【摘要】：目的：觀察全國(guó)名老中醫(yī)趙化南辨證用藥熱敷對(duì)血液透析患者動(dòng)靜脈內(nèi)瘺成熟的臨床療效,并初探中藥熱敷促進(jìn)動(dòng)靜脈內(nèi)瘺成熟、改善動(dòng)靜脈內(nèi)瘺血流量的可能機(jī)理。方法：采用前瞻性隊(duì)列研究方法,病例選擇為連云港市中醫(yī)院腎病科擬行動(dòng)靜脈內(nèi)瘺手術(shù)的血液透析患者60例,隨機(jī)分為觀察組30例及對(duì)照組30例。所有患者均選用腕關(guān)節(jié)遠(yuǎn)側(cè)端橈動(dòng)脈與頭靜脈行自體動(dòng)靜脈內(nèi)瘺成形術(shù),吻合方式采用端-側(cè)吻合方式。對(duì)照組在擬行動(dòng)靜脈內(nèi)瘺手術(shù)前五天開始用熱毛巾熱敷,術(shù)后第2天開始予喜療妥涂擦內(nèi)瘺處；觀察組在擬行動(dòng)靜脈內(nèi)瘺手術(shù)前五天采用趙化南益氣活血通絡(luò)方熱敷,術(shù)后第2天開始采用清熱解毒活血通絡(luò)中藥局部熱敷。每次30分鐘,每天兩次,療程均為2周。密切監(jiān)測(cè)內(nèi)瘺血管情況,及時(shí)更換敷料,術(shù)后注意監(jiān)測(cè)血壓、術(shù)肢防壓。觀察指標(biāo)為：①手術(shù)后1個(gè)月內(nèi)動(dòng)靜脈內(nèi)瘺通暢情況；②內(nèi)瘺初次使用時(shí)間。③術(shù)后1月多譜勒超聲測(cè)定內(nèi)瘺血液量。并記錄治療過程中產(chǎn)生的不良反應(yīng)。應(yīng)用SPSSl6.0統(tǒng)計(jì)軟件進(jìn)行數(shù)據(jù)分析,計(jì)量資料數(shù)據(jù)以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,2組比較采用t檢驗(yàn),計(jì)數(shù)資料比較采用X2檢驗(yàn),P0.05為有顯著性差異。結(jié)果：1.兩組內(nèi)瘺通暢情況：觀察組1月內(nèi)內(nèi)瘺通暢29例,1例出現(xiàn)栓塞；對(duì)照組6例出現(xiàn)栓塞。觀察組內(nèi)瘺通暢情況優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2.內(nèi)瘺初次使用時(shí)間：初次使用內(nèi)瘺時(shí)間觀察組為30±6天,對(duì)照組為35±5天,差異有統(tǒng)計(jì)學(xué)意義(P0.01)；提示中藥局部熱敷對(duì)促進(jìn)內(nèi)瘺發(fā)育成熟有較好的作用。3.術(shù)后1月多譜勒彩超測(cè)定：觀察組內(nèi)瘺血流量(280±54)ml/min,對(duì)照組(240± 62)ml/min,差異有統(tǒng)計(jì)學(xué)意義(P0.01)；提示中藥局部熱敷對(duì)改善內(nèi)瘺血流量有較好作用。結(jié)論：1.趙化南名老中醫(yī)采用辨證用藥局部熱敷促進(jìn)內(nèi)瘺成熟的臨床療效確切,可以提高血液透析患者動(dòng)靜脈內(nèi)瘺吻合術(shù)的成功率。并具有良好的安全性。2.術(shù)前局部辨證多為氣虛血瘀證,采用中藥益氣活血通絡(luò)方局部熱敷,軟化、擴(kuò)張靜脈血管,增加血流量,改善術(shù)前血管條件,可以提高手術(shù)成功率；3.術(shù)后局部辨證多為瘀熱阻絡(luò)證,采用中藥清熱解毒活血通絡(luò)方局部熱敷,減輕術(shù)后血管炎癥滲出、腫脹,減少組織出血、疼痛等并發(fā)癥,并減輕血管內(nèi)膜增生,可以促進(jìn)動(dòng)靜脈內(nèi)瘺術(shù)后發(fā)育成熟。4.手術(shù)前、后采用中藥方局部熱敷均有活血化瘀方藥,可能是通過改善血流變和血流動(dòng)力學(xué)作用而改善手術(shù)血管條件,提高動(dòng)靜脈內(nèi)瘺手術(shù)成功率；其抗血栓抗炎作用可預(yù)防動(dòng)靜脈內(nèi)瘺初期瘺管血栓形成,抑制血管內(nèi)膜炎癥。其具體作用機(jī)制有待于進(jìn)一步實(shí)驗(yàn)研究證實(shí)。
[Abstract]:Objective: To observe the old Chinese national Zhao Huanan on clinical curative effect of TCM drugs to compress venous fistula maturation of hemodialysis patients, and of the traditional Chinese medicine hot compress promoting arteriovenous fistula maturation, improve the dynamic mechanism of blood flow within the venous fistula. Methods: a prospective cohort study method, 60 cases of hemodialysis patients with intravenous action the fistula surgery department of nephropathy of Lianyungang Hospital of Traditional Chinese Medicine were randomly divided into observation group of 30 cases and 30 cases in the control group. All patients with wrist distal radial artery and cephalic vein arteriovenous fistula plasty anastomosis by end to side anastomosis. In the control group to five days of intravenous action fistula before the operation began with a hot towel, second days after the operation began to Hirudoid fistula inunction; observation group in five days to action of internal arteriovenous fistula surgery using Zhao Huanan Yiqi Huoxue Tongluo Decoction compress, surgery Second days after the detoxification Huoxue Tongluo Decoction of local hot compress. Every 30 minutes, two times a day for 2 weeks. Close monitoring of arterial venous fistula, postoperative change of dressing, pay attention to monitoring blood pressure, limb pressure proof. Observation index: fistula patency within 1 months in the movement the first time after surgery; fistula using in January. Postoperative Doppler ultrasound determination of internal fistula blood volume. And record the adverse reaction during the treatment. The application of SPSSl6.0 statistical software for data analysis, measurement data to mean + standard deviation (x + s) said that the 2 groups were compared with t test, counting data were compared with X2 test, P0.05 was the significant difference. Results: the fistula patency 1. in two groups: the observation group in January within fistula patency in 29 cases, 1 cases of embolization; 6 cases in the control group. The specific mechanism needs further experimental studies.
