【摘要】：目的通過理脾化痰祛瘀方對初診斷2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者的治療,觀察其肝臟病變的臨床療效,探討治療機制。方法選擇初診斷T2DM合并NAFLD患者156例,分為A(生活方式干預(yù)+口服降糖藥治療)、B(生活方式干預(yù)+口服降糖藥治療+血脂康)、C組(生活方式干預(yù)+口服降糖藥治療+理脾化痰祛瘀方),分析各組治療前后的體質(zhì)量指數(shù)(BMI)、血脂、糖化血紅蛋白(Hb A1c)、肝功能、脂肪肝指數(shù)(FLI)。結(jié)果 B組和C組,腰圍、BMI、血清甘油三酯(TG)水平及Hb A1c在治療24周后,均較治療前有明顯改善,差異有統(tǒng)計學(xué)意義(P0.05);肝功能指標(biāo)丙氨酸氨基轉(zhuǎn)移酶(ALT)、天冬氨酸轉(zhuǎn)氨酶(AST)、堿性磷酸酶(ALP)、谷氨酰轉(zhuǎn)移酶(GGT)及FLI亦如此(P0.01)。A組僅BMI改變具有顯著性差異(P0.05);B、C組治療后TG、Hb A1c、ALT、AST、GGT、ALP及FLI水平下降,與A組相比差異有統(tǒng)計學(xué)意義(P0.05);C組治療后上述指標(biāo)變化與B組相比,差異有統(tǒng)計學(xué)意義(P0.01)。3組治療前后Hb A1c(ΔHb A1c)、BMI(ΔBMI)、腰圍(Δ腰圍)的變化情況與FLI(ΔFLI)的變化情況并無顯著性相關(guān)(P0.05),與肝臟生化指標(biāo)變化、血脂變化ΔTG也無明顯相關(guān)性(P0.05)。結(jié)論 T2DM患者使用理脾化痰祛瘀方時,對NAFLD產(chǎn)生積極的治療效應(yīng),明顯改善了肝臟內(nèi)脂肪的蓄積。
[Abstract]:Objective to observe the therapeutic effect of Lipi Huatan Quyu recipe (LPRF) on patients with type 2 diabetes mellitus (T2DM) combined with nonalcoholic fatty liver disease (NAF LDD), and to explore the therapeutic mechanism. Methods 156 cases of T2DM complicated with NAFLD were selected. Divided into A (lifestyle intervention oral hypoglycemic drugs treatment of Hedong Bu (lifestyle intervention oral hypoglycemic drugs treatment Xuezhikangjiao C group (lifestyle intervention oral antiglycemic drugs treatment of lipi phlegm and blood stasis decoction) analysis of each group before and after treatment. Body mass index (BMI) of BMIA, lipids, Results the levels of HbA1cU, liver function, fatty liver index (FLI), waist circumference (BMIs), serum triglyceride (TGG) levels and HbA1c levels in group B and C were significantly improved after 24 weeks of treatment. There were significant differences in the levels of alt, AST, ALP and FLI between the two groups (P < 0.01) and FLI (P < 0.05), and the levels of alt, AST, ALP, glutamyl transferase (GGTT) and FLI in group A were also significantly lower than those in group A (P 0.05A, P 0.05A, P < 0.05), and the levels of FLI and ALP were significantly decreased in group A (P 0.05A ~ (-1) A ~ (-1) A / A) after treatment, and in group A (P < 0.05), the changes of BMI in group A were significantly higher than those in group A (P < 0.05). The levels of alt and FLI in group A were significantly lower than those in group A (P < 0.05). There was significant difference between group A and group A. The changes of the above indexes in group C were significantly different from those in group B. There was no significant correlation between the changes of HB A1c (螖 HbA1c), waist circumference (螖 waistline) and the changes of FLI (螖 FLI) before and after treatment in P0.01C.3 group, but there was no significant correlation between the changes of HbA1c (螖 HbA1cTc) and the changes of liver biochemical indexes, and there was no significant correlation between the changes of FLI (螖 FLI) and the changes of BMIs (螖 BMIs). There was no significant correlation between 螖 TG and blood lipids. Conclusion the effect of Lipi Huatan Quyu decoction on NAFLD was positive, and the accumulation of fat in liver was improved obviously in patients with T2DM.
