【摘要】：目的:本研究旨在觀察清火解毒消癭湯治療心肝火旺型橋本甲狀腺炎的臨床療效,并探討其相關(guān)作用機(jī)理。方法:將90例符合納入標(biāo)準(zhǔn)的心肝火旺型橋本甲狀腺炎患者,隨機(jī)分成兩組,試驗(yàn)組45例,對(duì)照組45例。試驗(yàn)組給予清火解毒消癭湯,對(duì)照組給予夏枯草口服液,療程均為12周。觀察治療前后兩組中醫(yī)證候積分、血中甲狀腺激素水平、甲狀腺自身抗體水平、甲狀腺峽部厚度等指標(biāo),并對(duì)相關(guān)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,評(píng)價(jià)清火解毒消癭湯的臨床療效。結(jié)果:試驗(yàn)組總有效率為84.44%,對(duì)照組總有效率為53.33%,兩組臨床療效比較,差異有統(tǒng)計(jì)學(xué)意義(PO.O1)。兩組中醫(yī)癥狀積分治療前后和治療后組間比較,有顯著統(tǒng)計(jì)學(xué)意義(P0.01)。試驗(yàn)組在改善頸前癭腫、煩躁易怒、口干口苦、怕熱多汗、神疲乏力、少寐多夢(mèng)癥狀方面明顯優(yōu)于對(duì)照組(PO.O1)。兩組FT3、FT4、TSH值比較,HT甲亢組治療前后有統(tǒng)計(jì)學(xué)意義(P0.01),治療后組間比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);兩組TPOAb、TGAb水平治療后比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.01)。兩組HT甲亢患者甲狀腺峽部厚度比較,兩組治療前后差異具有統(tǒng)計(jì)學(xué)意義(P0.05);治療后組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:清火解毒消癭湯可以明顯改善心肝火旺型橋本甲狀腺炎患者的臨床癥狀,降低甲狀腺自身抗體水平,調(diào)節(jié)甲狀腺功能并能緩解甲狀腺腫大,且無(wú)明顯不良反應(yīng),是一種安全有效的治療方法。
[Abstract]:Objective: to observe the clinical effect of Qinghuo jiedu Xiaoying decoction in treating Hashimoto's thyroiditis of heart-liver-Huowang type and to explore its related mechanism. Methods: 90 patients with Hashimoto's thyroiditis were randomly divided into two groups: the experimental group (n = 45) and the control group (n = 45). The experimental group was given Qinghuo jiedu Xiaoying decoction and the control group was given Prunella vulgaris oral liquid for 12 weeks. Observe the TCM syndromes integral, serum thyroid hormone level, thyroid autoantibody level, thyroid isthmus thickness and other indexes before and after treatment, and carry on the statistical analysis to the related data, evaluate the clinical curative effect of Qinghuo jiedu Xiaoying decoction. Results: the total effective rate of the trial group was 84.44 and the total effective rate of the control group was 53.33.The difference between the two groups was statistically significant. There was significant difference between the two groups before and after the treatment. The treatment group was superior to the control group in improving cervical gall swelling, irritability and irritability, dry mouth and bitter mouth, being afraid of heat and sweating, tired and tired, and sleeping and dreaming symptoms. There was significant difference in TSH between the two groups before and after treatment, but there was no significant difference between the two groups before and after treatment, and there was significant difference between the two groups after the treatment of TPOAb-TGAb. The thickness of thyroid isthmus in HT hyperthyroidism patients was significantly different between the two groups before and after treatment, but there was no significant difference between the two groups after treatment. Conclusion: Qinghuajiedu Xiaoying decoction can obviously improve the clinical symptoms of patients with heart-liver-Huowang type Hashimoto's thyroiditis, reduce the level of thyroid autoantibodies, regulate thyroid function and alleviate goitre without obvious adverse reactions. It is a safe and effective treatment.
