【摘要】：目的:小兒Hp相關(guān)胃脘痛是指在臨床上Hp檢測呈陽性反應(yīng)的胃脘痛,導(dǎo)師認(rèn)為此類胃脘痛多由于小兒脾胃虛弱,運化失暢,積滯中焦,胃氣壅滯,脈絡(luò)受阻,不通則痛而發(fā)生本病,考慮到小兒“純陽之體”,且近年來飲食結(jié)構(gòu)的改變,故“胃中蘊熱”型此類病尤為常見。目前臨床有效治療兒科胃痛的中成藥種類較少,因此,在總結(jié)導(dǎo)師臨床用藥的經(jīng)驗基礎(chǔ)上,自擬清胃和中湯治療小兒Hp相關(guān)胃脘痛(胃中蘊熱型)30例,通過觀察治療前后患兒癥狀體征及實驗室檢查結(jié)果,明確清胃和中湯的療效,從而減輕患兒痛苦,進行臨床推廣。方法:選取2014年12月至2015年12月于吉林省中醫(yī)藥科學(xué)院第一臨床醫(yī)院兒科二診就診,符合納入標(biāo)準(zhǔn)的30例患兒進行病例觀察研究,采取自身對照,以清胃瀉熱,和中止痛為治療原則,給予自擬清胃和中湯:黃芩15g,黃連6g,黨參10g,蒼術(shù)6g,金銀花10g,連翹10g,萊菔子15g,枳實6g,白芍10g,甘草5g,上藥均為免煎制劑(三九免煎顆粒)。用法用量:上諸藥各一袋,混合后用100ml開水沖泡,1劑1日,分3次口服,7天為一個療程,可連續(xù)觀察三個療程。停藥后或后期鞏固期復(fù)查HP,后期隨訪以觀察療效。結(jié)果:通過治療30例病例進行臨床觀察,其中治愈12例,有效15例,無效3例,總有效率為90%,并且無明顯的不良反應(yīng)。結(jié)論:隨著人類飲食結(jié)構(gòu)的調(diào)整,小兒Hp相關(guān)胃脘痛(胃中蘊熱型)病證數(shù)量日益增多,結(jié)合生活及作息習(xí)慣,在學(xué)齡期兒童尤為常見。從中醫(yī)辨證角度看,此病證為本虛標(biāo)實證,多由于小兒脾胃虛弱,運化無力,加之飲食不節(jié),積滯中焦,脈絡(luò)不通引起,且小兒本為陽熱體質(zhì),邪氣迅速入里化熱,引起諸如胃痛、惡心、噯氣、食欲不振、口臭、便秘等一系列癥狀。治療上以清胃瀉熱,和中止痛為治療原則,從而達到治病除疾的目的。本課題從臨床入手,經(jīng)過導(dǎo)師的悉心指導(dǎo),運用清胃瀉熱,和中止痛之品,擬清胃和中湯進行臨床療效觀察,符合納入標(biāo)準(zhǔn)的臨床患兒從癥狀、體征及理化檢查方面均有較好的改善,所服用的中藥類型及劑量也符合兒科用藥安全,基本無副作用,值得臨床推廣應(yīng)用。
[Abstract]:Objective : Hp - related abdominal pain in children refers to the abdominal pain which is positive for Hp detection in clinic . It is suggested that this kind of epigastralgia is mainly due to the weakness of the spleen and stomach of the children , the obstruction of transportation , the accumulation of middle jiao , the stagnation of the stomach qi , the obstruction of the vein and the pain , and the occurrence of this disease , taking into account the " pure yang " body of the children A clinical observation was made on 30 cases of children with Hp - related epigastralgia ( stomach ) by observing the symptoms and laboratory tests . Results : With the adjustment of dietary structure , the number of symptoms and symptoms in children with Hp - related abdominal pain ( stomach ) increased , and the total effective rate was 90 % . This disease is characterized by deficiency of spleen and stomach of children , inability to transport , diet , indigestion , constipation and other symptoms . It can be used for treating diseases and eliminating diseases . It is suitable for clinical observation of symptoms , signs and physical and chemical examination .
