【摘要】：目的:探討桃紅四物湯對(duì)痰瘀互結(jié)型糖尿病合并動(dòng)脈粥狀硬化患者心率變異性、血脂及C反應(yīng)蛋白(CRP)的影響。方法:收集廣東省中醫(yī)院內(nèi)科收治的痰瘀互結(jié)型糖尿病合并動(dòng)脈粥狀硬化患者120例,隨機(jī)分為治療組和對(duì)照組,各60例。2組患者均給予常規(guī)治療,二甲雙胍0.25 g·d-1,阿卡波糖50 mg·d~(-1)或注射胰島素。對(duì)照組予以消心痛及復(fù)方丹參滴丸,10粒/次,每日1次口服;治療組常規(guī)治療基礎(chǔ)上予以桃紅四物湯加減,200 m L·d~(-1),分早晚2次溫?zé)岷罂诜?每4周為1個(gè)療程。2組均連續(xù)治療4周。治療結(jié)束后,比較患者心率變異性,血脂,血糖,CRP水平及不良反應(yīng)發(fā)生情況。結(jié)果:治療后與對(duì)照組比較,治療組臨床總有效率較高(P0.05)。與治療前比較,兩組患者24 h正常R-R間期的標(biāo)準(zhǔn)差(SDNN),相鄰RR間期差值的均方根(RMSSD),相鄰正常R-R間期差值50 ms比率(PNN50)水平升高(P0.05),與對(duì)照組比較,治療組SDNN,RMSSD,PNN50水平較高(P0.05)。與治療前比較,空腹血糖(FPG),餐后2 h血糖(PBG)以及糖化血紅蛋白(Hb A1c)水平降低(P0.05);治療后與對(duì)照組比較,治療組FPG,PBG以及糖化血紅蛋白水平較低(P0.05)。治療后2組總膽固醇(TC),低密度脂蛋白膽固醇(LDL-C),甘油三酯(TG),CRP水平降低,高密度脂蛋白-膽固醇(HDL-C)升高(P0.05);與對(duì)照組比較,治療組TC,LDL-C,TG水平較低,CRP,HDL-C水平較高(P0.05)。結(jié)論:桃紅四物湯對(duì)痰瘀互結(jié)型糖尿病合并動(dòng)脈粥狀硬化患者的臨床療效顯著,能夠調(diào)節(jié)心率變異性,下調(diào)CRP水平,安全性高,適宜臨床應(yīng)用推廣。
[Abstract]:Objective: to investigate the effect of Taohong Siwu decoction on heart rate variability in patients with phlegm-blood stasis syndrome diabetes complicated with atherosclerosis. Methods: 120 patients with phlegm-blood stasis syndrome diabetes complicated with porridge sclerosis were randomly divided into treatment group and control group. 60 patients in each group were given routine treatment with metformin 0.25 g 路d-1. The control group was given 10 tablets of isosorbide and compound salvia miltiorrhiza drop pills once a day. On the basis of routine treatment, the treatment group was treated with Taohong Siwu decoction plus or minus 200 mL 路dzhuan-1, which was taken orally after warm heat twice in the morning and evening. After the treatment, the patients' heart rate variability, blood lipid, blood glucose and CRP levels and adverse reactions were compared. Results: after the treatment, the patients were compared with the control group. The total clinical effective rate of the treatment group was higher than that of the control group (P 0.05). Compared with before treatment, the standard deviation of the 24 h normal R-R interval in the two groups was SDNN. The RMS SDN of RMS SDN and the ratio of PNN50 of adjacent normal R-R interval (RMS SDN) were higher than those of the control group (P 0.05). The level of RMS SDNN50 in the treatment group was higher than that in the control group (P 0.05). 2 h postprandial blood glucose (PBG) and glycosylated hemoglobin (HbA1c) decreased P0.05a; Compared with the control group, the levels of PBG and glycosylated hemoglobin in the treatment group were lower than those in the control group (P 0.05). Low density lipoprotein cholesterol (LDL-C), triglyceride triglyceride (TGN) and high density lipoprotein cholesterol (HDL-C) increased (P 0.05). Compared with the control group, the level of TG in the treatment group was lower than that in the control group. Conclusion: Taohong Siwu decoction has a significant clinical effect on patients with phlegm-blood stasis type diabetes mellitus complicated with atherosclerosis, which can regulate heart rate variability and down-regulate the level of CRP. It is safe and suitable for clinical application.
