【摘要】：目的:通過觀察舒肝顆粒治療糖尿病周圍神經(jīng)病變伴輕中度抑郁癥的臨床療效,探討糖尿病周圍神經(jīng)病變的治療新方法,以提高糖尿病患者的生活質(zhì)量,減輕患者的心理痛苦,降低本病的致殘率。方法:將60例糖尿病周圍神經(jīng)病變伴輕中度抑郁癥的患者隨機(jī)分為治療組和對(duì)照組,治療組與對(duì)照組各30例。對(duì)照組在給予基礎(chǔ)降糖治療的情況下給予甲鈷胺注射500ug肌肉注射QD。治療組在對(duì)照組治療的基礎(chǔ)上加服舒肝顆粒,一次一袋,一日三次,開水沖服。療程8周,觀察中醫(yī)癥侯、血糖,糖化血紅蛋白,神經(jīng)傳導(dǎo)速度,漢密爾頓(HAMD)抑郁量表測定的變化。安全性療效觀察血常規(guī)、尿常規(guī)、肝腎功等變化。結(jié)果:治療組綜合有效率為73.3%,對(duì)照組綜合有效率為50%,治療組優(yōu)于對(duì)照組,P㩳0.05。治療組中醫(yī)癥侯與對(duì)照組相比,有所改善,且P㩳0.05;治療組血糖與對(duì)照組相比,有所改善,且P㩳0.05;治療組神經(jīng)傳導(dǎo)速度與對(duì)照組相比,有所改善,且P㩳0.05;治療組漢密爾頓(HAMD)抑郁量表得分較對(duì)照組明顯改善,且P㩳0.05。安全性指標(biāo)前后無明顯變化,P0.05。說明舒肝顆粒可以理氣疏肝,緩解患者抑郁狀態(tài),進(jìn)而提高神經(jīng)傳導(dǎo)速度,且安全有效。結(jié)論:舒肝顆粒可明顯改善糖尿病周圍神經(jīng)病變伴輕中度抑郁患者的臨床癥狀、血糖,神經(jīng)傳導(dǎo)速度,漢密爾頓(HAMD)抑郁量表得分等各項(xiàng)指標(biāo),臨床療效顯著,具有良好的臨床推廣意義。
[Abstract]:Objective: to observe the clinical efficacy of Shugan granule in the treatment of diabetic peripheral neuropathy with mild and moderate depression, and to explore a new method for the treatment of diabetic peripheral neuropathy, so as to improve the quality of life and alleviate the psychological pain of diabetic patients. Reduce the disability rate of the disease. Methods: 60 cases of diabetic peripheral neuropathy with mild and moderate depression were randomly divided into treatment group and control group, 30 cases in treatment group and 30 cases in control group. The control group was treated with basic hypoglycemic therapy. The patients in the control group were injected with 500ug intramuscularly. The treatment group in the control group on the basis of treatment plus Shugan granule, one bag, three times a day, boiled water. For 8 weeks, the changes of TCM symptoms, blood glucose, glycosylated hemoglobin, nerve conduction velocity and Hamd depression scale were observed. The changes of blood routine, urine routine, liver and kidney function were observed. Results: the comprehensive effective rate was 73.3 in the treatment group and 50 in the control group, which was better than that in the control group. Compared with the control group, the TCM symptoms in the treatment group were improved, and P0. 05; the blood sugar in the treatment group was improved compared with the control group, and P0. 05; the nerve conduction velocity in the treatment group was improved compared with that in the control group. The score of Hamd in the treatment group was significantly improved than that in the control group, and the score of the depression scale in the treatment group was better than that in the control group (P < 0.05). There was no significant change in the safety index before and after P0.05. It shows that Shugan granule can regulate qi and soothe liver, relieve depression and improve nerve conduction velocity, and it is safe and effective. Conclusion: Shugan granule can significantly improve the clinical symptoms, blood glucose, nerve conduction velocity, and the score of Hamilton's HAMD-depression scale in patients with diabetic peripheral neuropathy with mild and moderate depression. It has good clinical significance.
