【摘要】：目的：通過藥艾條灸加化療與單純化療在防治腫瘤化療患者骨髓抑制、生活質(zhì)量的比較,觀察藥艾條灸法對(duì)腫瘤化療患者骨髓抑制及功能狀態(tài)的改善情況,探討藥艾條灸法是否具有減輕化療對(duì)骨髓抑制、提高患者生活質(zhì)量的作用,為藥艾條灸在腫瘤化療臨床的應(yīng)用提供依據(jù)。方法：病人來自江蘇省中醫(yī)院和江蘇省中西醫(yī)結(jié)合醫(yī)院的腫瘤科。將60例符合納入標(biāo)準(zhǔn)的腫瘤患者根據(jù)隨機(jī)數(shù)字表法分為治療組和對(duì)照組[各30例],剔除脫落病例后,治療組26例,對(duì)照組27例。兩組病人均用西醫(yī)常規(guī)化療方案,治療組在常規(guī)化療方案基礎(chǔ)上加用藥艾條灸神闕、關(guān)元和足三里,每次、每穴灸15分鐘,每周灸6次,從患者化療開始,至下次化療為1個(gè)療程[約3周],共治療3個(gè)療程。記錄化療前、三次化療后的血象、功能狀態(tài)及化療后不良反應(yīng),比較兩組治療效果的差異。使用SPSS17.0進(jìn)行觀察數(shù)據(jù)的統(tǒng)計(jì)分析。結(jié)果：1.骨髓抑制情況：1.白細(xì)胞、中性粒細(xì)胞：治療組三次化療后的白細(xì)胞改變與對(duì)照組比較都有統(tǒng)計(jì)學(xué)意義(P0.05)。第一個(gè)化療周期結(jié)束后白細(xì)胞有所下降,但第二次化療結(jié)束后,P值略有增加。治療組的中性粒細(xì)胞在第一次和第二次化療后與對(duì)照組比較有顯著統(tǒng)計(jì)學(xué)意義。但第3次化療后的P值大于0.05,無統(tǒng)計(jì)學(xué)意義。2.血紅蛋白：整個(gè)治療組的血紅蛋白變化有顯著統(tǒng)計(jì)學(xué)意義[P0.05]。但第一次化療后比化療前有所下降,而第二次化療后的P值較第一化療后略有升高,第3次化療后P值比第二次化療后略有升高。3.血小板：治療組的血小板變化與對(duì)照組比較有統(tǒng)計(jì)意義(P0.05)。然而,第2次化療后的P值較第一化療后略有升高,第3次化療后P值比第二次化療后略有升高。4.紅細(xì)胞：治療組的紅細(xì)胞在第一次化療后與對(duì)照組比較有顯著統(tǒng)計(jì)學(xué)意義(P0.05)。但第2次和3次化療后的P值大于0.05,無統(tǒng)計(jì)學(xué)意義。治療組的RBC與對(duì)照組比較,無統(tǒng)計(jì)學(xué)意義(P值較高)。2.其他副反應(yīng)：于對(duì)照組比較,治療組的疲勞等副反應(yīng)發(fā)生顯著減少,有顯著統(tǒng)計(jì)學(xué)意義(P0.05)。其他副作用,如頭暈,口干等在治療組和對(duì)照組間沒有明顯差異(P0.05)。3.功能狀態(tài)：治療組的ZPS評(píng)分與化療前比較明顯升高,有顯著統(tǒng)計(jì)學(xué)意義(P0.05)。對(duì)照組化療后比化療前明顯下降,有顯著統(tǒng)計(jì)學(xué)意義(P0.05)�；熀笾委熃M患者全身功能狀態(tài)明顯好于對(duì)照組,有顯著統(tǒng)計(jì)學(xué)意義(P0.05)結(jié)論：1.藥艾條灸能減慢白細(xì)胞、中性粒細(xì)胞、血小板、血紅蛋白下降的速度和幅度,減輕化療對(duì)骨髓的抑制、保護(hù)血細(xì)胞。2.該研究顯示藥艾條灸對(duì)紅細(xì)胞的作用不明顯。3.藥艾條灸能提高患者的全身功能狀態(tài),提高生存質(zhì)量。4.藥艾條灸是安全有效的方法[研究中無不良事件發(fā)生]。
[Abstract]:Objective: to observe the effect of moxibustion plus chemotherapy on the improvement of bone marrow suppression and functional state in patients with tumor chemotherapy by comparing the quality of life and bone marrow suppression with chemotherapy alone. To explore whether moxa moxibustion can alleviate the effect of chemotherapy on bone marrow suppression and improve the quality of life of patients, and provide the basis for clinical application of moxibustion with moxa in tumor chemotherapy. Methods: the patients were from the oncology department of Jiangsu traditional Chinese Medicine Hospital and Jiangsu Integrated Chinese and Western Medicine Hospital. According to the random digital table, 60 patients with tumor were divided into two groups: treatment group (n = 30) and control group (n = 30 each). After removing exfoliation cases, there were 26 cases in treatment group and 27 cases in control group. Two groups of patients were treated with routine western medicine chemotherapy. The treatment group was treated with moxibustion of Shenque, Guan Yuan and Zusanli on the basis of routine chemotherapy. Each time, moxibustion at acupoint for 15 minutes and moxibustion 6 times a week was used in the treatment group, starting with the patient's chemotherapy. To the next chemotherapy for a course [about 3 weeks], a total of 3 courses of treatment. The hemogram, function state and adverse reaction after chemotherapy