【摘要】：目的:通過對古代醫(yī)案中產(chǎn)后痹的發(fā)病誘因、癥狀體征、所用方藥進行研究,分析探討產(chǎn)后痹證型與用藥規(guī)律。方法:采用理論研究和醫(yī)案研究相結(jié)合的方法。1.理論研究:結(jié)合近年相關(guān)書籍及文獻,探討產(chǎn)后痹的病因病機與治療規(guī)律的關(guān)系。2.醫(yī)案研究:查詢檢索電子出版軟件《中華醫(yī)典》醫(yī)案部分,并利用Excle,Spss軟件,對符合納入標準的106例產(chǎn)后痹醫(yī)案進行整理分析,將其分為不同證型,總結(jié)癥狀及用藥規(guī)律。結(jié)果:1.理論研究發(fā)現(xiàn)產(chǎn)后體虛,邪氣侵犯是產(chǎn)后痹的主要病機,驅(qū)邪扶正是產(chǎn)后痹的主要治法。產(chǎn)后痹可分為五種證型,分別為氣血虧虛證、肝腎虧虛證、外邪侵襲證、瘀血阻絡(luò)證、肝氣不舒證五個證型。2.醫(yī)案研究利用Excle軟件及Spss軟件的數(shù)據(jù)分析功能,分別得出:2.1產(chǎn)后痹的誘因頻次:癥狀頻次感寒2次、產(chǎn)育多胎2次、產(chǎn)后過勞2次等。2.2產(chǎn)后痹癥狀主要為:肢節(jié)疼痛48次、活動受限27次、腰酸痛27次、四肢麻木21次等。2.3產(chǎn)后痹的藥物頻次:當歸68次、茯苓47次、白芍33次、桂枝26次等。2.4產(chǎn)后痹藥物分類:補虛藥37種、解表藥18種、清熱藥22種、活血化瘀藥14種等。結(jié)論1.產(chǎn)后痹的主要病機是經(jīng)脈痹阻不通;產(chǎn)后痹的主要治法是扶正祛邪通絡(luò)。2.產(chǎn)后痹的證型有:氣血虧虛型、肝腎虧虛型、外邪侵襲型、瘀血阻絡(luò)型、肝氣不舒型。3.產(chǎn)后痹的發(fā)病誘因有感寒、產(chǎn)育多胎、產(chǎn)后過勞等。4.產(chǎn)后痹的常見癥狀主要是肢節(jié)疼痛、活動受限、腰酸痛、四肢麻木等。5.產(chǎn)后痹的常用藥物主要有當歸、茯苓、白芍、桂枝等。6.藥物分類有補虛藥、解表藥、清熱藥、活血化瘀藥等。
[Abstract]:Objective: to study the causes, symptoms and signs of postpartum arthralgia in ancient medical records, and to analyze the pattern of postpartum arthralgia syndrome and its drug use.Methods: the method of combining theoretical research with medical case study was used.Theoretical study: combined with relevant books and literature in recent years, to explore the etiology and pathogenesis of postpartum arthralgia and the relationship between treatment law. 2.Medical records research: the part of medical records of electronic publishing software "Chinese Medical Code" was searched, and 106 cases of postpartum arthralgia cases which were included in the standard were sorted out and analyzed by using the software of Excle-Spss, which were divided into different syndromes, symptoms and drug use rules.The result is 1: 1.Theoretical study found that postpartum body deficiency, invasion of evil qi is the main pathogenesis of postpartum arthralgia, exorcism is the main treatment of postpartum arthralgia.Postpartum arthralgia can be divided into five syndromes: deficiency of qi and blood, deficiency of liver and kidney, invasion of external evil, stagnation of blood and collaterals, and deficiency of liver qi.Based on the data analysis function of Excle software and Spss software, the results of medical record study showed that the inducement frequency of postpartum arthralgia was as follows: frequency of symptoms, frequency of cold, 2 times of birth and birth, and 2 times of postpartum overwork. 2.2 the main postpartum symptoms were: limb pain 48 times.Activity restriction 27 times, lumbar soreness 27 times, limb numbness 21 times. 2.3 drugs frequency of postpartum arthralgia: Angelica sinensis 68 times, Poria cocos 47 times, white peony arthralgia 33 times, Guizhi 26 times .2.4 postpartum medicine classification: tonifying and deficiency drugs 37, antitable drugs 18, heat clearing drugs 22,14 kinds of drugs for promoting blood circulation and removing blood stasis.Conclusion 1.The main pathogenesis of postpartum arthralgia is the obstruction of the meridian, and the main treatment of postpartum arthralgia is to strengthen the qi, dispel the evil and unblock the collaterals. 2.The syndrome types of postpartum arthralgia are: deficiency of qi and blood, deficiency of liver and kidney, invasion of external evil, obstruction of collaterals by blood stasis, and deficiency of liver-qi.The causes of postpartum arthralgia are cold, labor and labor.The common symptoms of postpartum arthralgia are limb pain, limited movement, pain in the waist, numbness of limbs, and so on.The commonly used drugs for postpartum arthralgia are angelica sinensis, Poria cocos, Radix Paeoniae Alba, Guizhi branch and so on.Drugs are classified as tonifying drugs, antitabular drugs, heat-clearing drugs, blood-activating drugs and so on.
