【摘要】：目的：利用統(tǒng)計(jì)學(xué)方法,對(duì)宋立群教授治療腎性血尿的方藥進(jìn)行分析,總結(jié)其用藥規(guī)律,為臨床辨治腎性血尿提供理論依據(jù)與辨治方法。方法：通過整理導(dǎo)師宋立群教授近5年門診腎性血尿的醫(yī)案,運(yùn)用SPSS19.0統(tǒng)計(jì)軟件和STATA13.0統(tǒng)計(jì)軟件,應(yīng)用頻數(shù)分析、聚類分析和普通最小二乘法等分析方法對(duì)宋立群教授的300則處方,共185味藥物進(jìn)行統(tǒng)計(jì)分析,探討臨床用藥規(guī)律。結(jié)果：總結(jié)出宋立群教授用于治療腎性血尿的頻次前19味藥物,分別為：側(cè)柏炭、地榆炭、焦白術(shù)、大薊炭、小薊炭、生地炭、熟地炭、炒山楂、炒麥芽、炒神曲、杜仲炭、棕櫚炭、茯苓、黨參、黃芪、藕節(jié)炭、牛蒡子、女貞子、炙甘草。四氣中使用頻次前兩位的為平性和溫性。五味中使用頻次前三位的依次是甘味藥、苦味藥和澀味藥。歸經(jīng)中使用頻率前三位的分別為肝、脾、胃經(jīng)。藥類涉及15類,前三位分別為補(bǔ)虛藥、止血藥和清熱藥。總結(jié)出宋立群教授用于治療腎性血尿的藥對(duì)7組,藥物組合12組。經(jīng)統(tǒng)計(jì)得到：組方用藥中炭類藥物應(yīng)用的味數(shù),相對(duì)于BLD(+)而言,BLD(++)和BLD(+++)的炭藥味數(shù)顯著上升,但是,BLD(++)和BLD(+++)的炭藥味數(shù)并未顯著性差異；有尿蛋白的病人炭藥味數(shù)顯著降低；炭類藥物味數(shù)在患者性別上無顯著差異。結(jié)論：1.總結(jié)得到其核心藥物以四君子湯及炭類藥為主,四氣以平溫為主,藥味以甘苦澀為主,歸經(jīng)以肝脾胃為主；藥類以補(bǔ)益藥、止血藥、清熱藥三類藥物為主。2.歸納得出其組方用藥特點(diǎn)：脾腎雙補(bǔ),肝腎同調(diào),視為扶正；運(yùn)利水濕,清熱涼血,視為驅(qū)邪；善用炭藥,收斂固澀,標(biāo)本并重。
[Abstract]:Objective: to analyze the prescriptions of Professor Song Liqun in the treatment of renal hematuria by statistical method, summarize the rule of drug use, and provide the theoretical basis and treatment method for clinical differentiation and treatment of renal hematuria. Methods: through sorting out the medical records of outpatients with renal hematuria in outpatient clinic of Professor Song Liqun in recent 5 years, using SPSS19.0 statistical software and STATA13.0 statistical software, using frequency analysis, cluster analysis and general least square method, 300 prescriptions of Professor Song Liqun were obtained. A total of 185 kinds of drugs were statistically analyzed to explore the rule of clinical drug use. Results: the first 19 kinds of drugs used by Professor Song Liqun in the treatment of renal hematuria were summarized as follows: Platycladus orientalis charcoal, Dielm charcoal, Atractylodes macrocephala, large thistle charcoal, small artichoke charcoal, raw land charcoal, cooked land charcoal, Hawthorn, stir-fried malt, fried Shenqu, Eucommia charcoal. Palm charcoal, Poria cocos, Codonopsis, Astragalus, Lotus root charcoal, Arctium lappa, Ligustrum lucidum, grilled licorice. The first two places in the use frequency of the four gases are flatness and temperance. The top three of the five flavors are sweet flavor medicine, bitter medicine and astringent medicine. The liver, spleen and stomach were the first three most frequently used in the meridian. Drug category involves 15 categories, the first three are tonifying drugs, hemostatic drugs and heat-clearing drugs respectively. Summing up Professor Song Liqun used for the treatment of renal hematuria in 7 groups, drug combination of 12 groups. According to the statistics, the taste number of carbon drugs used in prescription medicine was significantly higher than that of BLD- () and BLD- (), but there was no significant difference between BLD- () and BLD(). The number of taste of charcoal in patients with urine protein decreased significantly, but there was no significant difference in the number of taste of carbon drugs in patients. Conclusion 1. It is concluded that its core drugs are mainly Sijunzi decoction and charcoal drugs, four qi is mainly flat temperature, the medicinal taste is mainly sweet and astringent, the meridian is mainly liver, spleen and stomach, and the drugs are mainly three kinds of drugs: tonifying drugs, hemostatic drugs, and heat-clearing drugs. 2. Summed up its prescription medicine characteristics: spleen and kidney double tonifying, liver and kidney homology, as a strengthening; transport water and moisture, clear heat and cool blood, as exorcism; make good use of charcoal medicine, astringency, astringency and astringency, the specimen is equally important.
