【摘要】：目的:運(yùn)用系統(tǒng)評價(jià)方法對益氣活血法治療慢性萎縮性胃炎的臨床療效進(jìn)行客觀評價(jià),提供相應(yīng)臨床證據(jù),促使臨床醫(yī)療決策更加科學(xué)化。方法:全面收集1996年1月至2016年1月間發(fā)表在國內(nèi)外各雜志報(bào)刊的相應(yīng)文獻(xiàn),選擇符合納入標(biāo)準(zhǔn)且以中醫(yī)或中西醫(yī)結(jié)合療法治療CAG并設(shè)有相應(yīng)西醫(yī)對照組的臨床研究;制定相關(guān)標(biāo)準(zhǔn)以篩選合格研究;采用Jadad評分標(biāo)準(zhǔn)評價(jià)文獻(xiàn)質(zhì)量,并對效應(yīng)指標(biāo)進(jìn)行異質(zhì)性檢驗(yàn)、Meta-分析等。結(jié)果:此項(xiàng)研究納入文獻(xiàn)34篇進(jìn)行系統(tǒng)分析,共計(jì)患者3605例;納入研究方法學(xué)質(zhì)量評價(jià)較低,其中Jadad評分3分及以下的33篇,4-7分1篇;Meta分析結(jié)果為:中醫(yī)益氣活血法治療慢性萎縮性胃炎在提高綜合療效、改善胃鏡和病理變化、提高HP轉(zhuǎn)化率、降低復(fù)發(fā)率等方面與西醫(yī)治療相比,優(yōu)勢明顯;統(tǒng)計(jì)試驗(yàn)組內(nèi)中藥使用頻率,以益氣活血類為主;對納入文獻(xiàn)行偏倚性檢查,未呈“倒漏斗圖”分布,其圖形中各散點(diǎn)分布不對稱,表明可能存在一定偏倚。結(jié)論:Meta-分析結(jié)果表明,運(yùn)用中醫(yī)學(xué)理論為指導(dǎo),通過中醫(yī)學(xué)思維來辨證論治,同時(shí)結(jié)合臨床實(shí)際,中醫(yī)益氣活血法治療慢性萎縮性胃炎療效顯著,復(fù)發(fā)率亦比較低。然而由于納入文獻(xiàn)方法學(xué)質(zhì)量評價(jià)指標(biāo)單一、研究樣本量不夠大、文獻(xiàn)庫偏倚或發(fā)表偏倚存在等原因,本研究存在一定局限性實(shí)屬難免。
[Abstract]:Objective: to evaluate the clinical efficacy of Yiqi Huoxue method in the treatment of chronic atrophic gastritis by means of systematic evaluation, to provide the corresponding clinical evidence and to make the clinical medical decision more scientific. Methods: from January 1996 to January 2016, we collected the relevant literature published in various magazines and newspapers at home and abroad, and selected the clinical research that met the inclusion standard and treated CAG with traditional Chinese medicine or combination of traditional Chinese medicine and western medicine and had corresponding western medicine control group. Relevant criteria were established to screen qualified research, and Jadad scoring criteria were used to evaluate the quality of literature, and to test the heterogeneity of effect indicators by Meta-analysis. Results: a total of 3605 patients were included in 34 articles of the study, and the quality evaluation of the methodology was low. The results of meta-analysis showed that the treatment of chronic atrophic gastritis with the method of invigorating qi and activating blood circulation in traditional Chinese medicine could improve the comprehensive curative effect, improve the gastroscope and pathological changes, and increase the conversion rate of HP. Compared with the western medicine treatment, the advantages of reducing the recurrence rate were obvious; in the statistical test group, the use frequency of traditional Chinese medicine was mainly the type of invigorating qi and promoting blood circulation, and the bias of the literature was not "inverted funnel graph" distribution. The distribution of scattered points in the graph is asymmetric, indicating that there may be a certain bias. Conclusion the results of the analysis show that the Chinese medicine theory is used to treat chronic atrophic gastritis with TCM theory as the guide, and the TCM thinking is used to treat the patients with chronic atrophic gastritis by combining with the clinical practice, and the curative effect of traditional Chinese medicine on chronic atrophic gastritis is remarkable, and the recurrence rate is also relatively low. However, due to the single quality evaluation index of literature methodology, the lack of sample size, and the existence of library bias or publication bias, it is inevitable that this study has some limitations.
