【摘要】：目的:探討韓氏驗(yàn)方"補(bǔ)陽(yáng)益氣止崩湯"加減治療脾腎陽(yáng)虛型崩漏的作用機(jī)理,進(jìn)行臨床分析求證,明確補(bǔ)陽(yáng)益氣止崩湯對(duì)脾腎陽(yáng)虛型崩漏的止血效果,證實(shí)其有效性,推廣于臨床,解除崩漏患者之病痛,發(fā)揚(yáng)韓氏驗(yàn)方,使其有效和廣泛的應(yīng)用于臨床。方法:通過(guò)納入和排除標(biāo)準(zhǔn),收集2015年6月--2016年12月黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院婦科門診的脾腎陽(yáng)虛型崩漏患者33例,脫落3例,應(yīng)用補(bǔ)陽(yáng)益氣止崩湯加減治療3個(gè)月,通過(guò)自身前后對(duì)照,觀察患者的止血情況、臨床癥狀治療前治療后的改善情況、性腺激素(E2、P)指標(biāo)治療前后的改變,運(yùn)用統(tǒng)計(jì)學(xué)軟件SPSS17.0對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果:臨床療效與患者年齡的關(guān)系:P0.05,提示患者的年齡與臨床療效之間沒(méi)有顯著差異,無(wú)統(tǒng)計(jì)學(xué)意義,說(shuō)明補(bǔ)陽(yáng)益氣止崩湯對(duì)不同年齡段脾腎陽(yáng)虛型崩漏均有良好的療效。主要癥狀、次要癥狀前后癥狀積分評(píng)分結(jié)果P0.05,提示有統(tǒng)計(jì)學(xué)意義,證明補(bǔ)陽(yáng)益氣止崩湯對(duì)對(duì)臨床癥狀的改善有顯著作用。治療后E2值和P值均有明顯的升高,說(shuō)明補(bǔ)陽(yáng)益氣止崩湯對(duì)脾腎陽(yáng)虛型崩漏患者卵泡發(fā)育及排卵功能有明顯的改善和恢復(fù)作用。根據(jù)治療前后臨床總療效評(píng)價(jià),30例患者中治愈8例,顯效13例,有效5例,臨床總有效率是86.7%。結(jié)論:臨床研究結(jié)果證實(shí),補(bǔ)陽(yáng)益氣止崩湯治療脾腎陽(yáng)虛型崩漏臨床效果顯著,止血效果顯著,止血所用時(shí)間短,且臨床癥狀改善明顯,能夠通過(guò)改善患者的血清雌二醇和孕酮值發(fā)揮其作用機(jī)理,且無(wú)毒副作用。
[Abstract]:Objective: to explore the effect mechanism of Han's prescription "Buyang Yiqi Zhibong decoction" in treating the type of spleen and kidney yang deficiency, to make clinical analysis and proof, to make clear the hemostatic effect of Buyang Yiqi Zhibong decoction on spleen and kidney yang deficiency type collapse, and to confirm its effectiveness. Popularize in the clinic, relieve the pain of the patients with insipidus, carry forward the Han's prescription to make it effective and widely used in the clinic. Methods: by including and excluding the standard, From June 2015 to December 2016, 33 patients with spleen and kidney yang deficiency type were collected from gynecological outpatient department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The hemostasis, the improvement of clinical symptoms before and after treatment, the changes of gonadal hormone E2P before and after treatment were observed. The statistical software SPSS17.0 was used to analyze the data. Results: the relationship between clinical effect and patient's age was: P0.05, indicating that there was no significant difference between age and clinical effect, and there was no statistical significance. The results showed that Buyang Yiqi Zhibong decoction had a good curative effect on the type of spleen and kidney yang deficiency in different age groups (P 0.05), which indicated that the score of symptom score before and after the main symptoms and secondary symptoms (P0.05) indicated that there was statistical significance. It was proved that Buyang Yiqi Zhibong decoction had a significant effect on the improvement of clinical symptoms, the E _ 2 value and P value were significantly increased after treatment. The results showed that Buyang Yiqi Zhibong decoction could obviously improve and restore follicle development and ovulation function in patients with spleen and kidney yang deficiency. According to the total clinical curative effect before and after treatment, 8 cases were cured, 13 cases were effective, 5 cases were effective. Conclusion: the clinical study shows that the clinical effect of Buyang Yiqi Zhibong decoction is remarkable, the hemostatic effect is remarkable, the time of hemostasis is short, and the clinical symptoms are improved obviously. It can improve the serum estradiol and progesterone levels in patients with no side effects.
