【摘要】：目的：利用統(tǒng)計(jì)學(xué)軟件對(duì)謝晶日教授治療功能性便秘的中藥進(jìn)行分析,探討謝晶日教授的組方用藥規(guī)律,進(jìn)而總結(jié)謝晶日教授治療本病的用藥經(jīng)驗(yàn)。方法：收集2010年3月至2015年12月經(jīng)謝晶日教授門(mén)診診治的功能性便秘患者病例。運(yùn)用EXCEL、SPSS19.0統(tǒng)計(jì)軟件,從藥物、藥類(lèi)、四氣、五味、歸經(jīng)方面,對(duì)處方用藥進(jìn)行頻數(shù)分析、聚類(lèi)分析,探討謝晶日教授治療功能性便秘的臨床用藥規(guī)律。結(jié)果：1.根據(jù)病例納入標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn)共篩選出符合病例164例,共涉及用藥134味,使用2751次,頻數(shù)在20次以上的藥物共有28味,其累計(jì)使用頻率達(dá)80.44%。枳實(shí)(145次)、檳榔(145次)、大黃(129次)、柴胡(125次)、厚樸(123次)、火麻仁(116次)、郁李仁(114次)、烏藥(114次)、白豆蔻(111次)、草豆蔻(103次)依次是頻數(shù)頻率使用前10位的藥物。2.藥類(lèi)涉及11類(lèi),理氣藥(26.44%)、化濕藥(18.14%)、補(bǔ)虛藥(14.53%)、瀉下藥(14.41%)、清熱藥(7.58%)、消食藥(5.58%)、活血化瘀藥(3.93%)占藥類(lèi)累計(jì)使用頻率90.61%,依次是頻數(shù)頻率使用前7位的藥類(lèi)。3.溫性(47.84%)、微寒性(18.42%)、平性(17.78%)依次是頻數(shù)頻率使用前3位的四氣,辛味(37.64%)、苦味(26.63%)、甘味(23.90%)依次是頻數(shù)頻率使用前3位的五味,脾經(jīng)(22.27%)、胃經(jīng)(21.23%)、大腸經(jīng)(14.26%)、肺經(jīng)(13.19%)、肝經(jīng)(9.15%)依次是頻數(shù)頻率使用前5位的藥物歸經(jīng)。4.聚類(lèi)分析得到常用藥對(duì)組合8對(duì),常用藥物組合9組。結(jié)論：1.枳實(shí)、檳榔、大黃、柴胡、厚樸、火麻仁、郁李仁、烏藥、白豆蔻、草豆蔻是謝師治療功能性便秘的主要藥物；2.藥類(lèi)中的理氣藥、化濕藥、補(bǔ)虛藥、瀉下藥、清熱藥、消食藥、活血化瘀藥,四氣中的溫性、微寒性、平性,五味中的辛味、甘味、苦味,歸經(jīng)中的脾經(jīng)、胃經(jīng)、大腸經(jīng)、肺經(jīng)、肝經(jīng)是謝師治療功能性便秘用藥的主要藥類(lèi)、四氣、五味、歸經(jīng)；3.氣機(jī)不暢是功能性便秘的標(biāo)病機(jī),血瘀、濕阻、氣血陰陽(yáng)不足是本病機(jī)；臟腑辨證、八綱辨證、氣血津液辨證是謝師常用的辨證分析方法；辨病、辨證相結(jié)合是謝師治療功能性便秘的核心用藥法則。
[Abstract]:Objective: to analyze the effect of Professor Xie Jingri on the treatment of functional constipation by using statistical software, and to explore the rule of prescription used by Professor Xie Jingri, and to sum up the experience of Professor Xie Jingri in the treatment of this disease. Methods: from March 2010 to December 2015, the patients with functional constipation were collected. By using statistical software excel SPSS 19.0, the frequency and cluster analysis of prescription drug use were carried out from the aspects of drugs, four qi, five flavors and meridians, and the clinical drug use law of Professor Xie Jingri in the treatment of functional constipation was discussed. The result is 1: 1. According to the criteria of inclusion and exclusion of cases, 164 cases were screened out, involving 134 kinds of drugs, 2751 times of use, 28 kinds of drugs with frequency more than 20 times, the cumulative frequency of use reached 80.44%. Fructus Aurantii 145 times, Areca capsicum 145 times, Rhubarb chinensis 129 times, Bupleurum chinensis 125 times, Magnolia officinalis 123 times, Flamella sinensis 116 times, Lilium rubrum 114 times, black medicine 114 times, cardamom 111 times, cardamom 103 times) are the drugs in the first 10 places of frequency. There are 11 kinds of drugs involved in this study. The cumulative use frequency of these drugs is 96.61, 18.14, 14.53, 14.41g, 7.58g, 5.58g, 3.93g, respectively, and is followed by the seven drugs in the first seven places in order of frequency. The temperature is 47.84m, the slightly cold is 18.42, the flat is 17.78), the frequency is the first three, the Xin Wei is 37.64, the bitter is 26.63 and the sweetness is 23.90) the top three are the first three. The spleen meridian was 22.27m, the stomach meridian was 21.23m, the large intestine meridian was 14.26m, the lung meridian was 13.19m, and the liver meridian was 9.15.The order of the medicine was. 4. Cluster analysis showed 8 pairs of common drug pairs and 9 groups of common drug combinations. Conclusion 1. Fructus Aurantii, Areca nut, rhubarb, Bupleurum, Magnolia officinalis, Ramie seed, Tui Li Ren, black medicine, white nutmeg and cardamom are the main drugs used by Xie Shi to treat functional constipation. The drugs of regulating qi, dehumidifying dampness, tonifying deficiency, catharsis, heat clearing medicine, disinfecting medicine, activating blood circulation and removing blood stasis, the warmth of the four qi, the slight cold, the flat nature, the symplectic flavor of the five flavors, the sweet taste, the bitter taste, the spleen meridian, the stomach meridian, the large intestine meridian in the return to the meridian, Lung Meridian, liver Meridian is Xie Shi treatment of functional constipation of the main drugs, four qi, five flavor, Gui Jing 3. Qi Qi unobstructed is the standard pathogenesis of functional constipation, blood stasis, dampness obstruction, deficiency of qi, blood yin and yang are the pathogenesis of the disease; the syndrome differentiation of zang-fu organs, eight principles and qi, blood and body fluid are commonly used by Xie Shi for syndrome differentiation and analysis; The combination of syndrome differentiation is the core medication principle of Xie Shi in the treatment of functional constipation.
