【摘要】：目的:探討俞募配穴法針灸治療慢性喘息型支氣管炎患者的臨床療效,為針灸治療本病推廣于臨床提供依據(jù)。方法:按照隨機(jī)原則,將符合本方案納入標(biāo)準(zhǔn)的80例患者平分為治療組和對照組,每組40例。治療組采用俞募配穴的針灸方法治療,第一組穴位處方以中府為基礎(chǔ),加天突、膻中、列缺;第二組穴位處方以肺俞為基礎(chǔ),加膏肓俞、腎俞,兩組處方隔日交替使用,20次為一療程,觀察2個療程,并比較每個療程前后的療效變化。對照組予抗感染、止咳化痰平喘等常規(guī)西藥治療,其中抗感染藥根據(jù)癥狀和實驗室檢查結(jié)果及時停藥或更換用藥,其余用藥至治療組針灸療程結(jié)束,癥狀好轉(zhuǎn)者根據(jù)病情酌情減藥或停藥。比較治療前后兩組患者的臨床療效、肺功能、IL-4、ECP、IgE、IFN-γ等血清相關(guān)因子及SGRQ評分。結(jié)果:1.治療后,治療組總有效率達(dá)97.5%,對照組總有效率為87.5%,兩組數(shù)據(jù)經(jīng)卡方檢驗P0.05,差異具有統(tǒng)計意義。2.經(jīng)治療后,兩組患者FEV1、FEV1/FVC、PEF均有顯著改善(p0.05),治療組改善程度優(yōu)于對照組(P0.05);血清相關(guān)因子IL-4、ECP、IgE、IFN-γ等水平治療組改善程度明顯優(yōu)于對照組(P0.05);兩組患者治療后在呼吸癥狀、日常生活影響和社會心理影響方面SGRQ評分均明顯下降,且治療組較對照組下降顯著,差異有統(tǒng)計意義(P0.05)。結(jié)論:慢性喘息型支氣管炎遷延期患者采用俞募配穴針灸治療,其臨床療效優(yōu)于常規(guī)西藥治療,且能有效改善\能,降低血清致炎因子水平,提高患者生活質(zhì)量,值得臨床推廣。
[Abstract]:Objective: to explore the clinical effect of acupuncture and moxibustion on chronic asthmatic bronchitis with Shu-fu-combined acupoint method, and to provide the basis for the clinical application of acupuncture and moxibustion in the treatment of chronic asthmatic bronchitis. Methods: according to the random principle, 80 patients who met the standard of this scheme were divided into treatment group and control group with 40 cases in each group. In the second group, the acupoint prescription was based on Feshu, and the two groups were treated alternately 20 times every other day as a course of treatment. The therapeutic effects before and after each course of treatment were observed and compared. The control group was given anti-infection. In the treatment of cough, phlegm and asthma, and other routine western medicines, the anti-infective drugs were stopped or changed in time according to the symptoms and the results of laboratory tests. The rest of the drugs were treated to the end of the course of acupuncture and moxibustion in the treatment group. The patients with improved symptoms were treated with drug reduction or withdrawal according to their condition. The clinical effects of the two groups before and after treatment were compared, and the serum related factors such as IL-4 / ECPE / Ig IFN- 緯 and SGRQ scores were compared. Results: 1. After treatment, The total effective rate of the treatment group was 97.5, and the total effective rate of the control group was 87.5. The data of the two groups were tested by chi-square test (P0.05), the difference being statistically significant. The improvement of FEV1 / FEV1 / FVCU PEF in both groups was significantly better than that in the control group (P 0.05), and the level of serum IL-4 ECPP IgEIFN- 緯 in the treatment group was significantly better than that in the control group (P 0.05). The SGRQ scores of daily life and psychosocial effects were significantly lower in the treatment group than in the control group (P 0.05). Conclusion: the patients with chronic wheezing bronchitis were treated with acupuncture and moxibustion. The clinical curative effect is better than that of conventional western medicine, and it can effectively improve the level of serum inflammatory factor and improve the quality of life of patients. It is worth popularizing in clinic.
