【摘要】：目的:通過觀察益氣養(yǎng)陰法參與治療眼底出血的臨床療效,探索中醫(yī)藥控制眼底出血的有效方法。臨床資料與方法:將2016年8月至2017年1月就診于沈陽何氏眼科醫(yī)院氣陰兩虛型眼底出血的患者共計57例(69眼)應(yīng)用隨機數(shù)字表法隨機分為兩組:對照組28例(34眼)和治療組29例(35眼)。對照組予以基礎(chǔ)治療(控制血壓,血糖,血脂等)、口服卵磷脂絡(luò)合碘片。治療組在對照組處置的基礎(chǔ)上,給予我院自制益氣養(yǎng)陰的止血湯早晚溫服。觀察并記錄兩組患者治療前后的視力變化、眼底出血面積、黃斑OCT平均厚度、中醫(yī)證候積分等項,并對兩組患者進行比較做統(tǒng)計學(xué)分析評價。結(jié)果:1.總體療效:治療前與治療后一個月后比較,對照組的總有效率為64.47%,治療組為82.86%,經(jīng)統(tǒng)計分析差異具有統(tǒng)計學(xué)意義(P0.05),止血湯輔助治療眼底出血療效更佳。2.視力療效:治療一個月后兩組視力均有提高,且治療組優(yōu)于對照組(P0.05)。3.出血面積吸收:治療一個月后兩組出血面積均減少,且治療組優(yōu)于對照組(P0.01)。4.黃斑OCT平均厚度:治療一個月后兩組黃斑OCT平均厚度均有所降低,治療組優(yōu)于對照組,但差異無統(tǒng)計學(xué)意義(P0.05)。5.中醫(yī)證候積分:治療一個月后治療組各項中醫(yī)證候積分較前改變,治療組優(yōu)于對照組(P0.01)。6.中醫(yī)證候療效:對照組有效率為25%,治療組有效率為82.75%,治療組優(yōu)于對照組(P0.01)。7.糖尿病視網(wǎng)膜病變與視網(wǎng)膜靜脈阻塞比較:治療組內(nèi)視網(wǎng)膜靜脈阻塞的療效好于糖尿病視網(wǎng)膜病變,但是差異不具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:本次療效觀察中未出現(xiàn)不良反應(yīng),止血湯參與治療氣陰兩虛型眼底出血具有良好的臨床療效,證明止血湯輔助眼底出血相關(guān)疾病的療效優(yōu)于單純西藥組,總有效率高達82.86%。止血湯有助于眼底出血的吸收、水腫的吸收、視力的改善,提高患者的滿意度,中西醫(yī)結(jié)合治療眼底出血值得臨床推廣應(yīng)用。
[Abstract]:Objective: to explore the effective method of traditional Chinese medicine (TCM) to control fundus hemorrhage by observing the clinical effect of nourishing qi and nourishing yin in the treatment of fundus hemorrhage. Clinical data and methods: a total of 57 patients (69 eyes) with ocular fundus hemorrhage of Qi and Yin deficiency type from August 2016 to January 2017 were randomly divided into two groups: control group (28 cases, 34 eyes) ) and treatment group (29 cases, 35 eyes). The control group was treated with basic treatment (blood pressure, blood glucose, blood lipid, etc.), oral lecithin complex iodine tablets. On the basis of the control group, the treatment group was given the self-made Zhixue decoction for supplementing qi and nourishing yin in the morning and evening. The changes of visual acuity, the area of fundus hemorrhage, the mean thickness of macular OCT, and the score of TCM syndromes were observed and recorded before and after treatment. The result is 1: 1. The total effective rate was 64.47 in the control group and 82.86 in the treatment group. The difference was statistically significant (P 0.05). The curative effect of Zhixue decoction in the treatment of ocular fundus hemorrhage was better than that in the control group. Visual acuity: one month after treatment, the visual acuity of both groups were improved, and the treatment group was superior to the control group (P 0.05). 3. Haemorrhage area absorption: after one month of treatment, the haemorrhage area of both groups was decreased, and the treatment group was superior to the control group (P 0.01). 4. Mean thickness of macular OCT: after one month of treatment, the mean thickness of macular OCT in both groups was lower than that in control group, but the difference was not statistically significant (P 0.05). TCM syndromes integral: after one month of treatment, the TCM syndrome integral of the treatment group changed more than that of the former, and the treatment group was superior to the control group (P 0.01g. 6). The curative effect of TCM syndrome: the effective rate of the control group was 25g, the effective rate of the treatment group was 82.75m, the treatment group was superior to the control group (P0.01g. 7). Comparison of diabetic retinopathy and retinal vein occlusion: the therapeutic effect of retinal vein occlusion in the treatment group was better than that in diabetic retinopathy, but the difference was not statistically significant (P 0.05). Conclusion: there is no adverse reaction in this curative effect observation. Zhixue decoction has good clinical effect in treating ocular fundus hemorrhage with deficiency of Qi and Yin. It is proved that Zhixue decoction is superior to western medicine group in assisting ocular fundus hemorrhage related diseases, and the total effective rate is as high as 82.86. Zhixue decoction is helpful to the absorption of fundus hemorrhage, the absorption of edema, the improvement of visual acuity and the improvement of patients' satisfaction.
