【摘要】：[研究背景]川派中醫(yī)長(zhǎng)期以來在中醫(yī)藥界占有舉足輕重的地位,為更好地傳承中醫(yī)藥文化發(fā)展,2012年四川省中醫(yī)藥管理局組織和開展了川派中醫(yī)藥名家學(xué)術(shù)思想及臨床經(jīng)驗(yàn)研究項(xiàng)目。曾敬光(1918-2010),女,四川省雙流縣人,成都中醫(yī)藥大學(xué)教授。長(zhǎng)年耕耘在教學(xué)、臨床第一線,多次承擔(dān)教學(xué)和教材編寫工作。其在中醫(yī)婦科方面具有獨(dú)到見解。[研究目的]1整理、總結(jié)曾敬光教授的學(xué)術(shù)思想及臨床經(jīng)驗(yàn),提煉其臨床辨證、辨病特點(diǎn)及擬方潛藥規(guī)律。2回顧性分析124例妊娠惡阻入院患者臨床資料,探索妊娠惡阻中醫(yī)證型分布及臨床用藥規(guī)律。[研究方法]1通過圖書館、互聯(lián)網(wǎng)、數(shù)據(jù)庫、名醫(yī)及曾敬光教授家眷典藏等途徑挖掘有關(guān)曾敬光教授的論文、著作、醫(yī)案等資料,歸總曾敬光教授的學(xué)術(shù)思想、臨證經(jīng)驗(yàn)。2通過回顧性的臨床研究,搜集2012年6月到2016年01月自電子病歷系統(tǒng)使用以來成都中醫(yī)大學(xué)附屬醫(yī)院婦科124例妊娠惡阻患者原始資料,將上述資料填入“妊娠惡阻臨床調(diào)查表”中,并輸入EXCEL軟件進(jìn)行原始數(shù)據(jù)分類整理,運(yùn)用統(tǒng)計(jì)軟件包SPSS17.0,進(jìn)行計(jì)數(shù)資料分析、聚類分析、因子分析及相關(guān)性分析。[研究結(jié)果及結(jié)論]1學(xué)術(shù)思想研究結(jié)果及結(jié)論(1)強(qiáng)調(diào)“沖任損傷是婦科疾病的基本病機(jī)和最終病位”,從沖任不固、不盛、失調(diào)、阻滯四證統(tǒng)率月經(jīng)不調(diào)諸癥。(2)主張“既要治病,又要安胎”的妊娠病治療原則,提出惡阻轉(zhuǎn)歸三期,強(qiáng)調(diào)從腎氣、沖任、氣血論治胎漏、胎動(dòng)不安,從脾虛肝郁論治妊娠腫脹等。(3)擅“五證為綱”論治不孕癥。2妊娠惡阻臨床回顧性分析研究結(jié)論(1)妊娠惡阻為婦女妊娠期多發(fā)病、常見病,臨床病情嚴(yán)重程度以中度多見,中醫(yī)臨床分型以脾胃虛弱型較多,其次為肝胃不和型。(2)本研究治療妊娠惡阻在顧護(hù)脾胃的基礎(chǔ)上強(qiáng)調(diào)脾胃、肺、肝同治,兼以補(bǔ)腎。用藥以補(bǔ)益藥為主,止咳化痰平喘藥、理氣藥次之,甘溫并用,苦辛并用,寒熱同調(diào),結(jié)果表明中西醫(yī)結(jié)合能明顯改善惡心、嘔吐、厭食,疲倦乏力等一系列臨床證候,具有較好療效,即中西醫(yī)結(jié)合治療該病癥有一定的臨床指導(dǎo)意義。(3)本研究的設(shè)計(jì)啟發(fā)于對(duì)曾敬光教授關(guān)于妊娠惡阻學(xué)術(shù)思想及臨床經(jīng)驗(yàn)的研究,該研究結(jié)果與曾敬光教授治療經(jīng)驗(yàn)存在一定程度的一致性,推論曾敬光教授治療該病的臨床經(jīng)驗(yàn)有很強(qiáng)的實(shí)用價(jià)值。(4)本課題僅對(duì)本次研究結(jié)果分析,研究中患者存在復(fù)發(fā)可能,未行隨訪,在日后的研究工作中可加強(qiáng)跟蹤研究。
[Abstract]:[Research background] Chuan School TCM has been playing an important role in the field of traditional Chinese medicine for a long time, in order to better inherit the development of traditional Chinese medicine culture. In 2012, the Sichuan Provincial Administration of traditional Chinese Medicine organized and carried out a research project on the academic thought and clinical experience of Sichuan School of traditional Chinese Medicine famous scholars. [Objective: 1 to summarize Professor Zeng Jingguang's academic thought and clinical experience, and to analyze the clinical data of 124 patients with pregnancy obstruction. To explore the distribution of TCM syndromes and the regularity of drug use in pregnancy. Clinical experience .2 through retrospective clinical studies. From June 2012 to January 2016, we collected the original data of 124 pregnant women with adverse pregnancy since the electronic medical record system was used in the department of gynecology of affiliated hospital of Chengdu University of traditional Chinese Medicine. The main medication is tonifying medicine, cough, phlegm and asthma medicine, Qi-regulating medicine, sweet and warm combined, bitter combined, cold and heat homology, the results show that integrated Chinese and Western medicine can significantly improve nausea, vomiting, anorexia. A series of clinical symptoms such as fatigue and fatigue have good curative effect. The design of this study is inspired by Professor Zeng Jingguang's academic thought and clinical experience on pregnancy obstruction. The results of this study are consistent with Professor Zeng Jingguang's treatment experience to a certain extent. It is concluded that Professor Zeng Jingguang's clinical experience in the treatment of the disease is of great practical value. Patients in the study had the possibility of recurrence and were not followed up, and the follow-up study could be strengthened in the future research work.
