【摘要】：目的:觀察中藥補肺養(yǎng)心湯治療心肺氣虛兼血瘀型冠心病心力衰竭的臨床療效,探討中藥能夠改善心力衰竭患者的臨床癥狀及生活質(zhì)量。材料與方法:選入心肺氣虛兼血瘀型冠心病心力衰竭患者60例,隨機分成對照組和治療組各30例。對照組予以冠心病心力衰竭西醫(yī)標準化治療,治療組在此基礎上加服中藥補肺養(yǎng)心湯。連續(xù)治療3個月,比較兩組患者治療前后的相關指標,包括心功能分級(NYHA)、血漿BNP濃度、左室射血分數(shù)值(LVEF)、6分鐘步行試驗(6MWT)、中醫(yī)癥狀積分及生活質(zhì)量積分等,觀察補肺養(yǎng)心湯的臨床療效。結果:1.兩組患者治療后心功能分級均有所改善,且治療組療效優(yōu)于對照組(P0.01)。2.兩組患者治療后血漿BNP濃度值均下降,且治療組療效優(yōu)于對照組(P0.05)。3.兩組患者治療后左室射血分數(shù)值(LVEF)均有所增加,但兩組間比較,二者差異無統(tǒng)計學意義(P0.05)。4.兩組患者治療后6MWT距離均有所增加,且治療組療效優(yōu)于對照組(P0.01)。5.兩組患者治療后中醫(yī)癥狀積分均有所降低(癥狀好轉(zhuǎn)),且治療組療效優(yōu)于對照組(P0.05)。6.兩組患者治療后生活質(zhì)量積分均有所降低,且治療組療效優(yōu)于對照組(P0.01)。結論:1.補肺養(yǎng)心湯能夠改善冠心病心力衰竭(IHF)患者的心功能,提高6MWT距離,降低血漿BNP濃度。2.補肺養(yǎng)心湯能夠改善IHF患者的臨床癥狀,提高IHF患者的生活質(zhì)量。補肺養(yǎng)心湯能夠提高IHF患者的LVEF值水平,但與對照組比較無顯著性差異(P0.05)。
[Abstract]:Objective: to observe the clinical effect of traditional Chinese medicine Bufei Yangxin decoction on heart and lung qi deficiency and blood stasis type coronary heart failure. To explore the clinical symptoms and quality of life of patients with heart failure improved by Chinese medicine. Materials and methods: 60 cases of coronary heart failure with heart and lung qi deficiency and blood stasis were selected. Control group and treatment group were randomly divided into control group and treatment group of 30 cases. The control group was treated with western medicine standardized treatment of coronary heart failure, the treatment group on this basis plus Chinese medicine Bufei Yangxin decoction, continuous treatment for 3 months. The relative indexes before and after treatment were compared, including cardiac function grade, plasma BNP concentration, left ventricular ejection fraction (LVEF) and 6-minute walking test (6MWTT). TCM symptom score and quality of life integral to observe the clinical efficacy of Bufei Yangxin decoction. Results the two groups of patients after treatment improved cardiac function grading. The therapeutic effect of the treatment group was better than that of the control group (P 0.01). The plasma BNP concentration in the two groups decreased after treatment. The therapeutic effect of the treatment group was better than that of the control group (P 0.05. 3). The left ventricular ejection fraction (LVEF) of the two groups increased after treatment, but there was a comparison between the two groups. There was no significant difference between the two groups. The distance of 6 MWT increased after treatment in both groups. The curative effect of the treatment group was better than that of the control group (P 0.01). The scores of TCM symptoms in the two groups were decreased after treatment (the symptoms were improved). The curative effect of the treatment group was better than that of the control group (P 0.05N. 6). The quality of life score of the two groups decreased after treatment. The therapeutic effect of the treatment group was better than that of the control group (P 0.01). Conclusion: BFYXD can improve the heart function of patients with coronary heart failure and improve the distance of 6MWT. Reduce plasma BNP concentration .2.Bufei Yangxin decoction can improve the clinical symptoms of IHF patients, improve the quality of life of IHF patients. Bufei Yangxin decoction can improve the LVEF value of IHF patients. But there was no significant difference between the control group and the control group (P 0.05).
