【摘要】：研究目的:研究足三里穴位注射參附注射液對結(jié)直腸癌化療后脾腎陽虛型白細(xì)胞減少癥的療效,探討采用穴位注射的治療手段,以健脾益腎為主要治療法則,治療結(jié)直腸癌化療后脾腎陽虛型白細(xì)胞減少癥的治療方法。為中西醫(yī)結(jié)合治療結(jié)直腸癌化療后脾腎陽虛型白細(xì)胞減少癥治療提供新的思路。研究方法:隨機(jī)將72例結(jié)直腸癌化療后脾腎陽虛型白細(xì)胞減少癥的患者,分配至治療組(足三里穴位注射參附注射液1ml,每日1次)與對照組(口服十一味參芪片,每次4片,每日3次)。評價(jià)治療前后的證候積分、白細(xì)胞及中性粒細(xì)胞計(jì)數(shù)的改善情況并進(jìn)行統(tǒng)計(jì)學(xué)分析。研究結(jié)果:1.兩組治療手段均可以降低臨床中醫(yī)證候積分(P0.01)。在治療第4、7天治療組的證候積分明顯低于對照組(P0.01)。2.治療組的療效在治療第4天明顯優(yōu)于對照組(P0.01)。治療組的療效在治療第7天仍優(yōu)于對照組(P0.05)。3.兩組治療手段均可以升高血清白細(xì)胞計(jì)數(shù)(P0.01)。在治療第4、7天治療組的白細(xì)胞計(jì)數(shù)均明顯高于對照組(P0.01)。4.兩組治療手段均可以升高血清中性粒細(xì)胞計(jì)數(shù)(P0.01)。在治療第4天兩組的中性粒細(xì)胞計(jì)數(shù)并無差異(P0.05);在治療第7天治療組的中性粒細(xì)胞計(jì)數(shù)高于對照組(P0.05)。5.在治療第4天治療組的實(shí)驗(yàn)室療效優(yōu)于對照組(P0.05);在治療第7天治療組的實(shí)驗(yàn)室檢查療更是效優(yōu)于對照組(P0.01)。6.兩組治療手段均可以提升Karnofsky評分分?jǐn)?shù)(P0.01)。在治療后治療組的Karnofsky評分分?jǐn)?shù)明顯高于對照組(P0.01)。研究結(jié)論:本研究結(jié)果顯示足三里穴位注射參附注射液治療結(jié)直腸癌化療后脾腎陽虛型白細(xì)胞減少癥,在中醫(yī)證候的改善、白細(xì)胞和中性粒細(xì)胞計(jì)數(shù)的升高、Karnofsky評分的提高等方面明顯優(yōu)于口服十一味參芪片組,且具有統(tǒng)計(jì)學(xué)意義。說明采用足三里穴位注射參附注射液的治療方法,可改善臨床癥狀,提高生活質(zhì)量,有效治療白細(xì)胞減少癥,提高化療的耐受性。
[Abstract]:Objective: to study the therapeutic effect of Shenfu injection at Zusanli acupoint on leucopenia of spleen and kidney yang deficiency type after chemotherapy in colorectal cancer. The treatment method of leukopenia of spleen and kidney yang deficiency type after chemotherapy for colorectal cancer provides a new way of thinking for the treatment of leukopenia of spleen and kidney yang deficiency type after chemotherapy of colorectal cancer by combining traditional Chinese and western medicine. A case of leukopenia with deficiency of spleen and kidney yang after chemotherapy for colorectal cancer, To the treatment group (Zusanli acupoint injection of Shenfu injection 1 ml, once a day) and the control group (oral 10 blindly Shenqi tablets, 4 tablets each time, 3 times a day). The syndrome score before and after treatment was evaluated. The improvement of leukocyte and neutrophil count was analyzed statistically. The results showed that both treatment methods could decrease the clinical TCM syndrome score (P 0.01). The syndromes score in the treatment group was significantly lower than that in the control group on the 4th day of treatment. The therapeutic effect of the treatment group was significantly better than that of the control group on the 4th day. The therapeutic effect of the treatment group was still better than that of the control group on the 7th day. WBC count was significantly higher than that of control group (P 0.01). Both groups could increase serum neutrophil count (P 0.01). There was no difference in neutrophil count between the two groups on the 4th day of treatment, and the neutrophil count in the treatment group on the 7th day. The cell count was higher than that of the control group (P 0.05). The laboratory efficacy of the treatment group was better than that of the control group on the 4th day, and that of the treatment group on the 7th day was even better than that of the control group (P 0.01. 6. 6). Both treatment methods could improve the score of Karnofsky score. The Karnofsky score of the treatment group was significantly higher than that of the control group (P 0.01). Conclusion: the results of this study showed that injection of Shenfu injection at Zusanli acupoint was effective in the treatment of leukopenia with deficiency of spleen and kidney yang after chemotherapy for colorectal cancer. The improvement of TCM syndromes, the increase of leukocyte and neutrophil count and the improvement of Karnofsky score were obviously superior to those in the oral Shenqi tablet group, and had statistical significance. It can improve the clinical symptoms, improve the quality of life, effectively treat leukopenia, and improve the tolerance of chemotherapy.
