【摘要】：研究目的：本課題旨在通過(guò)對(duì)胃癌證素及其與胃癌患者常見(jiàn)癥狀、體征、舌苔脈象之間相關(guān)性的研究,對(duì)胃癌證候、證素的分布規(guī)律進(jìn)行初步的探討,為胃癌證型的規(guī)范化和標(biāo)準(zhǔn)化提供客觀的參考依據(jù),以期指導(dǎo)胃癌的中醫(yī)臨床治療。研究方法：本課題采用回顧性研究的方法,189例病例均來(lái)自于到2012年3月-2015年3月期間,就診于江蘇省中醫(yī)院、經(jīng)過(guò)胃鏡及病理檢查確診為胃癌的患者,符合《胃癌臨床實(shí)踐指南2013 V2(中文版)》診斷及分期標(biāo)準(zhǔn),并且通過(guò)四診資料的規(guī)范化采集,參照《中醫(yī)證候診斷治療學(xué)》及《中醫(yī)虛證辨證參考標(biāo)準(zhǔn)》等文獻(xiàn)中病性辨證及臟腑辨證的內(nèi)容,結(jié)合導(dǎo)師經(jīng)驗(yàn)與指導(dǎo)進(jìn)行辨證分析。建立數(shù)據(jù)庫(kù),統(tǒng)計(jì)分析胃癌主要證素及癥狀體征、舌苔脈象的整體的分布情況；分析總結(jié)胃癌主要病位、病性證素與癥狀體征、舌苔脈象的之間的相關(guān)性。研究結(jié)果：根據(jù)本課題研究結(jié)果顯示,胃癌患者常見(jiàn)的癥狀體征是乏力、納差、噯氣、胃脘部隱痛、泛酸、消瘦、便溏、畏寒、胃脘部嘈雜感、腹脹；主要的舌苔脈象為：脈細(xì)、苔白、舌淡、苔薄、舌紅、苔少、苔膩、舌有裂紋。Ⅰ期患者19例,主要癥狀體征為畏寒、乏力、納差、噯氣、泛酸、胃脘部隱痛,Ⅱ期35例,主要癥狀體征為乏力、噯氣、胃脘部隱痛、泛酸、納差,Ⅲ期63例,主要癥狀體征為乏力、納差、噯氣、泛酸、便溏,Ⅳ期72例,主要癥狀體征為乏力、納差、消瘦、噯氣、胃脘部隱痛。主要的病位證素為：脾、胃、大腸、肝、腎、肺；主要的病性證素為氣虛、陰虛、血虛、濕、陽(yáng)虛、血瘀。病性證素之間的組合方式以虛性證素和實(shí)性證素相兼組合最為常見(jiàn)。單一的病性證素出現(xiàn)頻率較少,通常為2個(gè)或3個(gè)病性證素相組合。便溏、胃脘部隱痛、噯氣、舌淡、苔少、脈細(xì)是和證素“脾”相關(guān)的癥狀體征、舌苔脈象；噯氣、泛酸、消瘦、脈澀是和證素“胃”相關(guān)的癥狀體征、舌苔脈象；便溏是和證素“大腸”相關(guān)的癥狀體征；舌紅、脈弦、脈弱是和證素“肝”相關(guān)的舌苔脈象；噯氣是與證素“腎”相關(guān)的癥狀體征；泛酸、乏力是和證素“肺”有關(guān)的癥狀體征；乏力、納差、苔薄,脈數(shù)是與證素“氣虛”有關(guān)的癥狀體征、舌苔脈象；泛酸、舌紅、脈細(xì)是與證素“陰虛”相關(guān)的癥狀體征、舌苔脈象；脈弱是與證素“血虛”相關(guān)的舌苔脈象；苔膩、脈濡是與濕相關(guān)的舌苔脈象；畏寒、脈沉是與證素“陽(yáng)虛”相關(guān)的癥狀體征、舌苔脈象；舌暗、脈澀是與證素“血瘀”相關(guān)的舌苔脈象；腹脹,苔膩是與證素“氣滯”相關(guān)的癥狀體征、舌苔脈象；舌紅、脈數(shù)是與證素“熱”相關(guān)的舌苔脈象。
[Abstract]:Objective: to study the relationship between the syndromes of gastric cancer and the symptoms, signs and tongue coating of gastric cancer patients, and to explore the distribution of syndromes and syndromes of gastric cancer.
