新安醫(yī)學固本培元派醫(yī)家痹病用藥特色研究

【摘要】:目的:借助分析名中醫(yī)的軟件-中醫(yī)傳承輔助系統(tǒng)(TCMISS),重點分析新安醫(yī)學固本培元派醫(yī)家治痹的整體用藥規(guī)律;來自新安地區(qū)的“風濕病五老”之一的李濟仁教授,崇尚汪機的固本培元思想,繼承和發(fā)揚了新安醫(yī)學治痹思想,對符合入選標準的臨床病例亦通過TCMISS進行分析;最后比較兩者的異同點。方法:以皖南醫(yī)學院圖書館、皖南醫(yī)學院第一附屬醫(yī)院(弋磯山醫(yī)院)國醫(yī)大師工作室為平臺,收集并整理固本培元派醫(yī)家的臨床醫(yī)案或臨床經(jīng)驗,嚴格按照納入標準選錄,共有6位醫(yī)家(汪機、孫一奎、程敬通、吳楚、程國彭、程杏軒),其著作依次為《石山醫(yī)案》、《孫一奎醫(yī)學全書》、《程敬通醫(yī)案》、《吳氏醫(yī)驗錄全集》、《醫(yī)學心悟》和《杏軒醫(yī)案》,有臨床驗案130例,處方106首,運用中醫(yī)傳承輔助平臺,采用規(guī)格分析、改進的互信息法、復雜系統(tǒng)熵聚類等數(shù)據(jù)挖掘方法,對106首處方的藥物進行頻次分析、規(guī)則分析、聚類分析,而產(chǎn)生常用藥物、核心組合、及新處方,進而總結新安醫(yī)學固本培元派醫(yī)家治痹的用藥規(guī)律;收集近5年已發(fā)表在中國知網(wǎng)的論文中提及的臨床驗案和已出版的書籍的臨床驗案共34例,與上述固本培元派醫(yī)家治痹用藥進行對比。結果:1.六位醫(yī)家的醫(yī)案中有關治痹藥物共有141味,總用藥頻次為1035次,其中出現(xiàn)頻次最高的藥物是甘草、當歸、人參、白術、茯苓、陳皮、秦艽、肉桂、黃芪、附子、川芎;前10位核心組合分別為人參-白術、人參-當歸、人參-茯苓、白術-茯苓、炙甘草-茯苓、炙甘草-白術、炙甘草-人參、白術-當歸、人參-黃芪、陳皮-茯苓;進一步產(chǎn)生新方9首。2.李濟仁教授臨床工作病例處理亦采用中醫(yī)傳承輔助系統(tǒng),錄入和整理所選的34首方劑,從多方面和多角度分析,得到相應的高頻用藥、核心組合和新方。結論:較為系統(tǒng)地提取了新安醫(yī)學固本培元派醫(yī)家治痹醫(yī)案;通過分析得出了新安醫(yī)學固本培元派醫(yī)家治痹的核心方藥、常用藥物組合及新方;通過異同點的比較,李濟仁教授繼承和發(fā)揚了新安醫(yī)學固本培元派的學術思想,將古代文獻研究結合李教授的現(xiàn)今臨床實踐,使本課題更富有現(xiàn)代研究意義。
[Abstract]:Objective: by analyzing the software of famous Chinese medicine, the assistant system of traditional Chinese medicine (TCMISS), this paper focused on the analysis of the overall drug use rule of the doctors who sent doctors to treat arthralgia in Xin'an Medicine, and Professor Li Jiren, one of the "five old people with rheumatism" from Xin'an area. Advocating Wang Ji's thought of cultivating Yuan, inheriting and carrying forward the thought of Xin'an medical treatment of arthralgia, and analyzing the clinical cases that meet the criteria of selection through TCMISS. Finally, the similarities and differences between the two are compared. Methods: the library of Southern Anhui Medical College, The Guoyi Master Studio of the first affiliated Hospital of Southern Anhui Medical College (Yaji Shan Hospital) was a platform for collecting and arranging the clinical cases or clinical experience of the medical practitioners sent by the Puiyuan School of Guimen, strictly according to the inclusion criteria. A total of 6 doctors (Wang Ji, Sun Yikui) were selected and recorded, and a total of 6 doctors (Wang Ji and Sun Yikui) were selected and recorded. Cheng Jingtong, Wu Chu, Cheng Guopeng, Cheng Xingxuan, whose works are "Shishan Medical record", "Sun Yi-kui Medical Book", "Cheng Jingtong Medical case", "Wu's Medical examination record complete works", "Medical Heart comprehension" and "Xing Xuan Medical record", has 130 clinical cases and 106 prescriptions. With the aid of traditional Chinese medicine inheritance platform, data mining methods such as specification analysis, improved mutual information method and entropy clustering of complex systems, 106 prescriptions of drugs were analyzed by frequency, rules, clustering, and common drugs were produced. The core combination, and new prescriptions, to sum up the rule of drug use in treating arthralgia by doctors sent by Peiyuan School of Medicine, and to collect 34 cases of clinical cases and published books, which have been published in papers published in China in the past five years. Results: among the six doctors' cases, there were 141 kinds of arthralgia drugs, and the total frequency was 1035. The most frequent drugs were liquorice, angelica sinensis, ginseng, Atractylodes macrocephala, Poria cocos. Chen Pei, Gentiana macrophylla, Cinnamomum Cinnamomi, Astragalus membranaceus, Radix Aconiti, Rhizoma Chuanxiong; the top 10 core combinations are Ginseng-Atractylodes, Ginseng-Angelica sinensis, Ginseng-Poria cocos, Atractylodes macrocephala-Poria cocos, Radix Glycyrrhizae-Fuling, Radix Glycyrrhizae Atractylo@@. Atractylodes macrocephala-Angelica sinensis, Ginseng-Astragalus, Chen Pei-Poria; further produce 9 new prescriptions. 2. Professor Li Jiren's clinical case handling also uses the Chinese Medicine inheritance Assistance system to input and organize 34 selected prescriptions, and analyzes them from many aspects and angles. The corresponding high frequency medicine, core combination and new prescription were obtained. Conclusion: the medical records of treating arthralgia by doctors of Peiyuan School of Medicine in Xin'an Medicine are extracted systematically, and the core prescriptions of treating arthralgia by doctors of Peiyuan School of Medicine in Xin'an Medicine are obtained through analysis. By comparing the similarities and differences, Professor Li Jiren inherited and carried forward the academic thought of the School of Medicine in Xin'an, and combined the study of ancient literature with Professor Li's current clinical practice. Make this subject more modern research significance.
