【摘要】：目的:通過對謝晶日教授治療的腹瀉型腸易激綜合征患者用藥統(tǒng)計分析,探討謝晶日教授治療本病的用藥規(guī)律。方法:收集自2011年1月至2016年12月期間于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院謝晶日教授門診就診,經(jīng)謝晶日教授診治的腹瀉型腸易激綜合征患者詳細(xì)的病歷資料。使用Microsoft Excel建立數(shù)據(jù)庫,進(jìn)行頻數(shù)分析,得出高頻中藥,常用的藥類、四氣、五味及歸經(jīng);應(yīng)用IBMSPSSStatistics18.0軟件進(jìn)行聚類分析,得出常用藥對及藥物組合。通過統(tǒng)計結(jié)果,探討謝晶日教授治療腹瀉型腸易激綜合征臨床用藥規(guī)律。結(jié)果:根據(jù)納入標(biāo)準(zhǔn)收集得到168例患者處方,統(tǒng)計得出治療本病所有涉及中藥115味,總使用頻數(shù)為4013次;其中使用頻率在50次以上的藥物為32味,累計頻率達(dá)74.36%,其中柴胡、白芍、薏苡仁、防風(fēng)、山藥、白術(shù)、茯苓、白扁豆、陳皮、黨參、甘草、黃芪、砂仁、蒼術(shù)、五味子、補骨脂、訶子、肉豆蔻、吳茱萸為治療本病的核心用藥。涉及用藥種類14類,其中補虛藥頻率最多,其次為收澀藥、理氣藥。統(tǒng)計得出藥物的四氣中以溫性藥使用為最多,其余依次為平性、寒性;藥味中的前三位是甘味、苦味、辛味;歸經(jīng)前四位歸經(jīng)為脾經(jīng)、肺經(jīng)、胃經(jīng)、肝經(jīng)。經(jīng)聚類分析得到9對常用藥對和6組藥物的常用組合。結(jié)論:1.柴胡、白芍、薏苡仁、防風(fēng)、山藥、白術(shù)、茯苓、白扁豆、陳皮、黨參、甘草、黃芪、砂仁、蒼術(shù)、五味子、補骨脂、訶子、肉豆蔻、吳茱萸為謝晶日教授治療腹瀉型腸易激綜合征的核心藥物。2.補虛藥、收澀藥、理氣藥為謝晶日教授治療腹瀉型腸易激綜合征的常用藥物類別。3.甘味、苦味、辛味;溫性、平性;脾經(jīng)、肺經(jīng)、胃經(jīng)、肝經(jīng)為謝晶日教授治療腹瀉型腸易激綜合征用藥的主要性味與歸經(jīng)。4.疏肝健脾化濕,溫腎固澀是謝晶日教授治療腹瀉型腸易激綜合征的基本方法。
[Abstract]:Objective: to analyze the drug use in patients with diarrhea irritable bowel syndrome treated by Professor Xie Jingri. Methods: from January 2011 to December 2016, Professor Xie Jingri, Professor of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine, was selected to visit the clinic. The detailed medical records of the patients with diarrhea irritable bowel syndrome treated by Professor Xie Jingri. The database was established by Microsoft Excel, and the frequency analysis was carried out. The high frequency traditional Chinese medicine, the commonly used drugs, the four qi, five flavors and the return of the meridian were obtained. By cluster analysis with IBMSPSSStatistics18.0 software, the common drug pairs and drug combinations were obtained. Through the statistical results, Professor Xie Jingri was studied for clinical drug use in the treatment of diarrhea irritable bowel syndrome. Results: according to the inclusive criteria, 168 prescriptions were obtained. Statistics show that all the drugs involved in the treatment of this disease are 115 kinds of Chinese medicine, the total frequency of use is 4013 times; among them, 32 kinds of drugs are used more than 50 times, and the cumulative frequency is 74.36. Among them, Bupleurum, Radix Paeoniae Paeoniae, Coix lachryma seed, Astragalus, yam, Atractylodes macrocephala, Poria cocos, White lentils, Chen Pei, Codonopsis, licorice, Astragalus, Amomum, Atractylodes, Schisandra, Psoralea, Chekophyllum, nutmeg and Evodia rutaecarpa are the core drugs for the treatment of this disease. There are 14 kinds of drugs involved in the treatment, among which tonifying and deficiency drugs are the most frequent, followed by astringent drugs. According to statistics, among the four kinds of drugs, warm drugs are the most used, and the rest are flat and cold; the first three places of medicinal taste are sweet, bitter and Xin Wei; the first four of them are spleen, lung, stomach, and so on, and the first four are spleen meridian, lung meridian and stomach meridian. Conclusion: 1. Radix Bupleurum, Radix Paeoniae Alba, Coix seed, Fangfeng, yam, Atractylodes macrocephala, Poria cocos, White lentils, Old Peel, Codonopsis, Glycyrrhiza, Astragalus, Amomum, Atractylodes, Atractylodes Atractylodes. Schisandra, psoralen, chebulae, nutmeg, Evodia rutaecarpa are the core drugs used by Xie Jingri to treat diarrhea irritable bowel syndrome. Qi regulating medicine is the common medicine category of Professor Xie Jingri's treatment of diarrhea irritable bowel syndrome .3.Sweet, bitter, symplectic, warm, flat, spleen, lung, stomach, The liver meridian is the main characteristic taste of professor Xie Jingri's treatment of diarrhea irritable bowel syndrome. 4. Soothing liver and invigorating spleen and transforming dampness and warming kidney and astringement are the basic methods of Professor Xie Jingri's treatment of diarrhea irritable bowel syndrome.
