【摘要】：目的:觀察桃紅四物湯加減聯(lián)合窄譜中波紫外線(NB-UVB)治療尋常型銀屑病氣滯血瘀證的臨床療效,以及治療前后患者血清IL-4及IL-22水平的變化,初步探討桃紅四物湯治療氣滯血瘀型銀屑病的作用機(jī)理。方法:將氣滯血瘀型銀屑病患者63例隨機(jī)分為治療組32例和對照組31例。治療組:桃紅四物湯加減內(nèi)服聯(lián)合NB-UVB照射,外用達(dá)力士軟膏。對照組:消銀膠囊內(nèi)服配合達(dá)力士軟膏外用。治療前后應(yīng)用ELISA法檢測血清中IL-4及IL-22兩種細(xì)胞因子的水平。結(jié)果:治療組痊愈率53.13%,總有效率為96.88%;對照組痊愈率為25.81%,總有效率為80.65%。治療組的治愈率明顯高于對照組(P0.05),治療組的總有效率也高于對照組,但沒有統(tǒng)計(jì)學(xué)意義(P0.05)。此外兩組患者起效療程比較也有顯著性差異(P0.05),顯示治療組療效好且起效快。治療組患者治療前后血清中IL-4、IL-22水平檢測,結(jié)果示:治療組患者治療前IL-4、IL-22水平較正常值明顯升高,治療后IL-4、IL-22指標(biāo)均較治療前降低(P0.05)。結(jié)論:桃紅四物湯加減內(nèi)服聯(lián)合NB-UVB照射,外用達(dá)力士軟膏治療氣滯血瘀型銀屑病較對照組治愈率高,起效快,療效肯定,并可顯著調(diào)控患者體內(nèi)IL-4及IL-22的水平,值得推廣。
[Abstract]:Objective: to observe the clinical effect of Taohong Siwu decoction combined with NB-UVB in the treatment of qi stagnation and blood stasis syndrome of psoriasis vulgaris, and the changes of serum IL-4 and IL-22 levels before and after treatment.To explore the mechanism of Taohong Siwu decoction in treating psoriasis of qi stagnation and blood stasis type.Methods: 63 cases of psoriasis with qi stagnation and blood stasis were randomly divided into treatment group (n = 32) and control group (n = 31).Treatment group: Taohong Siwu decoction plus or minus combined with NB-UVB irradiation, external use of Dalius ointment.Control group: Xiaoyin capsule combined with Dalius ointment for external use.Serum levels of IL-4 and IL-22 were measured by ELISA before and after treatment.Results: the cure rate of the treatment group was 53.13 and the total effective rate was 96.888.The cure rate of the control group was 25.81 and the total effective rate was 80.65.The cure rate of the treatment group was significantly higher than that of the control group (P 0.05), and the total effective rate of the treatment group was also higher than that of the control group, but there was no significant difference between the treatment group and the control group (P 0.05).In addition, there was a significant difference between the two groups in the effective course of treatment, which showed that the treatment group had good curative effect and rapid onset.The levels of IL-4 and IL-22 in serum of the patients in the treatment group before and after treatment were detected. The results showed that the levels of IL-4 and IL-22 in the treatment group were significantly higher than those in the normal value before and after treatment, and the indexes of IL-4 and IL-22 in the treatment group were lower than those in the control group before and after treatment (P 0.05).Conclusion: Taohong Siwu decoction plus or minus internal administration combined with NB-UVB irradiation and external application of Dalius ointment in the treatment of psoriasis of qi stagnation and blood stasis type has a higher cure rate, faster onset and better curative effect, and can significantly regulate the levels of IL-4 and IL-22 in patients, which is worth popularizing.
