【摘要】：目的:本文旨在應用導師的經(jīng)驗方“參夏加桂合劑”治療冠心病-穩(wěn)定型心絞痛(心陽不足、痰瘀阻絡證),并比較患者年齡、BMI等基本信息,血脂、凝血功能等血液變化,中醫(yī)癥狀分級量表、西雅圖心絞痛量表和心電圖療效等方面的變化,觀察臨床療效。方法:本課題選取符合納入標準的患者60例,并隨機分為對照組和治療組,每組30例。所有60例患者均給予西藥拜阿司匹靈、魯南欣康、立普妥基礎(chǔ)治療。治療組則在基礎(chǔ)治療上加服參夏加桂合劑,日一劑;對照組只給基礎(chǔ)治療。兩個治療組治療療程都是8周,期間如出現(xiàn)意外情況當給予對癥處理。治療前和8周后分別觀察治療組和對照組在中醫(yī)癥狀、軀體活動受限程度、心絞痛穩(wěn)定狀態(tài)、心絞痛發(fā)作狀況等方面的變化,并對血脂中總膽固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白(low density lipoprotein cholesterol,LDL-C)等指標及凝血指標進行比較。結(jié)果:治療組患者在中醫(yī)癥狀、軀體活動受限程度(PL)、心絞痛穩(wěn)定狀態(tài)(AS)、心絞痛發(fā)作情況(AF)、心電圖療效、治療滿意程度(TS)等方面改善效果良好,和對照組數(shù)據(jù)比較發(fā)現(xiàn),P0.05,具有顯著差異。在TC、TG、LDL-C、Fib及PT等血液變化方面,治療組和對照組比較,P0.05,有統(tǒng)計學意義。結(jié)論:1.參夏加桂合劑能夠有效的減少心絞痛的發(fā)作次數(shù),減輕發(fā)作時的不適癥狀,能夠更好、更全面的改善本病的發(fā)病狀態(tài)和伴隨癥狀;2.參夏加桂合劑能夠全方位的提高患者的生活質(zhì)量,更好的改善患者的發(fā)作癥狀、減少發(fā)作,從而提高日常活動耐量、增加治療滿意度;3.參夏加桂合劑可更有效地改善冠心病患者的心電圖;4.參夏加桂合劑能夠有效地調(diào)節(jié)血脂水平,降低血液粘稠度,更好地預防心絞痛的發(fā)作,更有效的控制冠心病的進展;5.參夏加桂合劑可較好的改善凝血水平。
[Abstract]:Objective: the purpose of this study was to apply "Shenxia Jiangui mixture" to treat coronary heart disease (CHD)-stable angina pectoris (deficiency of heart yang, phlegm stasis obstruction of collaterals syndrome), and to compare the basic information of age and BMI, blood lipid, coagulation function, and so on, in the treatment of coronary heart disease and stable angina pectoris. The clinical effects of TCM symptom scale, Seattle angina scale and electrocardiogram were observed. Methods: 60 patients who met the inclusion criteria were randomly divided into control group and treatment group with 30 cases in each group. All the 60 patients were treated with western medicine Beaspinone, Lunan Xinkang and Lipitor. The treatment group was treated with Shenxiajiangui mixture, and the control group was given basic treatment only. The course of treatment in both groups is 8 weeks, during which time, if an accident occurs, symptomatic treatment should be given. Before and 8 weeks after treatment, the changes of TCM symptoms, restricted physical activity, stable state of angina pectoris and angina pectoris attack were observed in the treatment group and the control group, respectively. Serum total cholesterol cholesterol (TC), triglyceride triglyceride (TG), low density lipoprotein (LDL-C) and low density lipoprotein (LDL-C) were compared. Results: in the treatment group, the symptoms of TCM, the degree of restricted somatic activity, the stable state of angina pectoris, the incidence of angina pectoris, the curative effect of electrocardiogram and the degree of satisfaction with treatment were improved well. Compared with the control group, it was found that P0. 05 had significant difference. There was significant difference between the treatment group and the control group in blood changes such as LDL-Cfib and PT. Conclusion 1. Shenxiajiangui mixture can effectively reduce the number of angina pectoris attack, relieve the symptoms of the attack, can be better, more comprehensive improvement of the state of the disease and associated symptoms. 2. Shenxiajiangui mixture can improve the quality of life of patients, improve the attack symptoms, reduce attacks, improve daily activity tolerance, and increase treatment satisfaction. Shenxia Jiangui mixture can improve electrocardiogram more effectively in patients with coronary heart disease. Shenxiajiangui mixture can effectively regulate blood lipid level, reduce blood viscosity, better prevent angina pectoris attack, more effective control of the progress of coronary heart disease. 5. Shenxia Jiangui mixture can improve coagulation level.
