【摘要】：目的:1、成功建立盆腔炎性后遺癥大鼠模型,觀察大鼠不同時(shí)期的病理變化及血液指標(biāo)的改變,確定盆舒飲及針刺治療的時(shí)間。2、通過對(duì)比各組對(duì)SPID大鼠治療觀察病灶的病理、血液指標(biāo)的變化,為臨床治療提供依據(jù)。3、從"正虛邪戀"的角度論述SPID的基本病機(jī),"瘀血阻滯"為其病理結(jié)局。并結(jié)合西醫(yī)(IL-4、IL-10指標(biāo)的變化)探討針?biāo)幗Y(jié)合的治療效果。方法:1、采用機(jī)械損傷+細(xì)菌感染造模法建立SPID大鼠模型,于造模后第15天查看病灶情況及取子宮組織做病理形態(tài)學(xué)的觀察。2、分別觀察6組大鼠(空白組、模型組、丹黃祛瘀膠囊組、盆舒飲組、針刺組、盆舒飲+針刺組)的病理情況及血液中IL-4、IL-10表達(dá)情況。結(jié)果:1、成功建立SPID大鼠模型,病理顯示造模15天,子宮病理顯示:內(nèi)膜上皮細(xì)胞部分脫落或增生,間質(zhì)細(xì)胞被纖維組織取代,腺體變形,腺體管腔壁空泡樣改變。2、模型組大鼠血液中IL-4及IL-10表達(dá)較正常組均有所升高,差異有極顯著統(tǒng)計(jì)學(xué)意義(P0.01)。各治療組大鼠血IL-4及IL-10濃度表達(dá)較模型組均有所降低,但未達(dá)到正常水平。針?biāo)幗Y(jié)合組血IL-4及IL-10濃度接近空白組,差異無統(tǒng)計(jì)學(xué)意義(P0.05),均優(yōu)于其它治療組。IL-4表達(dá)方面,針?biāo)幗Y(jié)合組優(yōu)于各治療組,有極顯著統(tǒng)計(jì)學(xué)意義(P0.01);與丹黃組比較,盆舒飲組和針刺組治療效果較好,盆舒飲組極顯著統(tǒng)計(jì)學(xué)意義(P0.01),針刺組具有顯著統(tǒng)計(jì)學(xué)意義(P0.05)。IL-10表達(dá)方面,針?biāo)幗Y(jié)合組優(yōu)于各治療組,與針?biāo)幗Y(jié)合組比較,盆舒飲組及丹黃組有極顯著的統(tǒng)計(jì)學(xué)意義(P0.01),針刺組具有顯著的統(tǒng)計(jì)學(xué)差異(P0.05);針刺組優(yōu)于丹黃組具有極顯著的統(tǒng)計(jì)學(xué)意義(P0.01),丹黃組優(yōu)于盆舒飲組差異具有顯著的統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:通過機(jī)械損傷加混合細(xì)菌造模法可成功建立SPID大鼠模型。在SPID的治療上,針?biāo)幗Y(jié)合療效優(yōu)于其它單一方法治療組,可減少炎細(xì)胞浸潤(rùn),并減輕組織粘連,促進(jìn)組織修復(fù);能夠下調(diào)模型大鼠SPID血液中的IL-4及IL-10濃度,緩解盆腔炎癥,改善盆腔粘連。
[Abstract]:Objective: to establish the rat model of pelvic inflammatory sequelae successfully, observe the pathological changes and the changes of blood indexes in different periods of rats, determine the time of treatment of Pan-Shuyin and acupuncture, and observe the pathological changes of the lesions by comparing the treatment of each group with that of SPID rats. Changes in blood indicators, In order to provide the basis for clinical treatment, this paper discusses the basic pathogenesis of SPID from the angle of "positive deficiency and evil love", and "blood stasis block" as its pathological outcome. In combination with the changes of IL-4 / IL-10 index in western medicine, the therapeutic effect of acupuncture and medicine combination is discussed. The SPID rat model was established by the method of mechanical injury bacterial infection. On the 15th day after the establishment of the model, the lesions were examined and the uterine tissues were taken for pathomorphological observation. The rats in 6 groups (blank group, model group, Danhuang Quyu capsule group, Pan-Shuyin group and acupuncture group) were observed respectively. Results: SPID rat model was established successfully. The pathological results showed that the endometrial epithelial cells were partly shed or proliferated. The interstitial cells were replaced by fibrous tissue, the gland deformed, the wall vacuolation of glandular lumen changed. The expression of IL-4 and IL-10 in the blood of the model group was higher than that of the normal group. The expression of serum IL-4 and IL-10 in each treatment group was lower than that in the model group, but did not reach the normal level. The serum IL-4 and IL-10 concentrations in the acupuncture and medicine combination group were close to the blank group. The difference was not statistically significant (P 0.05), which was better than that of other treatment groups in the expression of IL-4, the combination of acupuncture and medicine was superior to the other treatment groups (P 0.01), and compared with the Danhuang group, the treatment effect of the two groups was better than that of the control group, and that of the acupuncture group was better than that of the other treatment groups. The expression of P0.05 + IL-10 in the acupuncture group was significantly higher than that in the treatment group, and was significantly higher in the acupuncture group than in the acupuncture and medicine combination group, and was significantly higher in the acupuncture group than in the acupuncture and medicine combination group, and the expression of IL-10 in the acupuncture group was significantly higher than that in the acupuncture and medicine combination group. There were very significant statistical significance in the group of Pan-Shuyin and Danhuang, there was significant difference between the acupuncture group and the acupuncture group (P 0.05), the acupuncture group had a very significant statistical significance than the group of Danhuang, and the group of Danhuang was superior to the group of Peshuyin, and the difference of the group of acupuncture was more significant than that of the group of Pan-Shuyin. Conclusion: SPID rat model can be successfully established by mechanical injury and mixed bacterial modeling. The therapeutic effect of acupuncture combined with medicine is better than other single treatment group, can reduce inflammatory cell infiltration, reduce tissue adhesion, promote tissue repair, can down-regulate the concentration of IL-4 and IL-10 in SPID blood of model rats, alleviate pelvic inflammation and improve pelvic adhesion.
