【摘要】：目的探討復(fù)元飲聯(lián)合局部氨甲環(huán)酸(TXA)對(duì)全髖關(guān)節(jié)置換(THA)術(shù)后隱性失血及快速康復(fù)的影響。方法采用隨機(jī)數(shù)字表法將124例擬行全髖關(guān)節(jié)置換術(shù)患者隨機(jī)分為兩組:試驗(yàn)組62例,術(shù)前2 d及術(shù)后6 h后口服我院中藥制劑復(fù)元飲;對(duì)照組62例,于術(shù)前30 min靜滴TXA 1 g;兩組患者術(shù)中均局部使用TXA 1g。觀察并比較兩組患者術(shù)前、術(shù)后第1、3、7天血紅蛋白值(HB)、紅細(xì)胞比容(HCT)值;術(shù)中出血量、術(shù)后引流量、術(shù)后第1、3、7天隱性失血量;整體康復(fù)評(píng)分;不良事件包括輸血率、深靜脈血栓形成(DVT)、肺栓塞(PE)發(fā)生率。結(jié)果兩組顯性失血及術(shù)后隱性失血量相近,差異均無統(tǒng)計(jì)學(xué)意義(P0.05),但試驗(yàn)組的輸血率、DVT發(fā)生率更低,且整體康復(fù)評(píng)分更優(yōu),差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論復(fù)元飲聯(lián)合術(shù)中局部使用TXA能有效控制全髖關(guān)節(jié)置換術(shù)后隱性失血,降低輸血率,減少DVT發(fā)生率,且能促進(jìn)患者早期整體康復(fù),值得臨床應(yīng)用。
[Abstract]:Objective to investigate the effect of Fuyuan Yin combined with local aminoformic acid (TXA) on recessive blood loss and rapid recovery after total hip replacement. Methods 124 patients with total hip replacement were randomly divided into two groups: experimental group (n = 62), total hip replacement (THA) group (n = 62), total hip replacement (THA) group (n = 62) and total hip replacement (THA) group (n = 62). Two days before operation and 6 hours after operation, Fuyuan Yin, a traditional Chinese medicine preparation, was taken orally in our hospital; in the control group, 62 cases were given intravenous drip of TXA 1 g 30 min before operation. The patients in both groups were given TXA 1g locally during operation. The hemoglobin value and HCT value on the 1st day and 3rd day after operation, the volume of blood loss during operation, the volume of drainage after operation, the amount of recessive blood loss on the 1st day and the 7th day after operation, the overall rehabilitation score, the adverse events including the rate of blood transfusion, Results there was no significant difference between the two groups in the amount of dominant blood loss and recessive blood loss after operation, but the rate of blood transfusion and DVT in the trial group was lower than that in the control group, and the overall rehabilitation score was better than that in the control group, and there was no significant difference between the two groups in the incidence of DVT and PE.Results there was no significant difference between the two groups. Conclusion Fuyuan Yin combined with local use of TXA during operation can effectively control recessive blood loss after total hip arthroplasty, reduce blood transfusion rate, reduce the incidence of DVT, and promote the early recovery of patients, which is worthy of clinical application.
