膝部透灸控溫技術(shù)及其效應的研究

【摘要】:目的:本文以寒濕痹阻型膝關(guān)節(jié)骨性關(guān)節(jié)炎患者為研究對象,觀察膝部透灸過程中溫度、時間、灸感、透灸時程、WOMAC、VAS、療效、安全性,以探索、總結(jié)膝部透灸控溫技術(shù)及效應。方法:60例寒濕痹阻型膝關(guān)節(jié)骨性關(guān)節(jié)炎患者,隨機分成兩組,各30例,治療組患者予以透灸法治療,對照組患者予以傳統(tǒng)灸治療,治療4個療程,觀察并記錄基礎(chǔ)資料、溫度、時間、灸感、透灸時程、WOMAC、VAS、療效、安全性等相關(guān)數(shù)據(jù),整理數(shù)據(jù),以探索、總結(jié)膝部透灸控溫技術(shù)及效應。結(jié)果:1.基礎(chǔ)資料性別、年齡、病程、體重、身高等基本信息,均p㧐0.05,差異無統(tǒng)計學意義,具有可比性。2.控溫技術(shù)及其效應(1)兩組患者知熱溫度、舒適溫度、知降溫度比較(均P0.05),存在統(tǒng)計學差異,其中透灸組膝部知熱溫度、舒適溫度均值低于傳統(tǒng)灸組;透灸組知降溫度高于傳統(tǒng)灸組;(2)兩組知熱時間、膝部舒適感持續(xù)時間比較(均P0.05),存在統(tǒng)計學差異;降溫時間比較p0.05,不存在統(tǒng)計學差異,且透灸組的知熱時間均值低于傳統(tǒng)灸組,膝部舒適感持續(xù)時間均值高于傳統(tǒng)灸組;(3)兩組灸前溫度比較p0.05,不存在統(tǒng)計學差異,說明具有可比性;灸后溫度、皮溫差比較(均P0.05),存在統(tǒng)計學差異,其中皮溫差透灸組均值低于傳統(tǒng)灸組;(4)兩組從知熱時間到感知溫度滲透到腹股溝的時間(△時間);及此時膝部刻下溫度(△溫度)比較(均P0.05),存在統(tǒng)計學差異,△時間及△溫度透灸組均值低于傳統(tǒng)灸組;(5)兩組在0-10分鐘、10-20分鐘、20-30分鐘、30-40分鐘施灸過程中局部開始出現(xiàn)灸感的頻次經(jīng)檢驗,χ2=4.04,P=0.040.05,差異存在統(tǒng)計學差異,且透灸組總頻數(shù)明顯高于傳統(tǒng)灸組;(6)兩組出現(xiàn)灸感類型及頻次(舒適感、酸脹感、沉重感、傳導感、滲透感、癢感、痛感及其其他患者難以準確描述的灸感),經(jīng)檢驗,χ2=7.27,P=0.000.05,差異有統(tǒng)計學差異,且透灸組每種類型灸感較傳統(tǒng)灸組多;(7)兩組膝部開始滲透溫度、膝部完全滲透溫度比較(均P0.05),存在統(tǒng)計學差異,且透灸組均值低于傳統(tǒng)灸組;(8)兩組透灸時程Ⅰ、透灸時程Ⅱ、透灸時程Ⅲ比較(均P0.05),存在統(tǒng)計學差異;且透灸組的透灸時程Ⅰ、透灸時程Ⅱ、透灸時程Ⅲ值低于傳統(tǒng)灸組;(9)兩組患者治療前WOMAC、VAS積分差異無統(tǒng)計學意義(均P0.05),表明兩組間具有可比性;治療前后組內(nèi)比較:各組治療后WOMAC、VAS積分與治療前相比,差異有統(tǒng)計學意義(均P0.05),且治療后WOMAC、VAS積分優(yōu)于治療前;治療后組間比較:兩組患者治療后WOMAC、VAS積分比較,差異有統(tǒng)計學意義(均P0.05),且透灸組改善優(yōu)于傳統(tǒng)灸組。3.療效及安全性(1)透灸組臨床治愈8例,顯效17例,有效4例,無效1例,有效率為96.67%,傳統(tǒng)灸組臨床治愈4例,顯效13例,有效11例,無效2例,有效率為93.33%,差異有統(tǒng)計學意義(P0.05);(2)兩組在暈針、滯針、彎針、水泡、斷針、血腫等不良反應上不存在差異。結(jié)論:(1)透灸法較傳統(tǒng)灸法對溫度的變化更敏感;(2)透灸法較傳統(tǒng)灸法更容易達到灸感,且灸感持續(xù)的時間更長;(3)透灸法較傳統(tǒng)灸法更容易得到不同類型灸感;(4)透灸法更能改善WOMAC、VAS積分,緩解臨床疼痛癥狀,提高日常生活功能;(5)透灸法療效優(yōu)于傳統(tǒng)灸法;(6)透灸法與傳統(tǒng)灸法不良反應不存在差異。
