【摘要】：目的:通過(guò)臨床研究,評(píng)價(jià)藤菔降壓片治療高血壓病肝陽(yáng)上亢證的有效性和安全性,并通過(guò)體外實(shí)驗(yàn)探究藤菔降壓片有效組分對(duì)血管內(nèi)皮細(xì)胞自噬水平的影響,從而揭示其保護(hù)血管內(nèi)皮細(xì)胞的作用機(jī)理。方法:臨床研究:將60例符合納入要求的高血壓病肝陽(yáng)上亢證患者,隨機(jī)分為試驗(yàn)組和對(duì)照組,試驗(yàn)組給予藤菔降壓片口服,對(duì)照組給予牛黃降壓丸口服,治療療程為4周,觀察治療前后血壓及癥狀改善情況,評(píng)價(jià)其安全性。體外實(shí)驗(yàn):采用組織貼塊法,原代培養(yǎng)血管內(nèi)皮細(xì)胞,通過(guò)氧化低密度脂蛋白的氧化應(yīng)激反應(yīng),誘導(dǎo)對(duì)數(shù)生長(zhǎng)期的血管內(nèi)皮細(xì)胞發(fā)生自噬,并進(jìn)行藥物干預(yù),觀察其對(duì)自噬水平的影響及對(duì)細(xì)胞的保護(hù)作用。結(jié)果:臨床研究:患者血壓較治療前下降明顯,中醫(yī)證候積分明顯改善,均具有統(tǒng)計(jì)學(xué)意義(p0.05)。兩組治療過(guò)程中均未出現(xiàn)明顯不良反應(yīng)、過(guò)敏反應(yīng)等危險(xiǎn)事件。體外實(shí)驗(yàn):鉤藤總生物堿和芥子堿硫氰酸鹽能夠明顯上調(diào)氧化低密度脂蛋白誘導(dǎo)的血管內(nèi)皮細(xì)胞自噬水平,減輕氧化應(yīng)激的損害作用。結(jié)論:中藥復(fù)方藤菔降壓片治療高血壓病肝陽(yáng)上亢證患者具有明顯的療效,能夠有效地控制血壓并改善患者臨床癥狀,并且安全性高、未見(jiàn)不良反應(yīng)。其作用機(jī)制可能與鉤藤總生物堿和芥子堿硫氰酸鹽通過(guò)上調(diào)血管內(nèi)皮細(xì)胞的自噬水平以保護(hù)血管內(nèi)皮細(xì)胞免受ox-LDL損傷有關(guān)。
[Abstract]:Objective: to evaluate the efficacy and safety of sulforaphane in the treatment of hyperactivity of liver-yang in patients with essential hypertension, and to explore the effect of effective components of sulforaphane on the level of autophagy of vascular endothelial cells in vitro. Methods: clinical study: 60 patients with hyperactivity of liver yang were randomly divided into experimental group and control group. The experimental group was treated with sulforaphane tablets and the control group with Niuhuang Jiangya pills for 4 weeks. The blood pressure and symptom improvement were observed before and after treatment and the safety was evaluated. In vitro experiment: tissue patch method was used. Primary cultured vascular endothelial cells (VECs) were induced to autophagy in logarithmic growth phase by oxidative stress reaction of oxidized low density lipoprotein (LDL) and drug intervention was carried out. To observe the effect on autophagy level and its protective effect on cells. Results: clinical study: the blood pressure of patients was significantly lower than that before treatment, and the integral of TCM syndromes was obviously improved. There was no significant adverse reaction in both groups. In vitro experiments: total alkaloids and erucidine thiocyanate could significantly upregulate the level of autophagy induced by oxidized low density lipoprotein (LDL). Conclusion: traditional Chinese medicine compound sulforaphane is effective in treating patients with hyperactivity of liver yang in hypertension. It can effectively control blood pressure and improve the clinical symptoms of patients with hyperactivity of liver yang. And safe. The mechanism may be related to the up-regulation of autophagy of vascular endothelial cells by total alkaloids and erucidine thiocyanates in order to protect vascular endothelial cells from ox-LDL damage.
