【摘要】：目的:通過隨機對照臨床研究,比較腹針與單純針刺治療神經(jīng)性耳鳴在治療前后耳鳴嚴重程度、生活質量、耳鳴殘疾量表積分方面的變化,評價腹針治療神經(jīng)性耳鳴的臨床療效,分析腹針在治療神經(jīng)性耳鳴方面的安全性、可行性、可重復性和應用價值。方法:本研究納入66例符合神經(jīng)性耳鳴中西醫(yī)診斷標準的患者,以簡單隨機分組法分為治療組與對照組,每組33例。每組患者均每周治療3次,隔日治療一次,4周為一個療程。采用THI量表、耳鳴嚴重程度量表、SF-36生活質量量表分別于治療前、治療第4周結束、治療第8周結束進行評分觀察療效,建立數(shù)據(jù)庫進行統(tǒng)計分析,得出結論。結果:1.兩組治療后,治療組總有效率93.33%,對照組總有效率83.33%,可以認為治療組治療神經(jīng)性耳鳴的療效優(yōu)于對照組(P0.05)。2.兩組治療后THI量表評分均較治療前降低(P0.01),表明兩組均能緩解耳鳴,兩組治療后比較,說明治療組較對照組在改善耳鳴殘疾方面有較好的優(yōu)勢(P0.05)。3.兩組治療后耳鳴嚴重程度量表評分均降低(P0.01),表明兩組均能降低耳鳴程度,兩組治療后比較,治療組較對照組在降低耳鳴程度方面有較好的優(yōu)勢(P0.05)。4.兩組治療后SF-36量表評分均有所上升(P0.01),證明兩組均能改善生活質量,兩組治療后比較,可以認為治療組改善耳鳴患者生活質量效果較對照組明顯(P0.05)。5.兩組THI耳鳴殘疾量表功能性F評分、情感性E評分、嚴重性C評分均數(shù)治療前后對比,可以認為兩組均能改善耳鳴患者功能性、情緒性和自我嚴重性認知障礙(P0.01)。兩組治療后功能性F評分對比,治療組在改善神經(jīng)性耳鳴功能性障礙方面優(yōu)于對照組(P0.05)。兩組治療后情感性E評分對比,治療組能更好地控制患者的情緒障礙(P0.05)。兩組治療后嚴重性C評分對比,兩組在改善患者對耳鳴的自我認知方面無顯著差異,療效相當(P0.05)。結論:腹針和單純針刺治療神經(jīng)性耳鳴均有臨床療效,腹針療法在改善患者耳鳴殘疾程度,降低嚴重程度,提高生活質量方面均比單純針刺占優(yōu)勢,但兩組在THI量表嚴重性C因子方面改善相當,無明顯差異。通過隨訪,發(fā)現(xiàn)腹針的療效要比單純針刺更優(yōu)。本研究說明了腹針作為治療神經(jīng)性耳鳴的方式是可推廣的、安全的、有效的。
[Abstract]:Objective: to compare the severity of tinnitus, quality of life and integral score of tinnitus disability scale between abdominal acupuncture and simple acupuncture in the treatment of tinnitus before and after treatment. To evaluate the clinical efficacy of abdominal acupuncture in the treatment of neurotic tinnitus, and to analyze the safety and feasibility of abdominal acupuncture in the treatment of neurotic tinnitus. Methods: 66 patients who met the diagnostic criteria of traditional Chinese and western medicine for tinnitus were randomly divided into treatment group (n = 33) and control group (n = 33). THI scale, tinnitus severity scale and SF-36 quality of life scale were used before treatment, at the end of the 4th week, and at the end of the 8th week. Establish a database for statistical analysis, draw a conclusion. Results: 1. After two groups of treatment, The total effective rate of the treatment group was 93.33 and the total effective rate of the control group was 83.33. It can be considered that the curative effect of the treatment group on neurotic tinnitus was better than that of the control group (P 0.05) .2.After treatment, the score of THI scale in both groups was lower than that before treatment, indicating that both groups could relieve tinnitus, and compare the two groups after treatment. The results showed that the treatment group had a better advantage than the control group in improving tinnitus disability. The scores of the tinnitus severity scale decreased in both groups after treatment, indicating that both groups could reduce the tinnitus degree, and the comparison between the two groups after treatment, Compared with the control group, the treatment group had a better advantage in reducing tinnitus. 4. The scores of SF-36 scale increased after treatment in both groups, which proved that both groups could improve the quality of life, compared with the two groups after treatment. It can be concluded that the effect of improving the quality of life of tinnitus patients in the treatment group was significantly higher than that in the control group (P0.05.5.The functional F score, affective E score and severity C score of the THI tinnitus disability scale before and after treatment were compared between the two groups before and after treatment. It can be concluded that both groups can improve the functional, emotional and self-severity cognitive impairment of tinnitus patients (P 0.01). The scores of affective E in the treatment group were better than those in the control group in improving the functional disorder of neurotic tinnitus, and the scores of affective E in the treatment group were better controlled than those in the control group. There was no significant difference between the two groups in improving the self-cognition of tinnitus, and the curative effect was similar to that of P0.05.Conclusion: both abdominal acupuncture and simple acupuncture are effective in treating neurotic tinnitus. Abdominal acupuncture can improve the degree of tinnitus disability and reduce the severity of tinnitus. The improvement of quality of life was superior to that of acupuncture alone, but there was no significant difference between the two groups in terms of severity factor C of THI scale. It was found that abdominal acupuncture is more effective than simple acupuncture. This study shows that abdominal acupuncture is a popularized, safe and effective method for the treatment of neurotic tinnitus.
