【摘要】：目的:通過本項(xiàng)研究,證明補(bǔ)腎安胎方治療腎虛型先兆流產(chǎn)安全有效。并證實(shí)血清E2預(yù)測早期先兆流產(chǎn)妊娠結(jié)局比血清P、β-HCG更有參考價(jià)值。方法:將腎虛型早期先兆流產(chǎn)患者隨機(jī)分為中西藥組和西藥組各30例,西藥組肌注黃體酮,40mg/次,1次/日;中西藥組肌注黃體酮同時(shí)口服中藥湯劑補(bǔ)腎安胎方。至少治療14天,見胎心或胎兒基本成形后方可停藥。觀察兩組患者治療前后的臨床癥狀、實(shí)驗(yàn)室檢查結(jié)果,進(jìn)行統(tǒng)計(jì)分析。結(jié)果:1)中西藥組臨床綜合療效與西藥組比較,總有效率為93.3%vs 83.3%,有顯著差異(P0.05)。2)兩組治療前與治療后中醫(yī)癥狀積分比較均有明顯差異(P0.05)。中西藥組與西藥組中醫(yī)癥狀積分的治療前后的差值相比,有顯著性差異(P0.05)。3)兩組B超所見胎心、胎兒成形情況,治療后與治療前相比,都有顯著差異(P0.05),兩組間治療后的B超所見胎心及胎兒成形比較無顯著差異(P0.05)。4)兩組血清E2、P、β-HCG均明顯增高,與治療前相比有顯著性差異(P0.05);兩組治療后的血清E2、β-HCG與治療前E2、β-HCG的差值相比,有顯著差異(P0.05),P差值比較無明顯差異(P0.05)。但中西藥組的差值略大于西藥組。5)孕酮偏低的先兆流產(chǎn)患者中,最終保胎失敗組與保胎成功組相比較,治療前的E2值有顯著差異(P0.05);P、β-HCG值無明顯差異(P0.05)。結(jié)論:補(bǔ)腎安胎方治療腎虛型早期先兆流產(chǎn)安全有效,明顯減輕患者的臨床癥狀,改善血清性激素水平,促進(jìn)胚胎發(fā)育。血清E2水平評價(jià)臨床療效及預(yù)測早期先兆流產(chǎn)妊娠結(jié)局均比血清P、β-HCG更有參考價(jià)值。
[Abstract]:Objective: to prove the safety and effectiveness of Bushen Antii recipe in the treatment of threatened abortion of kidney deficiency type, and to confirm that serum E2 is more effective than serum P in predicting pregnancy outcome of early threatened abortion. Methods: patients with early threatened abortion of kidney deficiency type were randomly divided into Chinese medicine group and western medicine group (30 cases each). 40 mg / day of progesterone was injected intramuscularly in the western medicine group. Chinese and western medicine group intramuscular injection of progesterone and oral Chinese medicine decoction Bushen Anti prescription. At least 14 days treatment, see fetal heart or fetal basic shaping before and after treatment before and after treatment before and after the observation of the two groups of patients' clinical symptoms. Results compared with the western medicine group, the total effective rate of the Chinese and western medicine group was 93. 3 vs 83.3%. There was significant difference between the two groups before and after the treatment of TCM symptom scores were significantly different (P0.05). The difference of TCM symptom score between Chinese medicine group and western medicine group was compared before and after treatment. There were significant differences in fetal heart and fetal shape between the two groups after treatment and before treatment (P 0.05). There was no significant difference in fetal heart and fetal shape between the two groups after treatment (P0.05. 4) the serum levels of E2P and 尾 -HCG in the two groups were significantly higher than those in the control group. There was significant difference between before treatment and before treatment (P 0.05). The difference of serum E _ 2, 尾 -HCG between the two groups after treatment was significantly different from that before treatment (P 0.05). There was no significant difference in P difference between the two groups. However, the difference between the Chinese and western medicine group was slightly larger than that in the western medicine group. 5) in the threatened abortion patients with low progesterone, the final failure group was compared with the successful group. There was significant difference in E _ 2 value before treatment (P 0.05). Conclusion: Bushen Antii prescription is safe and effective in the treatment of early threatened abortion of kidney deficiency type. It can obviously alleviate the clinical symptoms of patients and improve the level of serum sex hormone. Serum E _ 2 level is more valuable than serum P, 尾 -HCG in evaluating clinical efficacy and predicting pregnancy outcome of early threatened abortion.
