【摘要】：目的:觀察散痹湯加減治療寒濕痹阻型腰椎間盤突出癥的療效及安全性。方法:將192例符合條件的患者隨機(jī)分為中藥組、西藥組和中西藥組,各64例。中藥組給予散痹湯加減治療,西藥組給予依托考昔片治療,中西藥組同時(shí)給予散痹湯加減結(jié)合依托考昔片治療,療程均為28 d。觀察各組治療前后改良日本骨科協(xié)會(huì)腰疼痛評(píng)分量表(M-JOA),Oswestry功能障礙指數(shù)問卷評(píng)分量表(ODI),視覺模擬評(píng)分法(VAS)和中醫(yī)辨證寒濕痹阻型腰椎間盤突出癥中醫(yī)證候評(píng)分;檢測(cè)治療前后檢測(cè)各組血清腫瘤壞死因子-α(TNF-α),白細(xì)胞介素-1β(1L-1β),C-反應(yīng)蛋白(CRP)和超氧化物歧化酶(SOD)的變化;比較各組有效率和不良事件發(fā)生率。結(jié)果:治療后中西藥組總有效率95.1%,高于中藥組的79.4%和西藥組的82.3%(P0.05);中藥組與西藥組比較無明顯差異;中西藥組和中藥組中醫(yī)證候評(píng)分較西藥組改善更為明顯(P0.05);其他臨床癥狀中西藥組改善較中藥組和西藥組更為明顯(P0.05);不良事件發(fā)生率中藥組低于西藥組和中西藥。結(jié)論:散痹湯加減治療寒濕痹阻型腰椎間盤突出癥的療效與西藥依托考昔片無明顯差異,具有有效性,且不良事件的發(fā)生率較依托考昔片低;散痹湯加減結(jié)合依托考昔片治療寒濕痹阻型腰椎間盤突出癥具有協(xié)同增效作用,療效優(yōu)于單用散痹湯加減和依托考昔片。
[Abstract]:Objective: to observe the efficacy and safety of Sanbi decoction in the treatment of cold and dampness obstruction lumbar intervertebral disc herniation. Methods: 192 patients were randomly divided into Chinese medicine group, western medicine group and western medicine group with 64 cases each. The traditional Chinese medicine group was treated with Sanbi decoction, the western medicine group was treated with etacoxib, and the Chinese and western medicine group was treated with Sanbi decoction combined with traditional Chinese medicine. The course of treatment was 28 days. Before and after treatment, the modified Japanese Orthopedic Association Lumbar pain scale (MJOAA) and Oswestry dysfunction Index (Oswestry) scale, visual analogue score (VASS) and TCM syndrome score of syndrome differentiation of cold dampness obstruction type lumbar intervertebral disc herniation (TCM) were observed. Before and after treatment, the serum levels of TNF- 偽 TNF- 偽, IL-1- 尾 -1- 尾 -C- reactive protein (CRP) and superoxide dismutase (SOD) were measured, and the effective rate and incidence of adverse events in each group were compared. Results: the total effective rate of Chinese and western medicine group was 95.1%, which was higher than that of Chinese medicine group (79.4%) and western medicine group (82.3%), but there was no significant difference between traditional Chinese medicine group and western medicine group. The scores of TCM syndromes in traditional Chinese medicine group and traditional Chinese medicine group were improved more obviously than that in western medicine group, other clinical symptoms in traditional Chinese medicine group and western medicine group were improved more obviously than that in traditional Chinese medicine group and western medicine group, and the incidence of adverse events in traditional Chinese medicine group was lower than that in western medicine group and western medicine group. Conclusion: the curative effect of Sanbi decoction on the treatment of cold dampness obstruction type lumbar intervertebral disc herniation is not significantly different from that of western medicine, and it is effective, and the incidence of adverse events is lower than that of Tongcaoxi tablet. Sanbi decoction combined with relying Coxie tablet in the treatment of cold dampness obstruction of lumbar intervertebral disc herniation has synergistic effect, the curative effect is better than the single use of Sanbi decoction and relying Coxie tablet.
