【摘要】：目的:本文觀察“疏肝理氣、活血化瘀”為治法的“化瘀止痛散”治療內(nèi)異癥的臨床療效與作用機(jī)理。方法:臨床研究將60例氣滯血瘀型子宮內(nèi)膜異位癥患者隨機(jī)分兩組,治療組30例,用化瘀止痛散治療;對照組30例,用達(dá)那唑治療。分別觀察兩組患者治療前后中醫(yī)證候、臨床癥狀、體征、婦科檢查、陰道B超(子宮動(dòng)脈血流動(dòng)力學(xué))以及血清CA125。動(dòng)物實(shí)驗(yàn)研究分別觀察化瘀止痛散和西藥達(dá)那唑?qū)M模型鼠的血清CA125水平、骨橋蛋白(OPN),血管內(nèi)皮生長因子(VEGF),高遷移率族蛋白B1(HMGB1),可溶性B7同源體4(sB7-H4),腫瘤壞死因子α(TNF-α)的影響。結(jié)果:臨床研究結(jié)果“化瘀止痛散”治療本病臨床療效總有效率為86.7%,其中痊愈率為26.7%,顯效率為36.7%,有效率為23.3%,同時(shí)臨床癥狀和體征也得到了改善,并且顯示本方可以改善患者的血液流變性、盆腔微循環(huán)情況,血清CA125水平的也明顯降低。動(dòng)物實(shí)驗(yàn)研究表明“化瘀止痛散”降低了模型大鼠的各項(xiàng)指標(biāo)的值,并抑制了異位內(nèi)膜的異常增生。結(jié)論:“化瘀止痛散”對EM療效顯著,可以通過糾正患者的血瘀的狀態(tài),從而使盆腔微循環(huán)得以改善;降低EM模型大鼠各項(xiàng)指標(biāo),并能抑制異位子宮內(nèi)膜的異常生長。
[Abstract]:Objective: to observe the clinical effect and mechanism of "Huayu Zhitong Powder" in treating internal heterosis with "soothing liver and regulating qi and activating blood circulation and removing blood stasis".Methods: 60 cases of endometriosis with Qi stagnation and blood stasis type were randomly divided into two groups: treatment group (30 cases) treated with Huayu Zhitong Powder and control group (30 cases) treated with danazol.The TCM syndromes, clinical symptoms, signs, gynecological examination, vaginal ultrasound (uterine artery hemodynamics) and serum CA125 were observed before and after treatment.The effects of Huayu Zhitong Powder (Huayu Zhitong Powder) and danazol on serum CA125 level, osteopontin (OPN), vascular endothelial growth factor (VEGF), high mobility group protein B1 (HMGB1), soluble B7 homologue 4 (sB7-H4) and tumor necrosis factor 偽 (TNF- 偽) in EM model rats were observed in animal experiments.Results: the total effective rate of Huayu Zhitong Powder in the treatment of this disease was 86.7, in which the cure rate was 26.7, the effective rate was 36.7 and the effective rate was 23.3.The clinical symptoms and signs were also improved.It also showed that it could improve the hemorheology, pelvic microcirculation and serum CA125 level.Animal experiments showed that Huayu Zhitong San decreased the values of various indexes and inhibited the abnormal proliferation of ectopic endometrium.Conclusion: Huayu Zhitong Powder has a remarkable curative effect on EM, which can improve the pelvic microcirculation by correcting the state of blood stasis in patients, decrease the indexes of EM model rats, and inhibit the abnormal growth of ectopic endometrium.
