【摘要】：目的:基于全國(guó)名老中醫(yī)侯玉芬治療閉塞性動(dòng)脈硬化癥(Arteriosclerosis obliterans,ASO)的療效分析,初步總結(jié)和研究名老中醫(yī)侯玉芬教授的思辨規(guī)律和用藥特點(diǎn),促進(jìn)中醫(yī)藥治療優(yōu)勢(shì)病種的臨床研究和名老中醫(yī)學(xué)術(shù)思想的傳承。方法:篩選符合氣虛血瘀型ASO研究標(biāo)準(zhǔn)的患者60例,并詳細(xì)記錄患者治療前后中醫(yī)證侯評(píng)分、踝肱比(Ankle-Brachial Index,ABI)、肢體遠(yuǎn)端血流灌注量和經(jīng)皮氧分壓以及血漿超敏C反應(yīng)蛋白和紅細(xì)胞沉降率。結(jié)果:60例患者經(jīng)治療后,結(jié)果顯示:(1)臨床癥狀和體征較治療前均有明顯改善,其顯愈率為83.33%。(2)中醫(yī)證候評(píng)分較治療前均明顯降低,具有非常顯著性差異(P0.01)。(3)ABI較治療前均明顯升高,具有非常顯著性差異(P0.01)。(4)肢體遠(yuǎn)端血流灌注量基線值、加熱后血流灌注量以及血流灌注量增加率較治療前均明顯升高,具有非常顯著性差異(P0.01)。(5)經(jīng)皮氧分壓較治療前均明顯升高,具有非常顯著性差異(P0.01)。(6)血漿超敏C反應(yīng)蛋白和紅細(xì)胞沉降率較治療前均明顯降低,具有非常顯著性差異(P0.01)。結(jié)論:名老中醫(yī)侯玉芬診治ASO,強(qiáng)調(diào)病癥結(jié)合,內(nèi)治與外治并舉,善用補(bǔ)氣活血藥物。采用名老中醫(yī)經(jīng)驗(yàn)治療氣虛血瘀型ASO,可明顯改善患者臨床癥狀和患肢血運(yùn),同時(shí)可降低血漿超敏C反應(yīng)蛋白和紅細(xì)胞沉降率,減輕病變動(dòng)脈的炎癥反應(yīng),從而提高ASO患者的生活質(zhì)量。
[Abstract]:Objective: based on the analysis of the curative effect of Hou Yufen in treating Arteriosclerosis obliterans ASO, we summarized and studied the law of thinking and the characteristics of drug use of Professor Hou Yufen, a famous old Chinese medicine, in the treatment of Arteriosclerosis obliterans.To promote the clinical research of traditional Chinese medicine (TCM) treatment of dominant diseases and the inheritance of famous Chinese medicine academic thought.Methods: 60 patients with Qi deficiency and blood stasis type ASO were selected and the scores of TCM syndromes before and after treatment were recorded in detail.Ankle-brachial Indextral ABI, distal limb blood perfusion, percutaneous partial pressure of oxygen, plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate.Results after treatment in 60 cases, the results showed that the clinical symptoms and signs were significantly improved, and the recovery rate was 83.33. 2) the scores of TCM syndromes were significantly lower than those before treatment, and the ABI was significantly higher than that before treatment (P 0.01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01).The baseline value of blood perfusion volume in distal extremity was significantly higher than that before treatment, and the blood perfusion volume and the increase rate of blood perfusion volume after heating were significantly higher than those before treatment, and the percutaneous partial pressure of oxygen was significantly higher than that before treatment.The plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate were significantly lower than those before treatment, and there was a very significant difference in plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate (P 0.01).Conclusion: Hou Yufen, a famous traditional Chinese medicine, emphasizes the combination of symptoms, internal treatment and external treatment, and makes good use of drugs for reinforcing qi and activating blood circulation.The treatment of Qi deficiency and blood stasis type ASO by the experience of famous old Chinese medicine can obviously improve the clinical symptoms and blood circulation of the affected limbs, at the same time, it can reduce the plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate, and alleviate the inflammatory reaction of the diseased arteries.So as to improve the quality of life of patients with ASO.