were recorded before and after three times chemotherapy, and the difference of therapeutic effect between the two groups was compared. SPSS17.0 was used for statistical analysis of observation data. The result is 1: 1. Bone marrow suppression: 1. WBC, neutrophils: the changes of leukocytes in the treatment group after three times chemotherapy were significantly different from those in the control group (P 0.05). At the end of the first chemotherapy cycle, leukocyte decreased, but the P value increased slightly after the second chemotherapy. The neutrophils in the treatment group were significantly higher than those in the control group after the first and second chemotherapy. But after the third chemotherapy, P value was more than 0.05, no statistical significance. 2. Hemoglobin: the change of hemoglobin in the whole treatment group was statistically significant [P0.05]. However, after the first chemotherapy, the P value after the second chemotherapy was slightly higher than that after the first chemotherapy, and the P value after the third chemotherapy was slightly higher than that after the second chemotherapy. Platelet: compared with the control group, the changes of platelet in the treatment group were statistically significant (P 0.05). However, the P value after the second chemotherapy was slightly higher than that after the first chemotherapy, and the P value after the third chemotherapy was slightly higher than that after the second chemotherapy. Red blood cells: red blood cells in the treatment group after the first chemotherapy compared with the control group has significant statistical significance (P 0.05). But after the second and third chemotherapy, the P value was more than 0.05, there was no statistical significance. The RBC of the treatment group was higher than that of the control group (P < 0.05). Other side effects: compared with the control group, fatigue and other side effects in the treatment group were significantly reduced, with significant statistical significance (P 0.05). Other side effects, such as dizziness and dry mouth, had no significant difference between the treatment group and the control group (P 0.05). Functional status: the ZPS score of the treatment group was significantly higher than that before chemotherapy (P 0.05). In the control group, it was significantly lower after chemotherapy than that before chemotherapy, with significant statistical significance (P 0.05). The systemic function of the patients in the treatment group after chemotherapy was significantly better than that in the control group (P 0.05). Moxibustion could slow down the decrease of leukocyte, neutrophil, platelet and hemoglobin, reduce the inhibition of chemotherapy on bone marrow and protect blood cell. The study showed that moxibustion of medicated moxibustion had no obvious effect on erythrocyte. Medicinal moxibustion can improve the whole body function of patients, improve the quality of life. 4. Herbal moxibustion is a safe and effective method [there are no adverse events in the study].
