【摘要】：目的:通過運(yùn)用Cochrane系統(tǒng)評價方法,客觀評價益氣養(yǎng)陰法治療功能性便秘的臨床療效,揭示該法治療功能性便秘的優(yōu)勢,為臨床治療方法的選擇及應(yīng)用提供研究證據(jù)。方法:詳細(xì)搜集1995.12-2015.12國內(nèi)公開發(fā)表的關(guān)于益氣養(yǎng)陰法治療功能性便秘的研究文獻(xiàn),嚴(yán)格按照納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)擇取相關(guān)文獻(xiàn),納入以中醫(yī)益氣養(yǎng)陰法治療為試驗(yàn)組,以西醫(yī)治療(PEG、促動力藥、酚酞片等口服)為對照組的試驗(yàn)研究,評價選取的文獻(xiàn)質(zhì)量,對所需詳細(xì)相關(guān)信息予以提煉。Meta分析選用最新Review Manager5.3軟件,分析綜合療效、癥狀積分、復(fù)發(fā)率及不良反應(yīng),描繪森林圖、檢驗(yàn)敏感性、分析漏斗圖。結(jié)果:1.共納入符合標(biāo)準(zhǔn)的37篇文獻(xiàn)進(jìn)行研究,共計3565例功能性便秘患者;2.大部分納入文獻(xiàn)研究的方法學(xué)質(zhì)量偏低,4-7分的僅有1篇,12篇3分文獻(xiàn),20篇2分文獻(xiàn),1分的有4篇;3.試驗(yàn)組中藥使用頻率統(tǒng)計顯示:主要為益氣養(yǎng)陰類藥物(黃芪、白術(shù)、生地黃、麥冬、當(dāng)歸、玄參等);4.Meta分析結(jié)果:除在減少不良反應(yīng)方面,中醫(yī)益氣養(yǎng)陰法與西醫(yī)治療無明顯差異外,在綜合療效、改善癥狀、降低復(fù)發(fā)率等方面中醫(yī)益氣養(yǎng)陰法治療FC優(yōu)勢明顯。結(jié)論:本研究對近20年中醫(yī)藥益氣養(yǎng)陰法治療功能性便秘的狀況進(jìn)行了客觀評價,結(jié)果表明,該法治療功能性便秘在綜合療效、改善癥狀及降低復(fù)發(fā)率方面效果顯著。由于符合此次研究的文獻(xiàn)質(zhì)量評價普遍較低,導(dǎo)致發(fā)表偏倚,期待以后能有質(zhì)量高、樣本大的臨床試驗(yàn)對結(jié)果的可靠性加以佐證,促進(jìn)中醫(yī)藥的理論與實(shí)踐相結(jié)合。
[Abstract]:Objective: to objectively evaluate the clinical efficacy of the method of supplementing qi and nourishing yin in the treatment of functional constipation by using Cochrane system evaluation method, and to reveal the advantages of this method in the treatment of functional constipation, and to provide evidence for the selection and application of clinical treatment methods. Methods: the literature on the treatment of functional constipation by invigorating qi and nourishing yin was collected in detail from May 12 to May 12, 2015.The relevant documents were selected strictly according to the inclusion standard and exclusion standard, and were included in the experimental group treated with traditional Chinese medicine to nourish qi and nourish yin. The control group was treated with western medicine as control group. The quality of the selected literature was evaluated, and the relevant information was extracted. The latest Review Manager5.3 software was used to analyze the comprehensive curative effect and symptom score. Recurrence rate and adverse reactions, forest map, sensitivity test, funnel map analysis. The result is 1: 1. A total of 3565 functional constipation patients were studied in 37 articles. In most of the literature studies, the quality of methodology was relatively low (4-7). Only one of 12 papers with 3 points was divided into 20 papers with 2 points, and 4 articles with 1 score were classified into 4 papers and 3 papers respectively. Statistics of the frequency of Chinese medicine used in the experimental group showed that the main drugs (Astragalus membranaceus, Atractylodes macrocephala, Radix Rehmanniae, Radix Ophiopogonis, Radix angelicae Sinensis, Radix Hyacinopsis et al.). 4. Meta-analysis results: except for the reduction of adverse reactions, There is no significant difference between traditional Chinese medicine and western medicine in the comprehensive efficacy, improve symptoms, reduce recurrence rate and other aspects of traditional Chinese medicine Qi nourishing yin treatment of FC advantages. Conclusion: this study made an objective evaluation on the treatment of functional constipation with traditional Chinese medicine in the past 20 years. The results showed that this method had remarkable effect in treating functional constipation, improving symptoms and reducing recurrence rate. Because the evaluation of literature quality according with this study is generally low, it leads to publication bias. It is expected that there will be high quality and large sample clinical trials to prove the reliability of the results in order to promote the combination of theory and practice of traditional Chinese medicine.
