【摘要】：目的:以毒損腎絡(luò)理論為指導(dǎo),“未病先防”、“已病防變”,對導(dǎo)師自擬方扶正通絡(luò)解毒湯治療早期糖尿病腎病之氣陰兩虛挾瘀毒證進行臨床研究,觀察其有效性及安全性,為中藥治療早期糖尿病腎病開辟新思路、提供新方法,奠定臨床基礎(chǔ)。方法:本研究是結(jié)合導(dǎo)師臨證思想,以毒邪學(xué)說和絡(luò)病理論為依據(jù),選取臨床常見的早期糖尿病腎病氣陰兩虛挾瘀毒證的患者,進行臨床觀察。將符合入選標準的60例早期糖尿病腎病患者,隨機分為治療組和對照組,每組各30例。兩組均采用常規(guī)基礎(chǔ)治療,治療組予扶正通絡(luò)解毒湯加減治療,對照組予氯沙坦鉀片加減治療,療程均為12周。觀察兩組治療前后臨床癥狀、減少尿微量白蛋白、β2微球蛋白、超敏C反應(yīng)蛋白等,以進一步評價該組方的有效性;觀察兩組患者治療前后的實驗室檢查血常規(guī)、尿常規(guī)、肝腎功能、心電圖作為安全性指標,以評價該組方的安全性。結(jié)果:治療組扶正通絡(luò)解毒湯在改善臨床癥狀,減少尿微量白蛋白、β2微球蛋白、超敏C反應(yīng)蛋白的排出等方面優(yōu)于對照組(P0.05)。臨床療效好,臨床觀察中未見毒副作用。結(jié)論:以毒損腎絡(luò)理論為指導(dǎo)組方的扶正通絡(luò)解毒湯,對腎臟起到了保護作用,能夠減少尿微量白蛋白的排出,降低炎癥反應(yīng),減輕腎臟負擔(dān),保護腎臟功能,延緩糖尿病腎病的進展,為中醫(yī)藥治療早期糖尿病腎病奠定了堅實的基礎(chǔ),值得臨床推廣與應(yīng)用。
[Abstract]:Objective: under the guidance of the theory of toxic damage to kidney and collaterals, "prevention before illness", "prevention of disease", and "prevention of disease", a clinical study was carried out on the treatment of Qi and Yin deficiency with stasis and toxin with deficiency of Qi and Yin in early Diabetic Nephropathy by his own prescription Fuzheng Tongluo jiedu decoction, and its efficacy and safety were observed. To open up new ideas and provide new methods for the treatment of early diabetic nephropathy with traditional Chinese medicine, and to lay a foundation for clinical practice. Methods: this study was based on the theory of toxic evil and collateral disease, and selected the patients with Qi and Yin deficiency of early diabetic nephropathy with blood stasis and toxin syndrome to carry on clinical observation. Sixty patients with early diabetic nephropathy were randomly divided into treatment group and control group with 30 cases in each group. The treatment group was treated with Fuzheng Tongluo jiedu decoction and the control group was treated with losartan potassium tablet for 12 weeks. The clinical symptoms were observed before and after treatment, the levels of urinary microalbumin, 尾 2 microglobulin and hypersensitive C-reactive protein were decreased in order to further evaluate the effectiveness of the group, and the blood routine and urine routine were observed before and after treatment. Liver and kidney function and electrocardiogram (ECG) were used as safety index to evaluate the safety of the prescription. Results: in the treatment group, Fuzheng Tongluo jiedu decoction was superior to the control group in improving clinical symptoms, reducing urinary microalbumin, 尾 2 microglobulin and excretion of hypersensitive C-reactive protein. The clinical effect was good, and no side effects were found in clinical observation. Conclusion: Fuzheng Tongluo jiedu decoction guided by the theory of toxic damage to the kidney can protect the kidney, reduce the excretion of urinary microalbumin, reduce the inflammatory reaction, lighten the burden of the kidney and protect the renal function. Delaying the progress of diabetic nephropathy lays a solid foundation for the treatment of early diabetic nephropathy with traditional Chinese medicine and is worthy of clinical popularization and application.
