【摘要】：目的:觀察養(yǎng)陰益氣湯在治療氣陰兩虛型子宮內(nèi)膜不規(guī)則脫落上的臨床療效,并初步探討其作用機(jī)理,并期待其在臨床治療本病有所借鑒和指導(dǎo)意義。方法:將所搜集60例診斷為氣陰兩虛型排卵性功血子宮內(nèi)膜不規(guī)則脫落的患者隨機(jī)分為治療組(30例)和對(duì)照組(30例),治療組給予中藥治療,對(duì)照組給予中成藥治療。所有患者均給予3個(gè)月經(jīng)周期的治療,主要觀察對(duì)比兩組患者的臨床療效:對(duì)比兩組患者治療前后臨床癥狀及體征、基礎(chǔ)體溫、孕激素水平及子宮內(nèi)膜厚度的改變情況。結(jié)果:臨床綜合療效顯示,治療組中痊愈43.33%,顯效30.00%,有效13.33%;對(duì)照組中分別為痊愈6.67%,顯效33.33%,有效13.33%,治療組的總有效率86.67%遠(yuǎn)遠(yuǎn)高于對(duì)照組的53.33%,經(jīng)檢驗(yàn)兩組的總有效率存在差異,有統(tǒng)計(jì)學(xué)意義(P0.05);改善中醫(yī)證候表現(xiàn)上,治療組中痊愈43.33%,顯效30.00%,有效16.67%;對(duì)照組中分別為痊愈6.67%,顯效33.33%,有效13.33%,治療組的總有效率90.00%遠(yuǎn)遠(yuǎn)高于對(duì)照組的53.33%,察兩組總有效率存在差異,且有統(tǒng)計(jì)學(xué)意義(P0.05);中醫(yī)證候積分方面,經(jīng)治療后,兩組證候積分均呈下降表現(xiàn)(P0.05),但治療組低于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。輔助檢查方面,治療組在縮短高溫相至低溫相的移行時(shí)間,調(diào)整血清孕酮水平及子宮內(nèi)膜厚度方面療效顯著,與對(duì)照組相比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:養(yǎng)陰益氣湯治療氣陰兩虛型排卵性功血子宮內(nèi)膜不規(guī)則脫落具有較好的臨床療效,可明顯改善患者月經(jīng)經(jīng)期、經(jīng)量、經(jīng)色、經(jīng)質(zhì)及伴隨癥狀、基礎(chǔ)體溫、孕激素水平及子宮內(nèi)膜厚度。體現(xiàn)了補(bǔ)腎健脾,養(yǎng)陰益氣,固沖寧血的治療方法在治療本病上有重要臨床意義。
[Abstract]:Objective: to observe the clinical effect of Yangyin Yiqi decoction on irregular exfoliation of endometrium in the type of deficiency of Qi and Yin, and to explore its mechanism. Methods: 60 patients diagnosed as deficiency of qi and yin were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group was treated with traditional Chinese medicine. The control group was treated with Chinese patent medicine. All patients were treated with three menstrual cycles. The clinical effects of the two groups were observed and compared: the clinical symptoms and signs before and after treatment, the basic body temperature, the clinical symptoms and signs of the patients in the two groups were compared before and after treatment. Changes in progesterone levels and endometrial thickness. In the treatment group, the total effective rate of the treatment group was 86.67% higher than that of the control group, and the total effective rate was significantly higher than that of the control group, and the total effective rate of the two groups was significantly higher than that of the control group, and the total effective rate of the two groups was significantly higher than that of the control group, and the total effective rate of the treatment group was significantly higher than that of the control group, and there were differences in the total effective rate between the two groups. In the treatment group, there were significant effects in shortening the transition time from hyperthermia to hypothermia, adjusting serum progesterone level and endometrial thickness. Conclusion: Yangyin Yiqi decoction has better clinical curative effect on irregular abscission of endometrium of ovulatory type of asthenia of Qi and Yin, and can obviously improve menstrual period, quantity and color of menstruation. Meridian and its concomitant symptoms, basic body temperature, progesterone level and endometrial thickness. The treatment of tonifying kidney and spleen, nourishing yin and invigorating qi, and nourishing Yin and tonifying blood have important clinical significance in treating this disease.
