【摘要】：目的:優(yōu)化一指禪穴位推拿抗眩暈效應(yīng)的動力學(xué)參數(shù),為推拿手法量化、標(biāo)準(zhǔn)化、規(guī)范化提供參考依據(jù)和研究思路。方法:選擇符合納入標(biāo)準(zhǔn)的54例頸性眩暈患者為研究對象,先采用單因素試驗設(shè)計初步篩選出單個因素的較佳水平,再以單因素試驗結(jié)果為基礎(chǔ)按照L9(34)的正交表進行三因素三水平的正交試驗,一指禪穴位推拿施術(shù)于患者雙側(cè)風(fēng)池穴,治療前后記錄患者眩暈癥狀和功能量表的評分值,并以眩暈改善指數(shù)為評價指標(biāo),對一指禪穴位推拿動力學(xué)參數(shù)-力量、頻率、時間進行優(yōu)化研究。經(jīng)過單因素優(yōu)化試驗和正交試驗,優(yōu)化篩選出一指禪穴位推拿抗眩暈效應(yīng)的最佳動力學(xué)參數(shù)組合。結(jié)果:單因素試驗得出的單個因素的較佳水平依次為:壓力16N、時間9mmin、頻率100次/分,正交試驗得出各因素對眩暈改善指數(shù)的影響順序為:A(力量)C(頻率)B(時間),最佳參數(shù)組合為A3B2C2,即力量18.5N,時間9min,頻率100次/分,利用最佳參數(shù)組合的一指禪穴位推拿手法對提高眩暈的改善指數(shù)最為顯著。結(jié)論:動力學(xué)力量、時間、頻率任意水平的匹配組合都可以改善眩暈的癥狀和功能;但不同水平的參數(shù)組合的一指禪穴位推拿手法改善眩暈的程度是不同的,其中以力量18.5N,時間9min,頻率100次/分的動力學(xué)參數(shù)組合為抗眩暈效應(yīng)的最佳組合;盲目的加大一指禪穴位推拿的力度,延長操作時間、加快手法頻率并不能提高手法抗眩暈效應(yīng)。
[Abstract]:Objective: to optimize the dynamic parameters of the anti-vertigo effect of Yichang acupoint massage, and to quantify and standardize the massage manipulation. Methods: 54 patients with cervical vertigo who met the inclusion criteria were selected as the study subjects, and the better level of single factor was preliminarily selected by single factor test design. Based on the results of single factor test, the orthogonal experiment of three factors and three levels was carried out according to the orthogonal table of L9Y34), and the first finger point massage was applied to the patients' bilateral Fengchi acupoints. The scores of vertigo symptom and function scale were recorded before and after treatment, and the improvement index of vertigo was taken as the evaluation index. Time optimization. Matching combinations at any level of frequency can improve the symptoms and functions of vertigo. But the degree of improvement of vertigo was different in different level of parameter combination of one finger massage, in which the strength was 18.5 Ns, time was 9 minutes. The best combination of anti-vertigo effect is the dynamic parameter of frequency 100 times per minute. Blindly increasing the strength of acupoint massage, prolonging the operation time and speeding up the frequency of manipulation can not improve the anti-vertigo effect of manipulation.
