超聲下觀察電針針刺外金津及外玉液對額舌肌收縮的影響

【摘要】:目的:超聲下應(yīng)用電針不同波形刺激外金津及外玉液,觀察頦舌肌收縮的即刻效應(yīng)。方法:應(yīng)用序貫試驗設(shè)計,將14例健康受試者隨機分為A、B兩組,A組為先給予電針連續(xù)波(頻率為30Hz)刺激外金津、外玉液腧穴,經(jīng)過3天的洗脫期后再給予電針斷續(xù)波(斷續(xù)比為1:1)刺激外金津、外玉液腧穴,在超聲下檢測頦舌肌的肌肉厚度,以評價頦舌肌收縮的強度。B組為先給予電針斷續(xù)波(斷續(xù)比為1:1)刺激,經(jīng)過3天的洗脫期后,再給予電針連續(xù)波(頻率為30Hz)刺激。選穴及檢測方法同A組。綜合兩組數(shù)據(jù),對比分析,應(yīng)用開放型雙側(cè)序貫t檢驗進行14例受試者的數(shù)據(jù)分析,頦舌肌收縮的強度通過超聲下測量受試者仰臥位頦舌肌的厚度來評價。結(jié)果:1.在超聲下引導(dǎo)針灸針刺外金津、外玉液,均向?qū)?cè)耳根方向進針,兩針針尖方向夾角約為90°;對于針灸進針的深度,在沿外金津、外玉液針刺頦舌肌時,當(dāng)針灸針刺入腧穴后有鏤空感,再次感到阻力時,為刺中頦舌肌,在超聲下得到證實。2.電針連續(xù)波與電針斷續(xù)波針刺外金津、外玉液,超聲下觀察頦舌肌的肌肉厚度,均能引起頦舌肌的收縮;在納入第14例受試者時,超聲下測量頦舌肌的肌肉厚度,經(jīng)分析后,zi=6.537Ui= 5.51,試驗線觸及上界U(即接受電針連續(xù)波療效優(yōu)于電針斷續(xù)波或接受電針斷續(xù)波療效優(yōu)于電針連續(xù)波),且∑di0,故接受電針斷續(xù)波療效優(yōu)于電針連續(xù)波。結(jié)論:1.選穴外金津、外玉液進針針刺頦舌肌,可以明顯引起頦舌肌收縮,擴張上氣道。2.超聲下能直觀、靈敏、動態(tài)的觀察并測量沿外金津、外玉液針刺頦舌肌,肌肉收縮的情況、肌肉的功能狀態(tài)及活動狀態(tài)。3.超聲下沿外金津、外玉液引導(dǎo)進針針刺頦舌肌,針灸及接通電針均可引起頦舌肌的收縮,改善肌肉的功能狀態(tài)和活動狀態(tài),其中,電針斷續(xù)波療效優(yōu)于電針連續(xù)波。
[Abstract]:Objective: to observe the immediate effect of genioglossus contraction by electroacupuncture (EA) with different waveforms. Fourteen healthy subjects were randomly divided into two groups: group A was treated with electroacupuncture continuous wave (frequency of 30 Hz) to stimulate external Jinjin and external jade acupoints, after three days of elution period, they were given electroacupuncture intermittent wave (1: 1) to stimulate external Jin Jin. The muscle thickness of genioglossus muscle was measured under ultrasound. Group B was stimulated by electroacupuncture intermittent wave (intermittent ratio 1: 1). After 3 days of elution, the muscle thickness of genioglossus was measured. The data of 14 subjects were analyzed by using open bilateral sequential t test by comparing and analyzing the data of the two groups, which were given electroacupuncture continuous wave (frequency 30 Hz) stimulation and the method of acupoint selection and detection was the same as that of group A. The intensity of genioglossus contraction was evaluated by measuring the thickness of genioglossus in supine position under ultrasound. The angle between the two needles in the direction of the needle is about 90 擄. For the depth of acupuncture and moxibustion into the needle, when the genioglossus is pricked along the outer Jin Jin and the external jade solution, when the acupuncture needle has a sense of hollowed-out after being punctured into the acupoint and feels the resistance again, it is the middle genioglossus muscle. It was confirmed by ultrasound that electroacupuncture continuous wave and electroacupuncture intermittent wave can cause the contraction of genioglossus muscle, both of which can cause contraction of genioglossus muscle in the 14th case, and the thickness of genioglossus muscle can be induced by electroacupuncture continuous wave and electroacupuncture intermittent wave acupuncture on outer Jin Jin, external jade liquid and ultrasonic observation of genioglossus muscle. The muscle thickness of genioglossus was measured by ultrasound.
