【摘要】：目的：觀察健脾益氣攝血法對(duì)慢性原發(fā)免疫性血小板減少癥(CITP)氣不攝血型患者的臨床療效,以及對(duì)外周血中T細(xì)胞亞群表達(dá)的影響,探討脾氣虛證與CITP的發(fā)病之間的關(guān)系,繼而深入研究健脾益氣攝血法治療CITP作用機(jī)理。方法：篩選2015-2016年黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院導(dǎo)師孫鳳教授門診就診的證屬氣不攝血型CITP患者30例,作為治療組,給予口服健脾益氣方劑進(jìn)行治療,觀察記錄入組患者治療前后血小板計(jì)數(shù),中醫(yī)證候積分、出血情況和T細(xì)胞亞群變化情況。結(jié)果：1.西醫(yī)臨床總緩解率為73.33%,治療組治療前、治療后比較血小板計(jì)數(shù)上升尚可但出血情況得到明顯改善有顯著性差異(P0.05)；2.中醫(yī)證候改善總有效率為86.66%,治療組治療前與治療后比較有顯著性差異(P0.05)；3.治療組治療前、治療后患者外周血中表達(dá)的T細(xì)胞分別與正常對(duì)照組比較,結(jié)果均有顯著性差異(P0.05)。結(jié)論：1.健脾益氣攝血法治療慢性紫癜病能明顯改善患者的臨床癥狀和出血情況,對(duì)外周血血小板計(jì)數(shù)有一定的提高。2.健脾益氣攝血法通過(guò)調(diào)整T淋巴細(xì)胞數(shù)量和比值,進(jìn)而可能起到調(diào)節(jié)機(jī)體免疫狀態(tài)的作用。
[Abstract]:Objective: to observe the clinical effect of invigorating spleen and invigorating qi and invigorating blood in patients with chronic primary immune thrombocytopenia (CITP), and to explore the relationship between deficiency of spleen syndrome and pathogenesis of CITP, as well as the effect on the expression of T cell subsets in peripheral blood of patients with chronic immune thrombocytopenia. Then the mechanism of invigorating spleen and nourishing qi and absorbing blood to treat CITP was studied in depth. Methods: thirty outpatients with CITP with Qi non-dipping blood type were selected from the outpatient clinic of Professor Sun Feng, a tutor of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine, from 2015 to 2016. The patients were treated as the treatment group by oral prescription for invigorating spleen and supplementing qi. Platelet count, TCM syndrome score, bleeding and T cell subsets were observed before and after treatment. The result is 1: 1. The total remission rate of western medicine was 73.33. Before and after treatment, the platelet count in the treatment group was higher than that in the control group, but the bleeding situation was obviously improved. The total effective rate of TCM syndrome improvement was 86.66. There was significant difference before and after treatment in the treatment group. The expression of T cells in peripheral blood of patients in the treatment group was significantly different from that in the normal control group before and after treatment (P 0.05). Conclusion 1. The treatment of chronic purpura by invigorating spleen and invigorating qi and absorbing blood can obviously improve the clinical symptoms and bleeding of patients, and improve the platelet count of peripheral blood. The method of invigorating spleen and invigorating qi and absorbing blood may regulate the immune state of the body by adjusting the number and ratio of T lymphocytes.
