【摘要】：目的:觀察祛瘀解毒法治療中醫(yī)辨證血瘀蘊(yùn)毒型子宮內(nèi)膜異位癥的臨床療效。方法:按照隊(duì)列研究設(shè)計(jì),篩選31例未服用祛瘀解毒方而直接經(jīng)腹腔鏡手術(shù)確診的血瘀蘊(yùn)毒型子宮內(nèi)膜異位癥(對(duì)照組31例),34例前期服用祛瘀解毒方而隨后經(jīng)腹腔鏡手術(shù)確診的血瘀蘊(yùn)毒型子宮內(nèi)膜異位癥(觀察組34例),同期篩選15例因不明原因性不孕癥而行腹腔鏡手術(shù)探查,未見(jiàn)盆腔明顯異常的患者(空白組15例)。將觀察組按照前期服用祛瘀解毒方時(shí)間長(zhǎng)度,分為1個(gè)月、2個(gè)月、3個(gè)月、4個(gè)月、5個(gè)月、6個(gè)月組,比較各組患者腹腔鏡手術(shù)中收集腹腔液中的IL-6、IL-13水平。結(jié)果:空白組腹腔液中IL-6濃度(20.73±9.86)pg/m L,低于觀察組(106.83±34.64)pg/m L和對(duì)照組(125.04±49.53)pg/m L,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組腹腔液中IL-6濃度低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。空白組腹腔液中IL-13濃度(9.51±4.37)pg/m L,高于觀察組(4.52±3.24)pg/m L和對(duì)照組(3.96±2.01)pg/m L,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組腹腔液中IL-13濃度,高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組1個(gè)月、2個(gè)月、3個(gè)月、4個(gè)月、5個(gè)月、6個(gè)月組IL-6濃度(pg/m L),分別為115.05±73.19、113.80±67.39、86.83±31.65、98.47±43.58、119.52±60.37、107.31±57.49,1個(gè)月組高于另外5個(gè)組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3、4個(gè)月組低于2、5、6個(gè)月組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3、4個(gè)月組,組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);2、5、6個(gè)月組,組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組1個(gè)月、2個(gè)月、3個(gè)月、4個(gè)月、5個(gè)月、6個(gè)月組IL-13濃度(pg/m L),分別為4.33±3.17、5.39±4.73、5.08±4.03、4.05±2.32、3.79±2.19、4.48±3.71,2、3個(gè)月組高于1、4、5、6個(gè)月組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2、3個(gè)月組,組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);1、4、5、6個(gè)月組,組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:祛瘀解毒法可影響中醫(yī)辨證為血瘀蘊(yùn)毒型子宮內(nèi)膜異位癥患者腹腔液IL-6、IL-13水平,通過(guò)抑制非特異性炎癥的病理生理機(jī)制,改善子宮內(nèi)膜異位癥患者盆腔微環(huán)境,可能是治療中醫(yī)辨證為血瘀蘊(yùn)毒型子宮內(nèi)膜異位癥的有效方法。
[Abstract]:Objective: to observe the clinical effect of removing blood stasis and detoxification in treating endometriosis with syndrome differentiation of blood stasis and toxin.Methods: according to the design of cohort study,Screening 31 cases of endometriosis with blood stasis and toxin accumulation which were diagnosed by laparoscopy without taking Quyu jiedu recipe (control group 31 cases were treated with Quyu jiedu recipe and then confirmed by laparoscopic surgeryEndometriosis (34 cases in the observation group, 15 cases in the same period were selected for laparoscopy because of unexplained infertility),There was no obvious abnormality in pelvic cavity (15 cases in blank group).Results: the concentration of IL-6 in peritoneal fluid of blank group was 20.73 鹵9.86)pg/m L, which was lower than that of observation group (106.83 鹵34.64)pg/m / L) and control group (125.04 鹵49.53)pg/m / L). The difference was statistically significant (P 0.05).The concentration of IL-6 in the peritoneal fluid in the observation group was lower than that in the control group, and the difference was statistically significant (P 0.05).The concentration of IL-13 in peritoneal fluid of blank group was 9.51 鹵4.37)pg/m / L, which was higher than that of observation group (4.52 鹵3.24)pg/m / L) and control group (3.96 鹵2.01)pg/m / L).
