【摘要】：目的:通過觀察五行人體質(zhì)針刺療法治療卒中后抑郁癥的臨床療效,初步為卒中后抑郁癥的治療提供一個(gè)安全有效的治療技術(shù)。方法:將150例卒中后抑郁患者隨機(jī)分為五行組、針刺對(duì)照組、西藥對(duì)照組各50例,三組均給予腦卒中常規(guī)治療,西藥對(duì)照組口服鹽酸帕羅西汀,針刺對(duì)照組在西藥對(duì)照組基礎(chǔ)上予常規(guī)針刺治療,五行組在西藥對(duì)照組基礎(chǔ)上采用五行針刺法治療,三組均治療4周。采用漢密爾頓抑郁量表(HAMD)、神經(jīng)功能缺損量表評(píng)估三組療效的差別。結(jié)果:1、三組治療前后的HAMD評(píng)分、神經(jīng)功能缺損評(píng)分分別進(jìn)行比較,差異均有統(tǒng)計(jì)學(xué)差異(P0.05)。2、治療1周后,三組的HAMD評(píng)分、神經(jīng)功能評(píng)分進(jìn)行組間比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。3、治療4周后,三組HAMD評(píng)分、神經(jīng)功能評(píng)分進(jìn)行組間比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。4、3個(gè)月后隨訪,三組HAMD評(píng)分、神經(jīng)功能評(píng)分進(jìn)行組間比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。5、三組治療后抑郁減分率和神經(jīng)功能減分率的總有效率分別為五行組87.75%、79.59%,針刺對(duì)照組83.33%、70.83%,西藥對(duì)照組68.09%、65.96%,三組抑郁減分率和神經(jīng)功能減分率分別比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。6、五行組、常規(guī)針刺組未發(fā)現(xiàn)不良反應(yīng),西藥組出現(xiàn)6例胃腸道不良反應(yīng)。結(jié)論:五行針刺治療PSD臨床療效顯著,且未發(fā)現(xiàn)不良反應(yīng),值得推廣應(yīng)用。
[Abstract]:Objective: to provide a safe and effective treatment technique for post-stroke depression by observing the clinical effect of five-pedestrian physical acupuncture therapy on post-stroke depression. Methods: 150 patients with post-stroke depression were randomly divided into five groups. Acupuncture control group (50 cases) and western medicine control group (50 cases) were treated with routine treatment of stroke. The western medicine control group took paroxetine hydrochloride orally, and the acupuncture control group received routine acupuncture treatment on the basis of western medicine control group. The five elements group was treated with five elements acupuncture method on the basis of western medicine control group, and all of the three groups were treated for 4 weeks. The difference of curative effect between the three groups was evaluated by Hamilton Depression scale (Hamd) and nerve function deficit scale. Results the HAMD scores of the three groups before and after treatment were evaluated. After 1 week of treatment, the HAMD score and the neurological function score of the three groups were compared. There was no significant difference between the three groups (P0.05. 3). After 4 weeks of treatment, the HAMD scores of the three groups were not statistically significant. The neurological function scores were compared among the groups, the differences were statistically significant (P 0.05). 4. After 3 months follow-up, the HAMD scores and neurological function scores of the three groups were compared among the groups. The total effective rates of depression reduction rate and neural function reduction rate in the three groups after treatment were 87.75 and 79.59, respectively; the acupuncture control group was 83.33 and 70.833; the western medicine control group was 68.09; the depression score reduction rate and the nerve function reduction rate were respectively compared among the three groups. The differences were statistically significant (P 0.05). 6. There were no adverse reactions in the five elements group and six cases in the western medicine group. Conclusion: the clinical curative effect of five elements acupuncture on PSD is remarkable, and no side effects are found, so it is worth popularizing and applying.
