【摘要】：目的：觀察參黃熏洗方促進(jìn)濕熱下注型肛瘺術(shù)后創(chuàng)面愈合的作用。方法：按病例選擇標(biāo)準(zhǔn)選擇60例肛瘺患者,隨機(jī)分為2組,參黃熏洗方組(治療組)和高錳酸鉀組(對(duì)照組)均為30例。治療組自術(shù)后第2天開始用參黃熏洗方熏洗,常規(guī)換藥,每日一次,14天為一個(gè)療程；對(duì)照組自術(shù)后第2天給予高錳酸鉀溶液(1：5000)熏洗,常規(guī)換藥,每日一次,14天為一個(gè)療程。分別于術(shù)后第2天、第7天、第14天記錄創(chuàng)面疼痛、肉芽形態(tài)、分泌物情況；手術(shù)當(dāng)天、術(shù)后第7天、第14天測(cè)算創(chuàng)面面積大小,計(jì)算創(chuàng)面愈合率；評(píng)估術(shù)后總體療效；記錄創(chuàng)面完全愈合時(shí)間。結(jié)果：1-組內(nèi)比較：用藥后創(chuàng)面疼痛、滲液及創(chuàng)口肉芽生長(zhǎng)情況均有顯著性差異(P0.05)；組間比較：術(shù)后第2天創(chuàng)面疼痛、滲液及創(chuàng)口肉芽生長(zhǎng)情況均無顯著性差異(P0.05)；術(shù)后第7、14天創(chuàng)面疼痛、創(chuàng)面滲液及創(chuàng)口肉芽生長(zhǎng)情況均有差異顯著(P0.05),治療組優(yōu)于對(duì)照組。2.創(chuàng)面愈合率：組內(nèi)比較,用藥后兩組均有差異顯著(P0.05)；組間比較,術(shù)后第7、14天均有顯著差異(P0.05),治療組優(yōu)于對(duì)照組。3.創(chuàng)面愈合時(shí)間：治療組創(chuàng)面愈合時(shí)間為15～28天,平均愈合時(shí)間為17.73±2.599天,對(duì)照組創(chuàng)面愈合時(shí)間為16-32天,平均愈合時(shí)間為20.62±3.820天,兩組比較,有顯著性差異(P0.05),治療組優(yōu)于對(duì)照組。4.兩組治療后癥狀、體征均消失,臨床總療效均為100%。兩組總療效比較差異無統(tǒng)計(jì)學(xué)意義。5.用藥過程中兩組未發(fā)現(xiàn)明顯不良反應(yīng)。結(jié)論：通過臨床研究表明,參黃熏洗方具有顯著清熱燥濕、散結(jié)止痛等作用,能明顯緩解肛瘺術(shù)后創(chuàng)面疼痛,減少創(chuàng)面分泌物、促進(jìn)創(chuàng)面肉芽組織的生長(zhǎng),提高創(chuàng)面愈合的質(zhì)量,縮短了創(chuàng)面愈合時(shí)間,療效優(yōu)于高錳酸鉀,值得臨床推廣應(yīng)用臨床。
[Abstract]:Objective: to observe the effect of Shenhuang fumigation and washing prescription on wound healing after wet-heat injection anal fistula. Methods: sixty patients with anal fistula were randomly divided into two groups: the treatment group (30 cases) and the potassium permanganate group (30 cases). The treatment group was fumigated and washed with Shenhuang fumigation formula from the second day after operation, routine dressing was changed once a day for 14 days as a course of treatment, the control group was given potassium permanganate solution 1: 5 000) fumigation washing from the second day after operation, routine dressing change, 14 days a day as a course of treatment. The wound pain, granulation morphology and secretion were recorded on the 2nd, 7th and 14th day after operation, the size of the wound was calculated on the 7th day and the 14th day after the operation, and the rate of wound healing was calculated, and the overall curative effect was evaluated. The time of complete wound healing was recorded. Results: there were significant differences in wound pain, exudate and granulation growth between the two groups (P 0.05). There was no significant difference in the growth of granulation in exudate and wound, but there was significant difference in wound pain, wound exudation and granulation growth on day 714 after operation. The treatment group was superior to the control group (P < 0.05). Wound healing rate: there was significant difference in wound healing rate between the two groups after treatment, and there was significant difference between the two groups on the 7th day after operation (P 0.05), which was better than that in the control group (P 0.05). Wound healing time: the healing time of the treatment group was 1528 days, the average healing time was 17.73 鹵2.599 days, and that of the control group was 16-32 days and the average healing time was 20.62 鹵3.820 days. There was a significant difference between the two groups (P 0.05), and the treatment group was better than the control group (.4). Symptoms and signs disappeared after treatment in both groups, and the total clinical efficacy was 100. There was no significant difference in total curative effect between the two groups. No obvious adverse reactions were found in the two groups. Conclusion: the clinical study shows that Shenhuang fumigation and washing prescription has the effects of clearing heat, drying and dampness, dispersing and relieving pain, relieving pain of wound after anal fistula, reducing the secretion of wound, and promoting the growth of granulation tissue of wound. To improve the quality of wound healing and shorten wound healing time, the curative effect is better than potassium permanganate.
