【摘要】：目的:探討益氣化瘀解毒方對(duì)慢性萎縮性胃炎癌前病變的療效及對(duì)細(xì)胞周期蛋白E表達(dá)和腫瘤標(biāo)記物的影響。方法:選擇2015年1月—2016年8月在該院各分院門(mén)診及住院的60例確診為脾虛濕瘀型萎縮性胃炎伴腸上皮或不典型增生患者為研究對(duì)象,按照隨機(jī)數(shù)字表法分為對(duì)照組(30例)和觀察組(30例),對(duì)照組給以胃復(fù)春治療,觀察組予口服益氣化瘀解毒方,24周后觀察臨床療效、中醫(yī)證候評(píng)分、胃鏡及病理,免疫組化法檢測(cè)細(xì)胞周期蛋白E(Cyclin E)、E-鈣黏著蛋白(E-cadherin)和基質(zhì)金屬蛋白酶-9(MMP-9)蛋白表達(dá),酶聯(lián)免疫方法檢測(cè)血清胃蛋白酶(PGⅠ和PGⅡ)。結(jié)果:治療6個(gè)療程后,觀察組總有效率93.33%高于對(duì)照組的66.67%(χ2=6.667,P=0.010);觀察組的胃痛、納差、乏力、噯氣、便溏中醫(yī)證候積分均低于對(duì)照組(P0.05);胃鏡下及病理組織學(xué)檢測(cè)觀察組胃黏膜萎縮、腸化及異型增生、感染和慢性炎癥、糜爛、膽汁反流均優(yōu)于對(duì)照組(P0.05);觀察組患者的胃組織Cyclin E和MMP-9蛋白表達(dá)低于對(duì)照組,而E-cadherin蛋白表達(dá)高于對(duì)照組(P0.05);觀察組患者的PGⅠ和PGⅡ含量高于對(duì)照組(P0.05)。結(jié)論:益氣化瘀解毒方對(duì)慢性萎縮性胃炎癌前病變具有較好的臨床療效,可降低中醫(yī)證候積分,降低細(xì)胞周期蛋白E表達(dá),提高血清胃蛋白酶水平,改善患者病情進(jìn)展。
[Abstract]:Objective: to investigate the effect of Yiqi Huayu jiedu recipe on precancerous lesions of chronic atrophic gastritis and on the expression of cyclin E and tumor markers.Methods: from January 2015 to August 2016, 60 patients with atrophic gastritis with intestinal epithelium or atypical hyperplasia of spleen deficiency and dampness stasis were selected.According to the random number table method, 30 cases were divided into control group (n = 30) and observation group (n = 30).
