【摘要】：目的:通過對葉天士診治婦科疾病的醫(yī)案進(jìn)行整理,探討葉天士在臨床中診治婦科疾病時(shí)的用藥規(guī)律以及用藥特色,以期能夠?qū)ΜF(xiàn)今婦科臨床用藥起到一定的啟發(fā)和借鑒意義。方法:整理現(xiàn)階段對葉天士婦科診療經(jīng)驗(yàn)的成果,遴選《臨證指南醫(yī)案》、《葉氏醫(yī)案存真》、《種福堂公選良方·附醫(yī)案》、《葉天士晚年方案真本》、《未刻本葉氏醫(yī)案》、《三家醫(yī)案合刊·葉氏醫(yī)案》、《眉壽堂方案選存》等7本醫(yī)案中的婦科醫(yī)案,整理其中葉天士處方中使用到的藥物,并將其按照頻次高低進(jìn)行排序,探究葉天士臨床用藥的基本規(guī)律。就葉氏婦科醫(yī)案中頗具特色的用法進(jìn)行整理,探究葉天士用藥的基本特色。結(jié)果:以《葉天士醫(yī)學(xué)全書》為底本,整理葉天士各個(gè)醫(yī)案集中的婦科醫(yī)案并將其中涉及到的藥物按照頻次高低進(jìn)行排序,發(fā)現(xiàn)《臨證指南醫(yī)案》中共涉及藥物234味,《葉案存真》中共涉及藥物114味,《葉天士晚年方案真本》中共涉及藥物120味,《種福堂公選良方·續(xù)醫(yī)案》中共涉及藥物68味,《眉壽堂方案選存》共涉及藥物181味;《三家醫(yī)案合刻·葉天士醫(yī)案》中涉及藥物87味,《未刻本葉天士醫(yī)案》涉及藥物90味。綜合7種醫(yī)案,使用頻次前十的藥物分別是:當(dāng)歸、人參、茯苓、白芍、甘草、生地黃、阿膠、茯神、香附、小茴香。結(jié)論:葉天士婦科用藥注重補(bǔ)益氣血,重視脾胃;其婦科用藥推重氣味、善調(diào)奇經(jīng)、注重血肉有情之品的應(yīng)用、善用絡(luò)藥、注重四時(shí)陰陽、藥分三焦以及善用炭藥。
[Abstract]:Objective: through sorting out the medical records of Ye Tianshi in the diagnosis and treatment of gynecological diseases, to discuss the law and characteristics of the medication of Ye Tianshi in the diagnosis and treatment of gynecological diseases, so as to provide some inspiration and reference for the clinical use of gynecological drugs in modern gynecology. Methods: the results of gynecological diagnosis and treatment of Ye Tianshi at the present stage were summarized. Selection of "Clinical evidence Guide Medical record", < Ye's Medical record stored in Truth >, "Variety Futang Public Choice of good prescription with Medical record", "True version of Ye Tianshi's Old Age Plan", < unengraved Ye's Medical case >, < three Medical cases jointly published in Ye's Medical case >, "Mei Shoutang Scheme selection and Survival" and other 7 copies Gynecological medical records in medical records, The drugs used in the prescription of Yetianshi were sorted according to the frequency to explore the basic law of the clinical drug use. The characteristic usage of Ye Tianshi's gynecologic medical case was arranged, and the basic characteristics of Ye Tianshi's medicine were explored. Results: taking Ye Tianshi Medical Book as the base, the gynecological medical records of Ye Tianshi were arranged and the drugs involved were sorted according to the frequency. It was found that 234 kinds of drugs were involved in the Medical record of Clinical practice, 114 kinds of drugs were involved in Ye case, 120 kinds of drugs were involved in the plan of Ye Tianshi's old age, and 120 kinds of drugs were involved in the trial. 68 taste, "Mei Shoutang scheme selection" involved 181 kinds of drugs, "three medical records combined engraved Ye Tianshi medical case" involved 87 kinds of drugs, < unengraved Ye Tianshi medical case > involving 90 kinds of drugs. Comprehensive 7 kinds of medical cases, the use of the first ten drugs are: angelica, ginseng, Poria cocos, Radix Paeoniae Alba, Glycyrrhiza, Rehmannia, Ejiao, Fuzhuan Shen, Xanthoxylum, fennel. Conclusion: Ye Tianshi's gynaecological medicine pays attention to replenishing qi and blood, attaching importance to spleen and stomach, and its gynecological drugs push heavy smell, adjust Qi meridian, pay attention to the application of blood and flesh, make good use of collaterals, pay attention to 04:00 yin and yang, divide medicine into three Jiao and make good use of charcoal medicine.
