【摘要】：頑固性腹脹是指合理應用調和肝脾、理氣除脹等治療而無明顯療效之腹脹。史載祥教授認為,在肝脾不調基礎上,頑固性腹脹尚存在脾陽不升、脾胃失和之病機,治宜升舉清陽、補中益氣、實衛(wèi)固表,方選補中益氣湯加減,小其制投藥;對瘀血留滯、絡脈瘀阻證之腹脹,宜活血化瘀、理氣行滯、通利血脈,方選膈下逐瘀湯合大黃?蟲丸;對肺失宣降、肅降不能之腹脹,宜宣肅肺氣、潤燥滑腸、行氣消脹,方選溫白丸加減;對沉寒痼冷、陽損寒凝之腹脹,宜溫通逐寒、溫補脾腎、溫里助陽,可選九轉玉壺丹;對奇經受累、陰陽俱損之腹脹,宜通補奇經、溫補陰陽、補益肝腎,方選肉蓯蓉丸加減。治療頑固性腹脹需整體辨證、變通施治。
[Abstract]:Obstinate abdominal distension is a kind of abdominal distension without obvious effect. Professor Shi Zexiang believes that on the basis of the non-regulation of the liver and the spleen, the stubborn abdominal distension still has the pathogenesis of spleen yang not rising and spleen and stomach disharmony. It is appropriate to treat the disease by raising the clear yang, supplementing the middle and supplementing the qi, the solid Wei Gu table, and selecting the prescription to supplement the middle and replenish the qi, so as to make the medicine small; and to leave the blood stasis behind. The abdominal distension of the syndrome of collaterals stasis is suitable for promoting blood circulation and removing blood stasis, regulating qi and moving stagnation, communicating blood vessels, and choosing the decoction for removing blood stasis under the diaphragm and rhubarb? Chong Pill; for the lung loss, purge can not be abdominal distension, should be Xuanzuo lung qi, dry smooth intestines, do Qi expandable, Fang Xuan Wen Bai Wan add and subtract; to Shen cold, Yang damage cold condensation of abdominal distension, should be warm by cold, warm spleen kidney, warm Li to help Yang, Can choose nine turn Yubu Dan; Qijing involved, Yin and Yang damage of abdominal distension, should be through Qi Jing, warm Yin Yang, tonifying liver and kidney, Cistanche pill plus or minus. The treatment of obstinate abdominal distension needs integral differentiation of symptoms and signs, alternative treatment.
