【摘要】：目的:通過究是經過針對150例2型糖尿病臨床病例患者搜集的信息以及相關數據,想要通過本次研究進一步的分析出2型糖尿病視網膜病變與2型糖尿病中醫(yī)辯證型關聯(lián),在此基礎之上能夠深層次的分析出2型糖尿病眼底視網膜其不斷改變的特征,從而再將其與2型糖尿病兩者完成一定的對比探究,通過視網膜眼底病變的進行性改變更好的防治2型糖尿病提供一定的幫助,反之,我們需要糖尿病眼底視網膜辯證分型變化來更好的幫助2型糖尿病中醫(yī)辯證分型和治療。方法:采用回顧性分析的調查的辦法,完成整體性的研究。采集相關指標的全部研究對象來自于2015年1月至2017年1月在大連醫(yī)科大學附屬第二院中醫(yī)病房和內分泌科2型糖尿病患者,按照患者中醫(yī)四診的資料和數據把患者區(qū)別成幾種不同的類型,主要是陰虛熱盛型,氣陰兩虛型,陰陽兩虛型,血瘀脈絡型。并摘錄其詳細的病史記錄情況,包括患者姓名、年齡,病史,眼底病變,查體等等各種輔助檢查信息和內容的采集。把輸入與記載全面的具體的詳細的探析指標在中醫(yī)不同的證型組中來深層次的完成一定的對比和探究,探討其相關性。結果:(1)性別、體重指數、年齡無顯著性差異(P0.05),2型糖尿病的發(fā)病過程中以氣陰兩虛證為主,吸煙史中醫(yī)證型之間有顯著差異,氣陰兩虛證與陰陽兩虛證存在差異明顯(P0.05),其它證型無明顯差異(p0.05)。(2)中醫(yī)各證型組之間病程有顯著性差異(P0.05),病程會隨著證型逐漸加重(3)中醫(yī)各證型組與甘油三醋、膽固醇、低密度脂蛋白基本上無顯著性差異(P0.05),高密度脂蛋白較其他組有明顯差異,陰陽兩虛證和陰虛熱盛兩組之間存在差異(P0.O5),氣陰兩虛證與陰虛熱盛證兩組存在差異(P=0.05),其它之間沒有差異(P0.05)(4)各證型組之間從視網膜眼底變化來看,血管瘤以氣陰兩虛證型最為明顯(P0.05),陰虛熱盛組,陰陽兩虛組與血癖脈絡組無明顯差異(P0.05)。陰陽兩虛組中新生血管及黃斑水腫表現(xiàn)明顯高于其他各組(P0.05,)氣陰兩虛組、陰虛熱盛組與血癖脈絡組無明顯差異(P0.05);玻璃體積血以血瘀阻絡型為主(P0.05),其他無顯著性差異(P0.05);結論:本研究的結果顯示,齡、性別、體重指數無顯著性差異,2型糖尿病的發(fā)病過程中以氣陰兩虛證為主,吸煙會加重糖尿病病情;站在病程的角度來觀察,中醫(yī)辯證分型陰虛熱盛證、氣陰兩虛證、血癖脈絡證陰陽兩虛證的病程是呈逐漸增長的趨勢的。并且隨著疾病的進程,脂代謝紊亂變得更加嚴重;糖尿病視網膜眼底病變中血管瘤以氣陰兩虛證型最為明顯。陰陽兩虛組中新生血管及黃斑水腫表現(xiàn)明顯高于其他各組;玻璃體積血以血瘀阻絡型為主,其它各組無明顯差別;符合中醫(yī)消渴發(fā)病進程,說明糖尿病視網膜眼底病變的發(fā)病率與2型糖尿病中醫(yī)辯證分型關系十分密切,有助于中醫(yī)辯證客觀規(guī)律化;若是迎合中醫(yī)消渴發(fā)病進程,那就意味著糖尿病視網膜眼底病變的發(fā)病率和2型糖尿病中醫(yī)辯證分型有著非常緊密的聯(lián)系,諸多方面有著深層次的影響,可以讓我們能夠更好的理解醫(yī)辯證客觀規(guī)律賦予一定的幫助;在治療2型糖尿病的過程中,我們需要主動的完成監(jiān)測工作,想要更好的掌握疾病的進程,隨之我們需要盡快和及時的完成一定的預防工作,由此一來,能夠比較全面和有效合理的延遲疾病的發(fā)展情形,并能減少疾病的惡化。
[Abstract]:Objective: To study the information collected after 150 cases of patients with type 2 diabetes clinical cases and relevant data, through this research to further analysis of type 2 diabetic retinopathy in patients with type 2 diabetes mellitus TCM Association, based on deeply analysis of the changing of fundus type 2 diabetic retinopathy thus, the type 2 diabetes mellitus and explore both complete certain changes in contrast, better prevention and treatment of type 2 diabetes to provide some help, but by retinopathy, we need diabetes retinal dialectical type changes to help the Chinese type 2 diabetes diagnosis and treatment. Methods: the retrospective investigation and analysis to complete the overall study. Collecting related index of all subjects from January 2015 to January 2017 in Dalian hospital Department of the Second Affiliated Hospital of Chinese medicine university affiliated ward and Department of Endocrinology patients with type 2 diabetes, the difference between patients into several different types of patients according to TCM data, mainly exuberant heat due to yin deficiency, deficiency of Qi and Yin, yin and yang two deficiency, blood stasis syndrome and its extract. Detailed records, including patient name, age history, fundus examination, auxiliary examination and so on all kinds of information and content acquisition. The input and record the specific comprehensive detailed analysis of the indicators to deep in syndrome type TCM group in different ratio and to complete certain inquiry, to explore their correlation. Results: (1) gender, body mass index.
