【摘要】：目的觀察隔姜灸臍法對(duì)宮寒性不孕癥患者子宮內(nèi)膜容受性的改善作用,進(jìn)一步了解中醫(yī)外治法在治療不孕癥方面的優(yōu)勢(shì),為臨床治療宮寒性不孕癥提供新的思路和方法,從而提高妊娠率和妊娠成功率。方法按照研究標(biāo)準(zhǔn)將確診為宮寒不孕癥的患者100例,隨機(jī)分為治療組和對(duì)照組各50例。治療組給予外治法隔姜灸臍法和內(nèi)服補(bǔ)腎活血、調(diào)經(jīng)暖宮的中藥(白芍、吳茱萸、桂枝、小茴、法夏、黨參、當(dāng)歸、丹皮、三棱、川芎、丹參、莪術(shù)、菟絲子、五味子)治療,隔姜灸臍法每隔十日一次,中藥一日一劑,水煎服,早晚各1次;對(duì)照組僅給予補(bǔ)腎活血、調(diào)經(jīng)暖宮中藥口服治療,一日一劑,水煎服,早晚各一次。1個(gè)月為1個(gè)療程,治療3個(gè)療程。以三維彩色多普勒超聲診斷儀作為監(jiān)測(cè)手段,檢測(cè)兩組患者治療前后子宮內(nèi)膜厚度(Em)、子宮內(nèi)膜類型、子宮內(nèi)膜容積、子宮動(dòng)脈血流(搏動(dòng)指數(shù)PI、阻力指數(shù)RI)、子宮螺旋動(dòng)脈血流(PI、RI)、子宮內(nèi)膜血流情況,并觀察患者治療前后臨床癥狀和體征改善、妊娠情況,三個(gè)月后將監(jiān)測(cè)所得數(shù)據(jù)運(yùn)用SPSS 19.0統(tǒng)計(jì)分析軟件進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果(1)兩組患者卵泡期和排卵期的子宮內(nèi)膜厚度、內(nèi)膜形態(tài)、內(nèi)膜容積、子宮動(dòng)脈和螺旋動(dòng)脈PI、RI以及子宮內(nèi)膜血流指標(biāo)經(jīng)治療后均得到了不同程度的改善;與對(duì)照組比較,治療組療效更好,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);(2)兩組患者的臨床癥狀和體征經(jīng)治療后均獲得改善,治療組的總有效率高達(dá)94%,與對(duì)照組比較,療效更為顯著,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);(3)兩組患者的妊娠率及妊娠成功率統(tǒng)計(jì)結(jié)果顯示,治療組的妊娠率為74%,妊娠成功率為32%,均高于對(duì)照組,尤其是妊娠率差異顯著,具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論隔姜灸臍法能夠有效改善宮寒性不孕癥的子宮內(nèi)膜容受性,減輕患者的臨床癥狀和體征,提高妊娠率及妊娠成功率。中藥聯(lián)合隔姜灸臍法治療宮寒不孕癥,體現(xiàn)了中醫(yī)內(nèi)外兼治的思想,且療效優(yōu)于單純的中藥內(nèi)服,在不孕癥的治療方面占有很大的優(yōu)勢(shì),此方法值得我們推廣。
[Abstract]:Objective to observe the effect of ginger-separated moxibustion on endometrial receptivity in patients with uterine cold infertility, and to further understand the advantages of external treatment of traditional Chinese medicine in the treatment of infertility. To provide new ideas and methods for clinical treatment of uterine cold infertility, so as to improve the pregnancy rate and pregnancy success rate. Methods 100 cases of uterine cold infertility were diagnosed according to the research standard. The treatment group was randomly divided into treatment group (n = 50) and control group (n = 50). The treatment group was treated with external treatment of ginger separated moxibustion and umbilical moxibustion, internal administration of tonifying kidney and activating blood circulation, regulating meridian and warming palace Chinese medicine (Radix Paeoniae Alba, Evodia Fructus Corni, Guizhi, small anise, Faxia, Codonopsis, Radix. Sanleng, Chuanxiong, Salvia miltiorrhiza, Curcuma, Cuscuta, Schisandra) treatment, ginger-separated moxibustion umbilical method every 10th, Chinese medicine 1st a dose, water decoction, morning and evening, 1; The control group was only given tonifying kidney and activating blood circulation, regulating meridian and warming palace Chinese medicine oral treatment, 1st, one dose, water decoction, once in the morning and evening. 1 month as a course of treatment. Three courses of treatment. Using 3D color Doppler ultrasound as a monitoring method, the endometrial thickness, endometrial type and endometrial volume were measured before and after treatment in two groups. Uterine artery blood flow (pulsatile index Pi, resistance index RIN), uterine spiral artery blood flow (Pi), endometrial blood flow, and observe the improvement of clinical symptoms and signs before and after treatment. Pregnancy data were analyzed by SPSS 19.0 statistical analysis software after 3 months. Results the endometrial thickness and endometrial morphology in follicular phase and ovulation stage were analyzed in both groups. Endometrial volume, PIRI of uterine artery and spiral artery and endometrial blood flow index were improved to some extent after treatment. Compared with the control group, the curative effect of the treatment group was better, the difference was statistically significant (P 0.05). 2) the clinical symptoms and signs of the two groups were improved after treatment, the total effective rate of the treatment group was as high as 94%, compared with the control group, the curative effect was more significant, the difference was statistically significant (P 0.05); The statistical results of pregnancy rate and pregnancy success rate of the two groups showed that the pregnancy rate of the treatment group was 74 and the success rate of pregnancy was 322.It was higher than that of the control group, especially the difference of pregnancy rate was significant. Conclusion Ginger-separated moxibustion with umbilical method can effectively improve the endometrial receptivity of uterine cold infertility and alleviate the clinical symptoms and signs of the patients. Improve the pregnancy rate and pregnancy success rate. Traditional Chinese medicine combined with ginger-separated moxibustion umbilical method for the treatment of uterine cold infertility, reflects the thinking of internal and external treatment of Chinese medicine, and the curative effect is better than the simple Chinese medicine. In the treatment of infertility has a great advantage, this method is worthy of our promotion.
