jī)效應(yīng)模型對(duì)納入文獻(xiàn)進(jìn)行Meta分析。結(jié)果:2013-2016年,某中醫(yī)院中藥注射劑品種數(shù)分別為31、32、33、33,中藥注射劑銷(xiāo)售額和中藥銷(xiāo)售總金額以及藥品銷(xiāo)售總金額三年內(nèi)分別增長(zhǎng)了23.85%和16.76%和10.43%。丹紅注射液治療糖尿病合并冠心病Meta分析共納入10篇文獻(xiàn),在心絞痛緩解總有效率和顯效率方面,丹紅注射液+常規(guī)療法VS常規(guī)療法異質(zhì)性結(jié)果分別為,P=0.900.1,I2=0%,OR[95%CI]為5.06[3.29,7.79]和P=0.800.05,I2=0%,綜合結(jié)果優(yōu)勢(shì)比(Odds Ratio)OR[95%CI]為2.20[1.63,2.96],差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在心電圖改善總有效率和顯效率方面,丹紅注射液+常規(guī)療法VS常規(guī)療法分別為OR[95%CI]為2.33[1.52,3.56](P0.05)和OR[95%CI]為1.82[1.23,2.70](P0.05)。結(jié)論:該中醫(yī)院中藥注射劑應(yīng)用呈逐年上升趨勢(shì),且應(yīng)用在合理范圍。丹紅注射液聯(lián)合常規(guī)治療在心絞痛緩解和心電圖改善方面均優(yōu)于常規(guī)治療。
[Abstract]:Objective: to analyze the clinical application of traditional Chinese medicine injection in a certain Chinese medicine hospital 2013-2016, and select Danhong injection as a typical drug to analyze the clinical effect of traditional Chinese medicine injection in treating diabetic coronary heart disease by Meta, and to provide reference for making rational treatment decision in clinic. Methods: according to the theory of TCM syndrome differentiation, the pharmacological classification of traditional Chinese medicine injection in a Chinese medicine hospital was carried out. The sales volume, quantity, frequency of drug use, daily amount of DDCand the percentage of sales amount of Chinese medicine injection to the total amount of drug sales in a Chinese medicine hospital were analyzed statistically. The literature was searched and screened by computer database. After evaluating the methodological quality of the literature that met the research standard, the Meta analysis was carried out by using the RveMan5.3 software and the random effect model. Results in 2013-2016, the number of Chinese traditional medicine injections in a certain Chinese medicine hospital was 31 ~ 32X ~ (33), respectively. The sales of traditional Chinese medicine injection, the total amount of Chinese medicine sales and the total sum of sales of Chinese medicine increased 23.85%, 16.76% and 10.43% respectively in three years. The Meta analysis of Danhong injection in the treatment of diabetes mellitus with coronary heart disease was included in 10 articles. The total effective rate and effective rate of angina pectoris relief were analyzed. The heterogeneity of vs conventional therapy in Danhong injection was 5.06 [3.297.79] and 0.800.05% (95%CI), respectively. The heterogeneity of conventional therapy vs conventional therapy of Danhong injection was 2.20 [95%CI] 2.20 [1.632.96], the difference was statistically significant. In improving the total effective rate and effective rate of electrocardiogram, the OR [95%CI] and OR [95%CI] of Danhong injection conventional therapy vs were 2.33 [1.52 鹵3.56] and 1.82 [1.232.70] respectively. Conclusion: the application of traditional Chinese medicine injection in this hospital is increasing year by year and applied in a reasonable range. Danhong injection combined with conventional therapy is superior to conventional therapy in angina pectoris relief and ECG improvement.
