【摘要】：尿酸性腎病,又稱(chēng)痛風(fēng)性腎病,是由于體內(nèi)嘌呤代謝異常,導(dǎo)致血尿酸水平升高或血尿酸排泄減少,尿酸鹽在關(guān)節(jié)周?chē)湍I間質(zhì)部位沉積,引發(fā)的一系列臨床綜合癥。臨床主要表現(xiàn)為尿酸結(jié)石、血尿、蛋白尿、浮腫,或伴有腰膝酸痛、小便清長(zhǎng)、夜尿頻多,或貧血,并常伴有關(guān)節(jié)疼痛、活動(dòng)受限等癥狀。中國(guó)古代醫(yī)家對(duì)本病并無(wú)精確的記載,但根據(jù)其癥狀表現(xiàn)及病癥特點(diǎn),歸屬中醫(yī)“痹癥”、“淋證”、“腰痛”、“虛勞”等范圍�，F(xiàn)代中醫(yī)對(duì)于本病沒(méi)有統(tǒng)一的診斷標(biāo)準(zhǔn)、療效評(píng)定標(biāo)準(zhǔn),臨床工作者僅根據(jù)自身的實(shí)踐經(jīng)驗(yàn),對(duì)本病進(jìn)行辨證論治,且取得較好的臨床療效。吾師趙剛教授臨床實(shí)踐近三十年,在本病的治療方面累積了淵博的經(jīng)驗(yàn)。導(dǎo)師認(rèn)為本病多因先天稟賦不足或久病體弱,脾腎虧虛,脾失健運(yùn),腎蒸騰氣化功能失司,水液代謝失常,濕濁毒邪內(nèi)蘊(yùn),加之平素飲食失調(diào)、七情勞倦內(nèi)傷,釀生濕熱,易感受風(fēng)、寒、濕邪,外邪侵犯肌體、經(jīng)絡(luò),寒郁化熱,釀濕成痰,流竄肢節(jié),阻滯經(jīng)絡(luò),累及腎絡(luò)而致,屬本虛標(biāo)實(shí)、虛實(shí)夾雜之證。吾師依據(jù)病情的輕重緩急和嚴(yán)重程度,對(duì)本病采用分期論治(急性發(fā)作期和穩(wěn)定期)的方法,并對(duì)各期辯證分型論治,同時(shí)重視患者平素飲食、起居、情志對(duì)疾病的影響,取得了良好的療效。
[Abstract]:Uric acid nephropathy, also known as gouty nephropathy, is a series of clinical syndrome caused by the abnormal metabolism of purine metabolism in the body, resulting in the increase of blood uric acid level or the decrease of blood uric acid excretion, and the deposition of uric acid around the joints and renal interstitium. The clinical manifestations are uric acid stone, hematuria, proteinuria, swelling, or with low back and knee pain, and the clear urine and fecal pain. The night urine frequency is more frequent, or anemia, and often accompanied by joint pain, limited activity and other symptoms. Chinese ancient doctors have no accurate records of this disease, but according to their symptoms and symptoms, belong to "arthralgia syndrome", "drenching syndrome", "low back pain", "deficiency" and so on. There is no unified diagnostic criteria and evaluation of curative effect in the present generation of Chinese medicine. According to their own practical experience, clinical workers are only treated with syndrome differentiation and treatment in accordance with their own practical experience. The clinical practice of Professor Zhao Gang, a teacher, has accumulated profound experience in the treatment of this disease for nearly thirty years. The vaporization function loses the division, the water metabolism disorder, the wet turbidity and toxic internal connotations, combined with the flat diet disorder, the seven emotional fatigue internal injuries, the moisture and heat, the cold, the damp evil, the external evil invading the body, the meridians, the cold and the heat, the dampness and the phlegm, the blockage of the meridians, the obstruction of the meridians and the kidney collaterals.
